TIRE FAILURE MAY BE RELATED TO INVESTIGATION PE00-020. MAJOR TEAR OCCURRED AT SIDEWALL/TREAD INTERFACE.( DOT NUMBER: TIRE SIZE: P235/75R15 )
WHILE DRIVING MY EXPLORER AT HIGHWAY SPEEDS, I HEARD A NOISE AS IF I HAD KICKED UP A ROCK. CHECKING THE DRIVERS SIDE MIRROR, I NOTICED A BLACK OBJECT BEING THROWN TO THE SIDE OF THE ROAD. I PULLED OVER TO STOP MY VEHICLE AND INSPECT IT. I FOUND A SECTION OF THE REAR DRIVERS SIDE TIRE HAD COME OFF. THE SECTION WAS ABOUT 3 INCHES IN DIAMETER AND WAS DOWN TO THE STEEL BELTS. THE TIRE WAS REPLACED BY FIRESTONE, (PRO-RATED), ON JULY 1ST, 2000. I JUST HAD ALL TIRES INSPECTED BY FIRESTONE AND THEY WANT TO REPLACE ALL 5 TIRES, (THIS INCLUDE THE ONE I JUST RECEIVED JULY 1ST.( DOT NUMBER: N/A TIRE SIZE: P235/75R15 )
TIRE BLEW OUT AND CAUSED VEHICLE TO SWERVE RIGHT AND TO COLLIDE WITH THE REAR TRAILER OF AN 18 WHEEL TRACTOR-TRAILER, AND THEN VEHICLE PROCEEDED OFF ROAD ACROSS SHOULDER OF HIGHWAY AND THROUGH THE ADJACENT FENCE. VEHICLE RECEIVED EXTENSIVE DAMAGE AND BOTH OCCUPANTS WERE INJURED AND REQUIRED MEDICAL TREATMENT.( DOT NUMBER: VDHL TIRE SIZE: P235/75R15 )
CONSUMER REPLACED FITESTONE ATX TIRE PRIOR TO RECALL NOTICE AND REQUEST REIMBURSEMENT BUT WAS TOLD HE HAD NO RECOURSE WITHOUT THE OLD TIRES. NLM
THE CONSUMER HAS FIRESTONE WILDERNESS AT TIRES (P235/75R15), THESE TIRES ARE NOT INCLUDED IN THE RECALL, HOWEVER ON 2 OCCASIONS THE TIRES EXPERIENCED BLOWOUTS WHERE THE TREAD PEELED FROM THE TIRES, THE CONSUMER PURCHASED NEW TIRES AND IS REQUESTING REIMBURSEMENT. *SLC
ON 6/18/2000 OUR 1998 FORD EXPLORER WITH FIRESTONE BLOW-OUT OR A TREAD PROBLEWILDERNESS AT TIRES WENT OUT OF CONTROL AFTER MAKING A SHARP STEERING MANEUVER TO AVOID ANOTHER CAR. OUR VEHICLE SUBSEQUENTLY ROLLED OVER (WE WERE TOLD) SIX TIMES. WE DO NOT BELIEVE THERE WAS A TIRE M, BUT WHAT WE SUSPECT HAPPENED IS DEBEADING OF THE TIRES. WE HAVE READ YOUR 130 PAGE REPORT FROM JULY 1999 ENTITLED "AN EXPERIMENTAL EXAMINATION OF SELECTED MANEUVERS THAT MAY INDUCE ON-ROAD UNTRIPPED, LIGHT VEHICLE ROLLOVERS - PHASE II OF NHTSA'S 1997-1998 VEHICLE ROLLOVER RESEARCH PROGRAM". WE ARE CONCERNED THAT WITH THE AMOUNT OF EMPHASIS CURRENTLY BEING PLACED ON THE BLOW-OUT/TREAD PROBLEM, THAT THIS OTHER PROBLEM OF TIRE DEBEADING IS BEING IGNORED. WE HAVE PICTURES OF OUR VEHICLE FROM AFTER THE ACCIDENT THAT SHOW THE TIRES SEPARATED FROM THE RIMS (DEBEADED). WE WOULD BE EAGER TO DISCUSS OUR ROLLOVER ACCIDENT FURTHER IN AN ATTEMPT TO HELP PREVENT FURTHER HORRIBLE ACCIDENTS OF THIS NATURE. IF YOU HAVE ANY FURTHER INFORMATION THAT WILL HELP US UNDERSTAND THE NATURE OF OUR ACCIDENT , WE WOULD BE VERY APPRECIATIVE. AFTER TAKING MY EXPLORER IN TO THE FIRESTONE DEALER IN MESA, HE REPLACED THE 2 REAR TIRES AND SPARE. HE INSPECTED THE OTHER TIRES AND SAID THEY WERE FINE. HE PLACED THE FRONT TIRES ONTO THE REAR AND PLACED THE NEW TIRES ON THE FRONT. 3000 MILES LATER W( DOT NUMBER: VDHL1PY068 TIRE SIZE: P235/75R15 )
DURING ROUTINE MAINTENANCE TIRE ROTATION, TECHNICIAN FOUND WIRE SHOWING THROUGH TIRES, OWNER REPLACED ALL 4 FIRESTONE WILDERNESS AT P23575R15, DOT # WZHL, OWNER WOULD LIKE TO BE REIMIBURESED IF ABOVE TIRES ARE PART OF RECALL (00T-005). CJ
WHEN USING THE LIGHT SIGNAL AT TIMES IT WON'T WORK. TECHNICIAN EXPLAINED THAT IN SOME PLACES SIGNAL WILL NOT WORK. *AK

PE00020; TIRE TREAD SEPARATION: WHILE DRIVING 73 MPH TREAD ON THE PASSENGER'S SIDE REAR TIRE CAME OFF. THIS CAUSED EXPLORER TO PULL TO THE RIGHT AND FLIP OVER. DRIVER WAS ALONE IN THE EXPLORER, AND SUSTAINED INJURIES TO HEAD,/BACK AND KNEE. TIRE WAS A FIRESTONE, ATX, NOT ORIGINAL EQUIPMENT. TIRES WERE PLACED ON VEHICLE BY FIRESTONE DEALERSHIP. WHEN I TRIED TO PUSH THE CONTROL ON THE END OF THE WINDSHIELD WASHER STALK TO ACTIVATE WASHER, THE FLUID DIDN;T COME OUT EVEN WHEN THE RESERVOIR WAS FULL. THE WIPERS STARTED WORKING BUT NO WASHER FLUID EJECTED LIKE IT IS SUPPOSED TO. *AK
THIS TIRE "SEPARATED" WHILE TRAVELING ON I 37 FROM CORPUS CHRISTI, TX TO SAN ANTONIO, TX. THERE WAS A STRONG VIBRATION AND AN EXTREME PULL TO THE RIGHT WHILE TRAVELING AT HIGHWAY SPEED. UPON ARRIVAL IN SAN ANTONIO, WE SOUGHT OUT A FIRESTONE DEALERSHIP AND STOPPED AT FIRESTONE TIRE & SERVICE CENTER AT 4906 WALZAM RD. I APPRISED THE TECHNICIAN OF THE SYMPTOMS AND HE ALREADY KNEW WHAT I WAS DESCRIBING. HE ADVISED THAT THE TIRE HAD SEPARATED BUT THAT THEY WOULD NEED TO CHECK IT FOR SURE. ON THIS TIRE, IT HAD "SEPARATED" BUT HAD NOT COME APART LIKE THE ONE WE LATER EXPERIENCED THAT I HAVE ALREADY NOTIFIED YOU. IT OCCURED ON 7/24/2000 AND CAUSED AN ESTIMATED $2,800.00 DAMAGE TO THE LEFT REAR QUARTER PANEL OF MY VEHICLE. THE PURPOSE OF THIS NOTIFICATION IS TO ADVISE YOU OF A TIRE THAT WAS NOT PART OF THE RECALL "VD......" TIRES ALSO FAILED AND THAT THERE MAY BE A DESIGN FLAW IN THE TIRES THEMSELVES. THE SERIAL NUMBER ON THIS TIRE WAS 8XHL1PY0300. ANOTHER TROUBLING ISSUE IS THE FAMILARITY OF THE TECHNICIAN WITH THE PROBLEM EVEN BEFORE HE HAD TO CHECK IT. HOW MANY OTHER SIMILAR TIRES HAD HE SEEN WITH THIS SAME PROBLEM? HAD FIRESTONE KNOWN OF THESE PROBLEMS PRIOR TO THE US RECALL?( DOT NUMBER: TIRE SIZE: 23575R15 )
THIS IS IN RESPONSE TO THE RECALL OF THE FIRESTONE TIRES. I'M VERY CONCERNED ABOUT THE FACT THAT MY TIRES WERE NOT INCLUDED IN THE FIRESTONE RECALL. I BOUGHT MY EXPORER NEW IN '98 AND AS I REPORTED IN THIS FORM I HAVE HAD TWO BLOW OUTS WITHIN 6 MONTHS. FORTUNATLY NEITHER ONE CAUSED AN ACCIDENT BUT BOTH TIMES THE TIRES WERE UNREPAIRABLE. I'M ALSO VERY CONSCIOUS ABOUT VEHICLE MAINTAINENCE AND FOLLOW ALL THE MANUFACTURES RECOMMENDATIONS. WE HAD A BLOWOUT ON JUNE 10,2000, BUT THE TIRE WAS NEW (11/22/99). IT TORE THE BACK PANEL OF THE TRUCK. WE WERE DRIVING ON THE FREEWAY AT ABOUT 65-70 MILES AN HOUR. ON 06/01/00 WHILE HEADING TO A FAMILY REUNION WE "HYDROPLANED" ON THE INTERSTATE JUST OUTSIDE OF LAS VEGAS, NM. THE VEHICLE STARTED TO "FISHTAIL" FROM THE BACK TIRES (APPROX 5-6 TIMES), THEN IT SWERVED SIDEWAYS OFF THE ROAD (RIGHT SIDE LANE) AND ONTO THE MEDIAN. WHEN THE VEHICLE CAME UP ONTO THE ONCOMING TRAFFIC SIDE IT "POPPED UP" ONTO TWO WHEELS, CAME BACK DOWN, AND THEN POPPED UP AGAIN AND ROLLED. WE WERE DRIVING ON HWY. 50 EAST IN SACRAMENTO, CA WHEN WE HEARD A NOISE, THEN THE CAR STARTED VIBRATING AND WEAVING. WE PULLED OVER ON THE SIDE OF THE ROAD AND DISCOVERED THAT THE TIRE HAD BLOWN. TOOK THE CAR TO A FIRESTONE DEALERSHIP AND HAD THE TIRE CHANGED TO THE SAME EXACT TIRE. TRAVELING 20MPH, HAD NO PRIOR WARNING;, AND LOST POWER, JUST STOPPED. A FEW SECONDS LATER, IT WOULD CATCH AGAIN. ALSO, WHEN ACCELERATING OR TURNING CORNERS, VEHICLE WOULD LOSE POWER. PROBLEM WAS INTERMITTENT. TOOK VEHICLE TO DEALERSHIP & MECHANIC INDICATED IT WAS PCM RELATED. PROBLEM MAY BE SOLVED*AK. THE DEALER COULD NOT SOLVE THE PROBLEM. *YH

REAR WIPER MOTOR FAILED AT APPROXIMATELY 25,000 MILES. REPLACEMENT MOTOR FAILED IN LESS THAN TWO WEEKS. DEALER ACKNOWLEDGED THAT THERE IS A KNOWN PROBLEM WITH EXPLORER REAR WIPER MOTOR FAILURES BUT STATED THAT FORD HAS NOT ACKNOWLEDGED THE PROBLEM NOR HAVE THEY MADE A REDESIGNED WIPER MOTOR AVAILABLE.*AK
AT HIGHWAY SPEEDS LUGGAGE RACK SEPARATED FROM THE ROOF OF VEHICLE, CAUSING DAMAGE TO THE VEHICLE. *AK
AFTER PARKING VEHICLE FOR 3.5 HOURS ENGINE COMPARTMENT BROKE OUT INTO A FIRE. THE FIRE WAS DETERMINED TO BE AN ELECTRICAL SHORT. DEALER HAS BEEN CONTACTED. *AK *ML
FORD SAID THAT THEY WERE NOT INTERESTED IN LOOKING AT THE VEHICLE TO DETERMINE WHAT THE CAUSE WAS. THE ENGINE FIRE OCCURRED 3 1/2 HOURS AFTER THE VEHICLE WAS PARKED. IT WAS PARKED IN THE STREET, BUT IT IS NORMALLY IN MY GARAGE. IF IT HAD BEEN IN THE GARAGE, MY HOUSE WOULD HAVE CAUGHT FIRE AND MY FAMILY COULD HAVE BEEN KILLED. I WILL NEVER FEEL CONFORTABLE WITH IT IN THE GARAGE AGAIN. IS THERE SOMETHING THAT YOU CAN DO ?*AK
HAVE RECEIVED TWO MFR'S RECALLS AND UNABLE TO GET HONOR; 99 V 164 000/SECONDARY HOOD LATCH & 99 V 062 001/ CRUISE /SPEED CONTROL CABLE. DEALER STATING PARTS ARE NOT AVAILABLE. PLEASE PROVIDE ANY FURTHER DETAILS:
I WAS INVOLVED IN A FATAL CAR ACCIDENT. I WAS THE DRIVER HEADING WEST ON THE 202 FREEWAY IN PHOENIX ARIZONA. I MERGED INTO THE 10 FREEWAY NEAR 16TH STREET WHEN MY REAR PASSENGER TIRE TREAD PEELED OFF. THIS CAUSED ME TO LOSE CONTROL OF MY FORD EXPLORER VEHICLE. WITH THE LOST OF CONTROL THE VEHICLE ROLLED ABOUT 4 TO 5 TIMES. CAUSING THE DEATH OF MY FRIEND (PASSENGER) AND MY YORKSHIRE TERRIER DOG ROLLED OUT AND RAN UNABLE TO BE FOUND. FOLLOWING THE P235/75/R15 THERE ARE SEVERAL OTHER NUMBERS, AS FOLLOWS 105 S M/S TIRE SIDE WALL BLEW OUT COMPLETELY AROUND THE CIRCUMFERENCE OF THE TIRE. I HAD TIRE REPLACED AT FIRESTONE TIRE AND SERVICE CENTER, 900 E KENNEDY BLVD, TAMPA, CUSTOMER INVOICE #007059 DATED 06/08/2000. LOCAL NEWS COVERAGE SAID Y'ALL WERE INVESTIGATING THE TIRE, THOUGHT THIS INCIDENT MIGHT HELP. TIRE BLEW OUT ON I-75 EAST OF NAPLES, FL (ALLIGATOR ALLEY). I WAS DRIVING ON A WEEKEND VACATION WITH THE CAR LOADED WITH LUGGAGE AND 3 CHILDREN AND 2 ADULTS. I WAS TRAVELING ABOUT 80 MPH, IT WAS ABOUT 95 DEGREES OUT. I WAS IN THE FAR LEFT LANE WHEN A BLOW-OUT OCCURED. I TOOK MY FOOT OFF THE ACCELLERATOR AND COASTED TO THE SIDE OF THE ROAD. LUCKILY NOBODY WAS INJURED. INS WAS TRAVELING SOUTH BOUND ON I-95 AND REAR PASSENGER TIRE BLEW OUT, WHICH WAS CAUSED BY A METAL PIECE IN ROAD. INS SAID THAT THE VEH FLIPPED ABOUT SIX TIMES. MILE, SMOKE EVERYWHERE FROM BRAKES, GOT DOWN TO 50MPH, PUT CAR IN NEUTRAL, WAS AT LEAST 1/4 T/ 1/2 MILE WITH RIDING BRAKES BEFORE CAR STOPPED, HAD IT HAULED TO DEALERSHIP, WAS INFORMED THIS HAD NEVER HAPPENED BEFORE, AM NOW AFRAID TO DRIVE CAR WHEN IT IS REPAIRED, IT WAS IN THE DEALERSHIP IN 99 FOR THE CRUISE CONTROL RECALL, THEY CLAIM THEY HAVE SENT THE PARTS IN TO THE FORD LABORATORY, THEY STILL DO NOT KNOW WHAT WENT WRONG, SAID EITHER THE CABLE OR THE MODULE. *AK

I HAD REPLACED 3 WILDERNESS AT TIRES WITH 3 WILDERNESS AT TIRES BY THE TIME THE 3RD ONE BLEW OUT AND THIS TIME I ENDED UP IN A LARGE DITCH AFTER ROLLING 1 AND 1/HALF TIMES. I COMPLAINED TO FIRESTONE AND FORD BOTH SINCE I BOUGHT THE NEW TIRES AT BOTH PLACES. THEY BLAMED EACH OTHER. I TRIED TO TELL THEM THAT THE TIRES WERE NOT WEARING RIGHT, VERY DIFFERENT THAN ANY OTHER TIRES I'VE EVER OWNED, BUT THEY WOULD NOT LISTEN TO ME, AND NOW ITS TOO LATE!!!!!( DOT NUMBER: TIRE SIZE: 23575R15 )
PE00 020; CONSUMER WAS TRAVELING ON FREEWAY AND HEARD A NOISE. SHE WAS SLOWING DOWN TO MAKE A RIGHT TURN, AND FELT LIKE SHE WAS LOSING CONTROL OF VEHICLE. VEHICLE ROLLED ONCE. CONSUMER SUSTAINED MINOR INJURIES TO HER ANKLE. THEY WERE FIRESTONE WILDERNESS #AT TIRES. *AK
THE RIGHT REAR TIRE STARTED TO COME APART AT 70 MPH ON THE TURNPIKE IN MIAMI. STOPPED THE CAR ON THE SIDE OF THE ROAD AND NOTICED THE TIRE COMING APART. DROVE TO A FIRESTONE DEALER AND REPLACED THE TWO REAR TIRES AND THE NEXT DAY THE TWO FRONT. THE FIRESTONE DEALER SAID THE TIRE SHOULD NOT COME APART LIKE IT DID AND WOULD SEND THE TIRES TO FIRESTONE ON 5/6/00.( DOT NUMBER: VDHLW02110 TIRE SIZE: 235/75R15 )
