NHTSA Owner Complaint Log
This page lists owner-reported complaints filed with NHTSA for the 2016 Mazda CX-9. Complaints are unverified consumer reports submitted to NHTSA and do not by themselves prove a defect or defect rate.
2016 Mazda CX-9 2.5L Turbo Touring with 93,197 miles. In September of 2022, I noticed a sweet smell coming from the engine bay after coming home from work. I took my car to my mechanic on 9/30/2022 for an oil change and asked if they could figure out the "sweet smell" coming from the engine bay. When they were doing the oil change, they checked for leaks and noticed that coolant was leaking from cylinder head. Since it was out of warranty, I had my mechanic repair the issue. Which resulted in a complete cylinder head replacement. This is a know issue by Mazda and there is lots of documentation online for this as well. There has since been updated parts to repair this issue, but this should be a recall covered by Mazda. Cause this repair cost me $5,700 out of pocket.
The cylinder head has cracked and it leaks antifreeze. This can cause the vehicle to overheat and destroy the engine. It is apparently a known issue as Mazda has issued 3 technical service bulletins addressing the problem. They have even redesigned a new part to replace the faulty one. In the TSB they even address that the crack may be caused by "stress generated during production" In other words, its a known problem yet they havent issued a recall on it. Im guessing they havent issued a recall because the quote for the repair was $5000. At $5000 to repair the faulty part it would be quite the expensive recall for the company.
Car overheated. check front engine and could not see any visible leaks. check all around the engine and found coolant leaking from the back of the engine. confirm by dealer that is leaking at the cylinder head.
Intermittent Airbag Warning Light (Code 18), Passenger Airbag Deactivation light not operating and no warning chime indicating unbuckled passenger side seat belt. The safety issue appears to be related to the airbag and related safety equipment. In the dash, within the speed and rpm display, the red airbag warning light is blinking, reporting from what I gather is a CODE18, as it blinks once, followed by a pause, followed by eight blinks. It appears to be linked an inoperable PAD warning light, which should be on while no one is sitting in the passenger seat and off while someone is sitting on the passenger seat. While the car is running and a passenger sits unbuckled on the passenger seat there is no warning chime indicating that this passenger is unbuckled. Thank you
My Engine is over heading because it has a crack and losing coolant externally. There are 74773 mills on my car. Every 3 weeks I have to put a gallon of coolant in the engine. if I don't put coolant, the engine can stop working on the freeway causing bodily injury to me and other drivers.
Due to Mazda's known problem in the design of their engine at just over 80,000 miles the head and turbo of my engine have cracked. They have issued a service bulletin to their dealerships, which acknowledges this as a common problem. (Technical Service Bulletins (TSBs) are official communications between Mazda & its dealerships that describe processes for troubleshooting or fixing certain common problems.) Unfortunately, for most Mazda CX-9 owners this bulletin was issued after most have just passed their 60,000-mile warranty, thus allowing them to get away with designing an engine for a fairly expensive car that is only good for 80,000 miles. Mazda further acknowledges this as a problem as newer engines are no longer designed the same way to avoid the problem and any parts put on the engines that are failing are of the new design because the OEM parts will just fail again. Dealing with Mazda customer service is a joke, they are rude, refuse to answer questions and will not call you back with answers when they say they will.
The clips on both my rear molding over the tires have broken causing the molding/trim to fall off while I have been driving
2016 cx9, after reviewing online. Known issue with coolent and engine cylinder. Water coolent leak to engine head. Dealer asking 75k to replace engine. Mazda refused to acknowledge the known issue.
Rear wheel trim partially detached from car while driving, safety risk of large plastic part being caught in rear wheel. Did not hit any debris but heard a loud noise as the part detached. Part is supposed to be held in place by 12 clips but multiple owners of Mazda CX-5 and CX-9 models report this problem. Mazda dealers deny there is a problem even when some new cars have only 3 clips installed. When I called Mazda USA to report, and asked them specifically for a federal government contact to report safety issues, they said calling them was the ONLY way to report an issue, which was not correct. Was told I would have to pay for the repair of this safety issue. See this website for the experience of other owners and a description of the problem: https://mazdas247.com/forum/t/2016-cx-9-wheel-well-trim-became-detached.123861160/ I also personally know another Mazda owner who had the same issue with the part falling off while driving.
Traveling on the highway going on a road trip. Had vehicle inspected prior and no fuel system problems were noted. I was going 75mph with my family when there was a coasting feeling in the car. I looked and my check engine light popped on and my car immediately would not accelerate and we quickly lost speed. By the grace of God were we able to pull over safely! Code P0132 was the engine code and after an hour, the engine light went back off and we were able to keep a safe speed traveling home. After speaking with a mechanic and dealership, I learned that this was a common problem with the Denso fuel system. My vehicle was not included in a recent recall that included a different year of my vehicle. If manufacturers are aware that a major defect is out there, how many people have to have an accident or potentially cause a huge accident because a car made 2 years prior with the same part from the same manufacturer has the same dangerous problem? It’s ridiculous and I hope you look at this near fatal issue that has effected thousands of people, whose vehicles’ were made in 2017, 2016, 2015 and are not included in the recall.
I droved my 2016 cx-9 off highway and idled it at front of restaurant, suddenly the engine coolant overheat light came on, shut engine off and found leaking coolant on the ground near the rear of engine bay. Brought the car to Corpus Christi Mazda, found out leaks come from cylinder head crack, happens to cx-9 manufactured before June 20, 2020 according to Mazda TSB 01-013/21. I was quoted $3380 to replace the cylinder head out of pocket because the car is just out of powertrain warranty. I contacted Mazda headquarter for covering the cost out of goodwill since this is due to manufacture defects of cylinder head. Mazda refused to pay.
Our head gasket is needing to be replaced and sounds like this is an apparent Mazda problem how wide spread it is. The cost is over $5,000 for this single repair, possibly over $6,000 if head is cracked. I only have a parts list showing $1,700 in parts. Service manager said about 18 hours at $189hr totalling $3,402.
I have noticed cracks on both sides of the front bumper on my 2016 Mazda CX-9. Mazda has issued a TSB 09-022-20-3720, however, the cracks recently appeared and my car is no longer under the manufacturer's warranty.
15 minutes after getting a wheel bearing replaced at the dealer, my engine coolant high temperature warning came on (never before seen). This occurred while I was on the interstate with no safe place to stop. When I was able to safely stop, I called the dealer. They said bring it back. I was unwilling to drive that far so chose the closest Mazda dealer. They informed me I have a cracked cylinder and the coolant is leaking into exhaust system. I was told the only option due to the mileage on the car (134,000) is to replace the engine for $8,000. This appears to be a known issue so I am shocked that it wasn't being checked for prior to the problem.
Cylinder head cracked, leaking coolant and causing engine overheated , potential fire hazard . Coolant also leaked and mixed in with oil cycling through engine , causing permanent damage to the engine , resulting engine to be fully replaced
On 04/21/22, I took my car to the dealer after noticing antifreeze on our garage floor and a burning smell. NO check engine lights had illuminated. They proceeded to tell me that there was a coolant leak from head/block and cavity leak (something about channel pressure). They also stated this was a casting flaw during manufacturing of the engine. They told me I needed a whole new engine for $8,479.97! Mazda corporate refused to replace the engine at their cost because I had 64,575 miles on it and it was out of powertrain warranty. I called them and asked to speak to a supervisor and never heard from them. I called on 4/26, 4/27, 4/28, 5/4, 5/5 (5 times), 6/9, & 6/20. I finally gave up agreed to pay $7,479.97 (the dealer gave me $1,000 off). I received the car back on 05/20 and it would not start later that day. They towed it back to Mazda and they replaced the crankshaft sensor that was damaged upon install of the new engine. I received it back again on 05/31 and later in the day, the check engine light came on and the automatic braking system engaged while I was driving (thankfully only 40 mph). I managed to pull over in a turn lane before the engine died. I was so thankful I did not make it onto the interstate I was about to enter! Mazda came and picked me up and later got my car. They could not figure out why it was dying and finally got it to recreate it. They replaced another engine sensor and then drove it a lot to make sure it was truly fixed. I finally received my vehicle back on 07/07. I am afraid to take my car out of town and it makes me nervous it will die and automatically start braking again. They KNOW this is a manufacturer issue and they refuse to take responsibility. I am so disappointed in them. This is a huge safety issue. I am also lucky I didn't blow a head gasket before I took it into the shop the first time.
Purchased the car used with 67k miles on it. Within 2 weeks of purchase warning lights went off saying that the engine was overheating due to censor malfunction. After three visits to mechanics (2 to the dealership and 1 to a private mechanic) it was determined that coolant was leaking into the engine causing the engine block to crack. This of course means that the engine will have to be replaced in order for it to be useable. That repair has been quoted by the dealership as a $14,000 repair. For a car that has less than 70k miles.
At an intersection the engine stalled and car was extremely hard to restart (took several minutes). I later noticed oil and coolant dripping from the engine and coolant mist/vapor from the exhaust. Later I learned it was possibly related to Mazda's own TSB Bulletin No: 01-013/21 where a manufacturing defect caused coolant to enter the combustion chamber and hydro lock the engine (where I stalled and had hard time restarting). I had to have the car towed home. At home I noticed the passenger underside of the vehicle coated with oily/watery residue of possible oil or oil/coolant mix. Vehicle had 66K miles on and Mazda dealer refused to fix.
Dealer diagnosed manufacturing defect that caused crack in head. {Which could lead to engine failure, and subsequent accident due to engine failure during operation.} Engine lost coolant. No overheating noted. No warning/error codes noted.
The contact owns a 2016 Mazda CX-9. The contact stated that while driving at an undisclosed speed, the air conditioning became warm and the fan was abnormally loud. Additionally, the engine temperature gauge was high. The contact pulled over and refilled the coolant reservoir. The vehicle was taken to an independent mechanic who initially diagnosed that the water pump was faulty and needed to be replaced. However, when the contact took the vehicle back to the independent mechanic for the water pump replacement, the mechanic diagnosed that the cylinder head was warped and referred her to the dealer. The vehicle was not repaired. The dealer was made aware of the failure and informed the contact that there was no recall associated with the VIN. The manufacturer was made aware of the failure and informed the contact that there was no recall associated with the VIN and referred the contact to file a complaint with the NHTSA. The failure mileage was approximately 120,000.
Brake Booster damaged. I ended up with no brakes and nearly crashed.
UKNOWN-Seat Heating system in my car gets too hot to the skin where it burns the skin if not turned off. I’m not able to use it often because even on low setting it gets extremely hot.
Cracked cylinder head due to manufacturing defect, left stranded at car wash with no engine coolant due to issue, took several months without a vehicle before it was replaced by Mazda. The vehicle has been repaired. I was left unable to drive in hot weather and could not use air conditioning while waiting for assistance. Dealer had seen the issue before, reported by many on online Mazda forums. There were no warning lamps prior to the incident happening.
I HEARD A PINGING SOUND COMING FROM ONE OF THE WHEELS. WHEN I GOT OUT AND LOOKED, I DISCOVERED THE WHEELS ARE BEGINNING TO FLAKE AND DEGRADE. I CONTACTED MAZDA CUSTOMER SERVICE AND WAS TOLD I WOULD HAVE TO COVER THE COST OF REPLACEMENT AS THE VEHICLE IS OUT OF WARRANTY.
- THE CAR WAS PURCHASED NEW FROM A MAZDA DEALER IN MAY 2017 - THE DRIVER SIDE CUSHION HEATER FAILED IN MARCH 2020. - THE PASSENGER SIDE CUSHION HEATER FAILED IN NOVEMBER 2020 - BOTH SIDES EXHIBITED THE SAME PROBLEM DESPITE THE FACT THAT THE USAGE OF THE PASSENGER SIDE WAS ABOUT 25% OF THE DRIVER - FROM THE TIME THE CUSHION HEATERS FAILED, WE OBSERVED RANDOM OVERHEATING OF THE BACK HEATERS AND RECORDED "HOT SPOTS" OF UP TO 149 DEG F (PICTURE ATTACHED) - WHILE THE CUSHION HEATERS DON'T OPERATE, THE BACKS GET TOO HOT FOR OCCUPANTS (ATTACHED PICTURE OF SKIN REACTION) AND AT ONE POINT THE PASSENGER SIDE PARTIALLY MELTED A PLASTIC SHOPPING BAG - MAZDA RESPONDED TO THE COMPLAINT THAT THE CAR WAS OUTSIDE OF THE 3-YEAR WARRANTY (EXPIRED IN MAY 2020) AND THERE WERE NO RELATED RECALLS SO THE COMPANY WAS NOT INTERESTED IN ANY TESTING OR INVESTIGATION OF THE PROBLEM ATTACHED: - PICTURE #1: TEMPERATURE IR READINGS - PICTURE #2: EFFECT OF THE OVERHEATING ON ONE OF THE OCCUPANTS
MY WIFE AND DAUGHTERS TOOK A TRIP, AND ON THE WAY HOME, THE ENGINE LIGHT CAME ON. WHEN TURNING ON THE ENGINE TO CHECK THE ENGINE LIGHT, I NOTICED THAT THE BRAKE PEDAL WAS HARD. I WAS THINKING THE ENGINE LIGHT HAD TO DO WITH THE GAS CAP NOT BEING ON TIGHT SINCE FAMILY PURCHASED GAS ON THE TRIP BACK HOME. I DROVE THE CX-9 TO AUTOZONE TO DETERMINE THE CODE FOR THE ENGINE LIGHT. THE ATTENDANT AT AUTOZONE WAS UNABLE TO REGISTER A CODE. HOWEVER, I NOTICED THE BRAKES FELT DIFFERENT ON THE DRIVE TO AUTOZONE. ALTHOUGH I HAD TO APPLY MORE PRESSURE TO STOP, IT TOOK LONGER TO STOP. IT ROLLED TO A STOP. WHEN I GOT BACK HOME, I HEARD A HISSING SOUND COMING FROM UNDER THE DASHBOARD. MAZDA'S OWNER'S MANUAL REGARDING THE ENGINE LIGHT INDICATES 1.) THE FUEL TANK LEVEL BEING VERY LOW, 2.) POSSIBLE ELECTRICAL SYSTEM PROBLEM, 3.) EMISSION CONTROL PROBLEM, OR 4.) FUEL FILLER CAP IS MISSING OR LOSE. THE NEXT MORNING I DROVE THE CX-9 TO SEE IF TIGHTENING THE GAS CAP WOULD CORRECT THE ENGINE LIGHT. DURING THE DRIVE, THE BRAKING SEEMED DIFFERENT. I WOULD PRESS THE BRAKE, BUT THE VEHICLE WOULD ROLL TO A STOP INSTEAD OF STOPPING. WHEN I WAS APPROACHING A CAR STOPPED AT A RED LIGHT, I HAD TO PRESS THE BRAKE VERY HARD TO AVOID THE VEHICLE. I PARKED IT AND CALLED THE LOCAL MAZDA DEALER. AFTER THEY PERFORMED A DIAGNOSTIC TEST, THE BRAKE BOOSTER WAS DETERMINED TO BE FAULTY. I WAS CONFUSED ABOUT HOW A BRAKE BOOSTER AND AN ENGINE LIGHT CORRELATES. I'M ALSO TRYING TO UNDERSTAND WHY THE BRAKE SYSTEM WARNING LIGHT WAS NOT ACTIVATED SINCE THIS RELATES TO THE BRAKING SYSTEM. AFTER ADDITIONAL RESEARCH, I'M IN COMPLETE DISBELIEF THAT MAZDA IS AWARE, AND IT HAS NOT DECIDED TO ADDRESS THIS SAFETY ISSUE. PLEASE TAKE ACTION TO HAVE MAZDA RECALL THE BRAKE BOOSTERS TO ADDRESS THIS ISSUE SINCE IT'S AWARE THE PROBLEM IS AN EQUIPMENT/MATERIALS ISSUE.
MY MAZDA CX-9 RAN OUT OF GAS ON A MAJOR HIGHWAY (ROUTE 90 IN BOSTON) AND THE GAS GAUGE READ HALF FULL. NO 'OUT OF GAS' LIGHT WENT ON AND RANGE (UNDER INFORMATION) SAID 191 MILES REMAINING. MY MAZDA DEALER TOLD ME THE SOFTWARE NEEDED TO BE RESET (NOT DONE YET AS OF THIS WRITING). IT IS A MAJOR ISSUE IF THE GAS GAUGE IS NOT READING PROPERLY AND THIS IS A SOFTWARE ISSUE (A BUG?). MAZDA NEEDS TO FIX THIS FOR ALL MAZDA'S AND ISSUE A RECALL SO IT DOES NOT HAPPEN AGAIN.
I JUST PURCHASED MY VEHICLE ABOUT 3 MONTHS AGO. I WAS RUNNING ERRANDS ABOUT A WEEK AFTER PURCHASE AND WAS HEADING HOME ON THE HIGHWAY WHEN THE GAS NOTIFICATION CAME ON DISPLAYING 27 MILES LEFT AND ADVISED ME TO GET GAS. I WAS ABOUT HALF MILE FROM THE EXIT AND THE VEHICLE CHECK ENGINE LIGHT CAME ON AND IT STARTED STALLING. THANK FULLY I WAS ABLE TO GET OVER TO THE MEDIAN JUST IN TIME WHEN IT SHUT DOWN COMPLETELY. I CALLED ROADSIDE AND THE DEALER AND EXPLAINED WHAT HAPPENED. I HAD IT TOWED BACK TO THE DEALER AND THEY ADVISED ME IT WAS BECAUSE I RAN OUT OF GAS. I EXPLAINED IT HAD A QUARTER OF A TANK AND DISPLAYED 27 MILES LEFT AND IT DIDN'T EVEN GIVE ME 1 MILE TO GET OFF THE EXIT. IT HAPPENED TO MY HUSBAND AS WELL RECENTLY AND I SPOKE TO A CERTIFIED MAZDA DEALER AND THEY EXPLAINED IT COULD BE THAT THE GAS TANK HAS A DENT CAUSING THE READING TO BE OFF OR ONE OF TWO FLOATS COULD BE DAMAGED AND NOT READING CORRECTLY. I WAS ADVISED THIS WAS A COMMON ISSUE WITH MY YEAR BUT IT'S NOT ON A RECALL. I USUALLY GET GAS WHEN IT HITS A QUARTER OF A TANK BUT NOW I'M AFRAID THAT I DO IT AT A HALF A TANK. THIS SHOULD BE A RECALL PROBLEM AS IT CAUSES THE CAR TO STALL IMMEDIATELY WHICH IS DANGEROUS. I HAD KIDS IN MY VEHICLE IN AN ICE STORM...I NEED TO HAVE THIS FIXED BUT IT'S WELL OVER $800.
WHILE DRIVING THE CAR IN THE 287 INTERSTATE HIGHWAY IN NEW JERSEY, I GOT THE CHECK ENGINE LIGHT OUT OF THE BLUE AND THE CAR STARTED DECELERATING IN THE HIGH SPEED LANE. LUCKILY I WAS ABLE TO MANAGE AND GET THE CAR TO THE ROAD SIDE SHOULDER. I WAS NOT ABLE TO START THE CAR AFTER THIS INCIDENT. WE HAD TO TOW THE CAR TO THE MAZDA SERVICE CENTER IN GREENBROOK, NEW JERSEY. AFTER RUNNING THE CAR DIAGNOSTICS (ERROR CODE - P0192), THEY INFORMED ME THAT THEY HAD TO UPDATE THE PCM (POWER CONTROL MODULE) SOFTWARE. THATS THE ONLY EXPLANATION WHICH THEY GAVE WHICH MADE NO REAL SENSE. THEY DID NOT PROVIDE ANY INFORMATION WHY THIS ISSUE OCCURRED AND WHAT WAS DONE TO MAKE SURE THIS ISSUE DOES NOT OCCUR IN THE FUTURE.
WITH NO WARNING THE SMART BRAKE SYSTEM MALFUNCTION AND THE FRONT RADAR SYSTEM MALFUNCTION WARNINGS COME ON AND I AM UNABLE TO USE THE CRUISE CONTROL. IN ADDITION THE PARKING BRAKE LIGHT COMES ON AND THE PARKING BRAKE IS NOT ON AS I AM DRIVING.
DRIVER STOPPED VEHICLE IN A DRIVEWAY WITH MINIMAL SLOPE, SHIFTED THE VEHICLE IN PARK, TURNED OFF THE VEHICLE AND EXITED. DRIVER PROCEEDED TO REAR PASSENGER DOOR TO RETRIEVE 1 YEAR OLD FROM CAR SEAT. AFTER RETRIEVING CHILD FROM CAR SEAT, DRIVER FELT REAR PASSENGER DOOR PUSHING INTO HER LEG, BACK AND HEAD. VEHICLE PROCEEDS TO ROLL OUT OF THE DRIVEWAY BACKWARD WHILE IN PARK. DRIVER AND CHILD ESCAPE VEHICLE'S REAR PASSENGER SIDE DOOR, WHICH IS PUSHING THEM ALONG IN THE VEHICLE'S PATH TOWARD RESIDENTIAL STREET. VEHICLE CONTINUES ROLLING IN PARK BACKWARD DOWN RESIDENTIAL STREET, THROUGH A 3-WAY STOP INTERSECTION, UP A CURB, THROUGH A YARD AND INTO A BUSH ON A PRIVATE PROPERTY. A WITNESS WHO HELPED RETRIEVE THE VEHICLE ON BEHALF OF THE DRIVER NOTED THE VEHICLE WAS STILL IN PARK WHEN HE TRIED TO REMOVE IT FROM THE BUSH. NO ONE WAS SEVERELY INJURED BUT DRIVER HAS SUSTAINED BRUISING ON LEG AND SEVERE EMOTIONAL DISTRESS.
BOTH REAR BRAKES LOCKED UP ON SEPARATE OCCASION WHILE DRIVING ON THE FREEWAY AT ABOUT 70 MPH. EACH TIME THE BRAKE ENGAGED THEMSELVES AND LOCKED ONTO THE DISK/ROTOR. THIS CAUSED THE PADS TO BURN UP AND PRODUCE A LARGER QUANTITY OF SMOKE. ON ONE OCCASION THE ROTOR BECAME RED HOT, FLAME WAS VISIBLE, LOTS OF SMOKE WAS BEING PRODUCED, AND IT WARPED ROTOR. EVERY TIME IT OCCURRED I STOPPED THE VEHICLE AND TURNED IT OFF. AFTER THE RESTART OF THE VEHICLE THE BRAKES WERE NOT LOCKED BUT THE VEHICLE WOULD SHAKE AS NOW THE ROTORS WERE WARPED AN THE PADS WERE DESTROYED. I TOOK MY VEHICLE TO THE MAZDA DEALERSHIP AND THEY INFORMED ME THAT I NEEDED NEW PADS, I DID BUT IT WAS DUE TO THE REAR BRAKES LOCKING UP ON THEIR OWN. I GOT THE PADS REPLACED AND 6 MONTHS LATER IT HAPPENED AGAIN. THIS TIME I RECORDED A VIDEO ON MY PHONE AND SHOWED THE DEALER SHIP WHAT I EXPERIENCED AGAIN. THIS TIME THEY INFORMED THAT I NEEDED, NEW BRAKE PADS(AGAIN), ROTORS, AND PISTON BUT THEY DID NOT ADDRESS WHAT IS CAUSING THE REAR BRAKES ONLY TO LOCK UP ON THEIR OWN CREATING EXCESSIVE WEAR AND DAMAGE TO THE BRAKE COMPONENTS. SOMETHING IS AND CONTINUES TO ENGAGE MY REAR BRAKES. IT LAST OCCURRED ON 10-04-19 AND IT HAS ONLY BEEN 3 DAYS SINCE THE MAZDA DEALER REPLACED THE DAMAGED PARTS AND UNDER HARD BRAKING THE VEHICLE CONTINUES TO SHAKE WHICH ARE EARLY SIGNS OF WHAT HAPPENED BEFORE, SO ITS ONLY A MATTER OF TIME BEFORE IT OCCURS AGAIN.
Engine Coolant Leaking from rear of engine near Exhaust / turbocharger at Cylinder Head. No Warning Lights indicating problem until coolant level low causing overheating and fire risk. Rear Brakes not releasing completely from "City Mode" where brakes are kept close to rotor "to increase braking response in City driving" - causes premature brake wear, warped brake rotors (reoccurs within 10K miles from new / replaced rotors) Front Bumper Fascia keeps coming loose at clips near headlights on both Driver and passenger side. Had replaced at dealership 3 times, but continues. Bumper coming loose and can obstruct headlights while moving around at highway speeds
IN 2017, MY CAR SHUT OFF TWO TIMES, WITHOUT WARNING, ONE TIME AT A STOP LIGHT AND AGAIN ON THE HIGHWAY GOING 70MPH. WE WERE ACCELERATING AT THE STOP LIGHT WHEN IT SHUT OFF AND THEN DRIVING ON THE HIGHWAY. TOOK THE CAR TO THE DEALERSHIP AND 72 CODES CAME UP. THEY SAID IT WAS THE DVD PLAYERS (THAT THEY SOLD AND INSTALLED!!!) THAT WERE CAUSING THE ISSUE. THEY SUPPOSEDLY 'FIXED' THE ISSUE. NOW IN JULY 2019, THE CAR SHUT OFF AN ADDITIONAL FIVE TIMES. THREE TIMES WHILE DRIVING AND TWICE IN A PARKING LOT. WE WERE ON A HIGHWAY AGAIN WHEN THE CAR JUST SHUT OFF WITH NO WARNING, LIGHTS ON THE DASH STARTED FLASHING, AND THE POWER STEERING WENT OUT ONE OF THE TIMES. MY HUSBAND HAD TO PUT IT IN NEUTRAL, COAST TO THE SHOULDER, COME TO A COMPLETE STOP AND THEN ATTEMPT TO START IT EACH TIME. CARS WERE DODGING US EVERY TIME AND LUCKILY WE WEREN'T HURT. MY CHILDREN WERE IN THE VEHICLE EVERY SINGLE TIME. WE HAD THE CAR TOWED TO THE DEALERSHIP WHERE 68 CODES CAME UP. THIS TIME THEY'RE SAYING IT'S THE FUEL PUMP AND THAT THEY CAN 'FIX' IT, BUT CAN'T 100% GUARANTEE IT WON'T HAPPEN AGAIN. WE WERE PUT IN A LIFE THREATENING SITUATION EVERY TIME, WITHOUT WARNING, AFTER GIVING THE DEALERSHIP A CHANCE TO FIX IT. THE CAR IS UNDER THREE YEARS OLD AND ONLY HAS 30,400 MILES ON IT.
FAILS TO ACCELERATE AND CHANGE GEARS. THIS HAS BEEN HAPPENING SINCE WE PURCHASED THE VEHICLE. IT HAS BEEN BACK TO THE DEALERSHIP MULTIPLE TIME AND HAS NOT BEEN RESOLVED. THEY ARE UNABLE TO FIND ANYTHING WRONG. THE ENGINE IS A 2.5 L TURBO. THIS HAPPENS UNEXPECTEDLY WHILE DRIVING MAKING IT HARD TO GET ON TO THE HIGHWAY OR CHANGE LANES.
P 255 / 50 R 20 104V M+S tread physically detached while driving at 70MPH Was able to steer and stop the vehicle and avoid a crash, partially due to fewer traffic on the road and the tire in question was on the rear driver side. Was able to replace the tire with my insurance. However no intimation about the failure was mentioned or no further action taken about safety of other tires previously on the vehicle The incident took place suddenly with a huge noise due to the broken tire hitting the wheel well and ripping off the fender. The accident happened on 17th Apr 2019 A supporting link referring a similar incident to a with different model year and date code https://static.nhtsa.gov/odi/rcl/2019/RCAK-19V770-4232.pdf Makes/Models/Model Years: MAZDA/CX-9/2019 Mfr's Report Date: October 28, 2019 NHTSA Campaign Number: 19V-770 Components: TIRES:TREAD/BELT Potential Number of Units Affected: 146 Mazda's number for this recall is 4019J
BRAKE LIGHTS COME UP INTERMITTENTLY, ESPECIALLY WHEN TEMPERATURE IS LOW. ALSO, FRONT RADAR SENSOR SYSTEM MALFUNCTION AND SMART BRAKE SYSTEM MALFUNCTION WARNINGS COME UP ALSO,. THIS IS DURING THE DRIVE.
I WAS DRIVING ON THE INTERSTATE AND THE TPMS WARNING CAME ON. I PULL OFF THE INTERSTATE AND PARK. THERE IS SMOKE COMING FROM THE RIGHT REAR WHEEL AND THE CENTER OF THE WHEEL IS EXTREMELY HOT, TOO HOT TO TOUCH. I CAN FEEL THE HEAT FROM ABOUT A FOOT AWAY. THERE IS METAL PINING AND GROANING AS IT COOLS DOWN, THE TIRE IS WARM, BUT NOT AS HOT AS THE CENTER A THE WHEEL. THE LEFT REAR WHEEL IS COOL TO THE TOUCH, AS ARE THE FRONT WHEELS. I TOGGLE THE ELECTRONIC PARKING BRAKE A FEW TIMES AND AFTER THE WHEEL HAS COOLED DOWN I DRIVE ON. THE PROBLEM DID NOT OCCUR AGAIN IN THE LAST 60 MILES, HOWEVER, THE RIGHT REAR WHEEL NOW MAKES A NOISE WHEN BRAKES ARE BEING APPLIED CONSISTENT WITH THE ROTATION OF THE WHEEL.
MULTIPLE TIMES WHILE DURING INCLEMENT WEATHER, POURING RAIN/SLEET, THE DRIVER'S SIDE WINDSHIELD WIPER BLADE FLEW OFF OF THE CAR - CAUSING A SAFETY ISSUE WITH VISIBILITY. WE HAD TO PULL OVER TO THE SIDE OF THE HIGHWAY AND SWITCH TO THE PASSENGER SIDE'S WIPER BLADE, SO THAT WE COULD GET TO A SAFER PLACE TO BE OUT OF THE WAY OF TRAFFIC. IT WAS NIGHT TIME DURING ONE INCIDENT AND DAYTIME DURING THE OTHER. THE ISSUE SEEMS TO BE THE WAY THE WIPER BLADE ATTACHES TO THE ARM.

I WAS T-BONED ON PASSENGER SIDE OF MY MAZDA CX9 AIR BAGS DID NOT WORK ON MY MAZDA.OTHER VEHICLE AIR BAGS DID GO OFF.I WAS TRAVELING ABOUT 45 MPH WHEN OTHER CAR PLOWED INTO MINE.
THIS INCIDENT HAPPENED ON 31ST JULY, WHEN MY WIFE WAS DRIVING THIS CAR ON HIGHWAY 4 MARTINEZ, CA AT AROUND 5.00 PM WITH OUR DAUGHTER (2 YEARS OLD) RETURNING FROM DAYCARE. SHE WAS DRIVING AT APPROX 60 MILE/HR, WHEN THE CAR STARTED GIVING AUDIO VISUAL WARNINGS ON MULTIPLE FAILURES AND THE CAR STARTED SLOWING DOWN WITHOUT RESPONDING TO THE GAS PEDAL AND ENGINE PROPULSION REDUCED TO 'ZERO'. MY WIFE PANICKED AND WAS LUCKILY ABLE TO PULL OVER ON THE CURB SIDE SAFELY, EVEN THOUGH TRAFFIC WAS VERY HIGH. SHE STOPPED THE CAR AND TURNED OFF THE ENGINE AND WAITED FOR 10 MINS AND RESTARTED THE CAR MULTIPLE TIMES AND MADE SURE THAT NO MORE WARNINGS ARE DISPLAYED, BEFORE DRIVING AGAIN, HOWEVER THE 'ENGINE WARNING LIGHT' STAYED ON. AFTER A WHILE SHE DROVE HOME ALMOST 40 MILES AWAY VERY CAREFULLY WITHOUT USING ANY CRUISE CONTROL. HERE ARE THE WARNINGS, THAT CAME ON DURING THIS FAILURE, PICTURES ATTACHED. ENGINE MALFUNCTION LIGHT. TIRE PRESSURE INDICATOR. STABILITY CONTROL INDICATOR. TEMPERATURE INDICTOR CRUISE CONTROL FAILURE WARNING MESSAGE 'FRONT RADAR SENSOR SYSTEM MALFUNCTION' WARNING MESSAGE 'FORWARD SMART CITY BREAK MALFUNCTION' AFTER A DAY WE DROPPED THE CAR TO MAZDA DEALERSHIP FOR INSPECTION AND THEY FOUND MULTIPLE FAILURE CODES AND THEY DID NECESSARY REPAIR (INSPECTION AND REPAIR INVOICE ATTACHED). EVEN BEFORE THIS INCIDENT , WE REPORTED MULTIPLE TIMES TO DEALERSHIP THAT THE CAR CENTRAL INFORMATION SYSTEM ABRUPTLY SHUTS DOWN DURING DRIVE. BUT THEY ALWAYS SAID THAT THERE WAS NO PROBLEM. THIS INCIDENT ON A FREEWAY WAS VERY SERIOUS, MY FAMILY OR OTHER DRIVERS COULD HAVE BEEN IN A INJURY ACCIDENT BECAUSE OF THIS ISSUE WITH MAZDA.
SUNROOF EXPLODED WHILE DRIVING APPROX 40 MPH. WERE IN A VALLEY ON VACATION IN COLORADO, NO TREES OR ANYTHING OVERHEAD WHEN THIS HAPPENED. WE HAD JUST CLOSED THE WINDOWS AND TURNED ON THE AIR CONDITIONER AS IT WAS GETTING HOT. ABOUT 2 MINUTES LATER WE HEARD A LOUD EXPLOSION. ONLY WHEN WE PULLED OVER DID WE DISCOVER IT WAS THE SUNROOF.
DRIVING TO WORK IN THE MORNING AT 5:45AM ON 06/05/2018 GOING 65 MPH AND MY CAR CUTS ENGINE POWER AND STARTS LOWLY DECLINING PRESSING GAS DID NOTHING AND MULTIPLE NOTIFICATIONS FLASHING SUCH AS VEHICLE SYSTEM MALFUNCTION, FORWARD SMART CITY BRAKE SUPPORT MALFUNCTION, SMART BRAKE SYSTEM MALFUNCTION. I HAD TO PULL OFF TO THE SIDE OF THE HIGHWAY I CAN'T EVEN SAY I CAN PULL OFF SAFELY BECAUSE ALL POWER WAS CUT. SO LET THE CAR SIT FOR 5 MINUTES WHILE OFF AND THEN RESTARTED AND WAS ABLE TO MAKE IT WORK. I TOOK PICTURES AS EVERYTHING WAS HAPPENING BECAUSE I KNOW THE DEALER SHIP WILL SAY SOMETHING LIKE "WELP IT AIN'T HAPPENING HERE SO YOUR CAR IS FINE." I AM VERY FORTUNATE WHEN I BOUGHT THE CAR I DECIDED TO LEASE THE VEHICLE SO I CAN JUST TAKE THE CAR BACK. THESE MALFUNCTIONS SEEM LIKE A VERY SEVERE PROBLEM AND I DO NOT WANT TO HANG ON TO THE VEHICLE. ONCE 30K PASSES I WILL BE SUPER SCREWED! I CALLED DEALER SHIP AND THEY ARE BOOKED OUT 2 WEEKS SO I CANT EVEN TAKE THE CAR IN RIGHT AWAY, PLUS THEY AREN'T EVEN SURE IF THEY'LL HAVE A RENTAL FOR ME WHEN I DROP IT OFF. ALSO TO MENTION, AC UNIT ALREADY HAD TO BE REPLACED, AND A SUSPENSION RECALL. WORSE CAR I'VE HAD SO FAR ITS SUCKS BECAUSE MAZDA USED TO BE SO GREAT. THEIR SERVICE DEPARTMENT SUCKS BIG TIME! I NEVER HAD THIS TYPE OF SERVICE OR ISSUES THIS SEVERE WITH MY CHEVY. 08/07/2018 I TOOK CARE IN AGAIN FOR SAME SITUATION ALONG WITH AC AND ALL MAZDA TELLS ME IS THEY ARE LOOKING INTO AND ASSURE ME IT WILL BE TAKEN CARE OF THIS TIME. (ALSO SAID LAST TIME) I THINK THAT THEY DO NOT KNOW HOW TO FIX THE ISSUE! I CALLED CORPORATE TO ASK FOR A REFUND AND GIVE THE CAR BACK AND ALL THE TOLD ME WAS TO GIVE DEALERSHIP ANOTHER CHANCE TO FIX THE ISSUE. IT IS SO UNSAFE TO DRIVE AND HAVE KIDS IN THE CAR, I AM VERY FORTUNATE TO NOT HAVE MY BABY NEPHEW AND NIECES WITH ME WHEN THESE THINGS WERE HAPPENING.
THE INFOTAINMENT SCREEN CONTAINING THE BACKUP CAMERA GOES BLANK . THE CAR HAS 16000 MILES ON IT AND HAS HAD THIS PROBLEM SINCE NEW. THE DEALER HAS THE CAR MULTIPLE TIMES AND HAS NOT FIXED THE PROBLEM. RESEARCH ON THE INTERNET REVEALS THAT THERE IS A MAJOR ISSUE WITH MANY MAZDA VEHICLES. THIS PROBLEM HAPPENS ON STARTUP AND WILL CONTINUE TILL SHUTOFF . IT MAY OR MAY NOT COME ON UPON RESTART
WHEN DRIVING AT 35-50 MPH, MY 2016 MAZDA CX-9 PERIODICALLY BRAKES SUDDENLY WHILE DRIVING TO A SCRUBBING FORCEFUL STOP TOSSING THINGS OFF THE BACK SEAT, PLACING ME IN DANGER OF A REAR-END COLLISION. SOMETIMES A VERY QUICK BEEPING SOUND IS MADE, "B-R-A-K-E" APPEARS ON THE PANEL IN LARGE LETTERS --- THEN THE CAR AUTOMATICALLY COMES TO A SUDDEN STOP OR IT OCCURS WITH NO NOTICE. I HAVE TAKEN IT TO THE DEALER 3 OR 4 TIMES ONLY TO BE TOLD THAT THEY COULD FIND NOTHING WRONG. NO OBJECTS OF VEHICLES WERE IMMEDIATELY IN FRONT OF ME AND THE SENSOR WAS CLEAN WHEN THIS HAS OCCURRED.THIS SUDDEN STOPPING HAS OCCURRED ON THE FREEWAY, IN RUSH HOUR TRAFFIC AND ON CITY STREETS. THIS HAS OCCURRED AT LEAST 6 TIMES SINCE PURCHASED 03/17/17. I AM FEARFUL THAT THIS PROBLEM WILL RESULT IN INJURY OR DEATH TO ME AND OTHERS.
IMMEDIATELY UPON MAKING A SHARP RIGHT TURN INTO A PARKING SPOT, THE CAR SUDDENLY ACCELERATED AT A VERY FAST PACE WITHOUT ME PUSHING THE GAS PEDAL. MY IMMEDIATE REACTION WAS TO PRESS ON THE BRAKE, BUT IT DID NOT STOP. THE CAR SLAMMED INTO A PLANTER IN FRONT OF ME. THE CAR MOVED SO QUICKLY THAT IT WAS LODGED INTO THE BUSHES AND HAD TO BE PULLED OUT BY A TOW TRUCK. I IMMEDIATELY CALLED THE DEALER SERVICE DEPARTMENT AND TOLD THEM WHAT HAPPENED. THE SERVICE MANAGER'S IMMEDIATE REACTION WAS TO SCOFF AT THE IDEA THAT THE CAR ACCELERATED ON IT'S OWN. MY HUSBAND MET ME AT THE DEALER AND HAD SUBSEQUENTLY FOUND INFORMATION ABOUT A RECALL OF CX-9 MODELS FROM 2012 FOR A SIMILAR MALFUNCTION. HE SHOWED IT TO THE SAME MANAGER WHO IMMEDIATELY KNEW THE ISSUE OF THE RECALL BUT WAS QUICK TO MENTION IT WAS THE 2012 MODEL, NOT THE 2016. THE DEALER KEPT THE CAR FOR A FEW DAYS AND TOLD ME IT WAS DRIVEN TWICE AND ALSO SOME INFO FROM THE COMPUTER WAS REVIEWED WITH MAZDA TECH SUPPORT PERSON. NO DATA INDICATING ANY KIND OF MALFUNCTION IN THE CAR AND THE SERVICE GUY DID NOT HAVE A PROBLEM WITH IT IN TWO SHORT TEST DRIVES, SO THEY SAID COME PICK IT UP IT'S SAFE TO DRIVE. HOWEVER, I WILL NEVER DRIVE THIS CAR AGAIN. THIS IS THE FULLY-LOADED SIGNATURE CX-9 THAT THEY FEATURE IN ALL THEIR ADS. I LOVED THIS CAR AT FIRST SIGHT AND DROVE IT OVER 24000 MILES IN LESS THAN 2 YEARS. THIS EXPERIENCE SCARED ME TO THE POINT THAT I WILL NEVER DRIVE THIS OR ANOTHER MAZDA AGAIN. I HAVE HAD OVER A WEEK TO PROCESS THIS EXPERIENCE AND AM JUST HAPPY MY KIDS WEREN'T IN THE CAR OR THAT I DIDN'T HIT SOMEONE. I HAVE FOUND SEVERAL OTHER INSTANCES OF SUDDEN UNINTENDED ACCELERATION ONLINE AND CANNOT BELIEVE THAT THIS HAPPENS IN 2017. I HOPE THAT THIS DOCUMENTATION HELPS SOMEONE ELSE IN MY SHOES FEEL THAT THEY ARE NOT CRAZY AND THIS DOES HAPPEN. SHAME ON MAZDA FOR NOT FULLY ADDRESSING THIS ISSUE.
THE PUSH BUTTON ON/OFF SWITCH DOES NOT PROTECT AGAINST TURNING THE VEHICLE OFF IF TRANSMISSION IS ACCIDENTALLY LEFT IN ANY POSITION OTHER THAN 'P'. FOR EXAMPLE ITS POSSIBLE TO PUT THE TRANSMISSION IN 'R' AND TURN THE VEHICLE OFF. FOR A BUSY PARENT (TARGET DEMOGRAPHIC OF THIS VEHICLE), THIS IS VERY DANGEROUS. IF ON AN INCLINE, THE VEHICLE WILL START ROLLING FORWARD OR BACKWARD (DEPENDING ON INCLINE/SLOPE AND R/D TRANSMISSION POSITION). THERE SHOULD BE A SAFEGUARD OR A BEEP/WARN SYSTEM TO PROTECT THE DRIVER/OCCUPANTS THIS HAPPENED TO ME ON AN INCLINED DRIVEWAY WHEN I ACCIDENTALLY LEFT TRANSMISSION IN 'R' AND TURNED VEHICLE OFF. I TOOK FOOT OFF THE BREAK AND GOT OUT AND REALIZED VEHICLE WAS ROLLING BACKWARDS.
VEHICLE MAKES A CRANKING NOISE WHEN STEERING AT A SLOW SPEED MEANING WHEN TURNING, REVERSING OR SLOWLY GOING OVER SPEED BUMPS IT MAKES A CRANKING NOISE ITS COMING FROM THE FRONT OF VEHICLE I DON'T KNOW IF THAT NORMAL HAS BEEN LIKE THAT SINCE I BOUGHT THE VEHICLE, IT WAS TAKEN IN DUE TO A RECENT RECALL COUPLE MONTHS AGO FOR THE REAR SAME ISSUES WITH A CRANKING NOISE, NOW IT'S THE FRONT.
AFTER A MINOR REAR END ACCIDENT, THE DRIVER FRONT SEAT NOW ROCKS SIDE TO SIDE AND IS DISTRACTING AND UNSAFE. IT WAS TAKEN TO THE LOCAL MAZDA DEALERSHIP THAT PROVIDED AN INSPECTION AND SAID THAT THERE IS NO PROBLEM WITH THE SEAT. THEY STATE THAT IT IS NORMAL FOR THEIR SEATS TO ROCK SIDE TO SIDE. HOW UNSAFE AND FRIGHTENING. PLEASE SEE ATTACHED DOCUMENTATION FROM THE DEALERSHIP.
