The contact owns a 2023 Tesla Model Y. The contact stated while driving 75 MPH with Self Driving feature activated, the vehicle experienced phantom braking and abruptly decelerated to 35 MPH. The contact stated that there was no vehicle or object in front of the vehicle. The contact stated that the failure reoccurred several times. The contact stated that the initial failure occurred after a software update was remotely completed the day before. The manufacturer was not notified of the failure. The vehicle was not diagnosed nor repaired. The failure mileage was approximately 4,500. The consumer stated the manufacture was notified on 04/17/23.
The screen multiple times has glitches and froze. We weren’t able to use backup camera, turning lanes cameras or clearly see our screen. We couldn’t access any of the icons that allowed us to control any features of the car. It was like driving blind.
The second row passenger side seat actuator will randomly engage or disengage the seat. This was discovered while a booster seat with a child was using the seat and the seat unlatched itself while driving causing the seat back to flip forward with the child in it hitting the back of the front passenger seat. The seat actuator continued to randomly engage or disengage the seat with or without passenger or when completely stowed flat. Once the initial incidence occurred the manufacturer was notified and a mobile service visit occurred. At the mobile service visit the technician verbally indicated they've seen another similar incident report on a similar make/model with a similar VIN number. The mobile technician was not able to resolve the issue and a follow up service appointment is scheduled. The incident reoccurs randomly with no warning lamps, messages or symptoms prior.
1: While using cruise control, the car will apply emergency braking without cause or warning. I have learned that this is known as phantom braking. 2: Occupants in my car as we'll as anyone driving behind me are at risk of injury or death due to this issue. 3: The Tesla dealer determined there is no hardware issue. Stated further that a future software update may correct the problem and referred me to the users manual for limitations of the system. 4: The car has been serviced by the dealer without resolution. 5: No warnings or messages are executed by the car prior to the phantom breaking. *** Note *** At minimum there needs to be a warning that must be acknowledged by the driver whenever cruise control is activated like the warning messages post by some cars when activating a GPS system.
The contact owns a 2023 Tesla Model Y. The contact stated that while the vehicle was parked with the car seat installed, the technician pulled on the seat belt and the rear passenger’s side seat belt buckle fractured. The vehicle was not diagnosed or repaired. The manufacturer was notified of the failure. The failure mileage was approximately 3,000.
Approximately 5 times while using Adaptive Cruise Control my 2023 Tesla Y will abruptly brake and slow down approximately 10-15 mph even through there are not roadway dangers. It is a startling event every time.
I bought this car on March 2023. The windshield blades won't stop running on auto. I kept it turned off, but when I engage the adaptive cruise control, it goes back on and I can't turn it off. I made a service request on May 10 and Tesla cancelled the appointment saying the issue was resolved because it was a software issue, not a hardward issue. I was advised that wait for over the air update. I have had 3 over the air updates and the issues continuous. This is no my first tesla and this was never a software problem with my previous car. I also have another tesla Y 2021 that doesn't have the problem either. I have contacted Tesla again and they don't have a solution. One of the main reason I keep buying teslas is because I have problems with my arms and need to use the adaptive cruise control with lane centring to easy my arm pain. It is a feature I bought and it has to work otherwise I don't want this car. what seems to be easy it is become a problem. I always happen to get disconnected when talking to people at tesla. They must really have phone problems.
Car just applying brake on its own when I was driving on freeway on April 12, 2023 at 1105AM and 115PM and on April 18 at 1414PM. I was lucky nobody was following me closely, otherwise it could be a different result. I was driving at around 70 mph on cruise control. This incidents happened on Freeway 5 in southern California and the weather was nice during that time. This issue could be accident waiting to happen.
Around 3:54 PM EST, I was traveling on US 1 Route 1 North Bound, and I tried to turn right into Target Complex (4196 US-1, Monmouth Junction, NJ 08852) from Route 1. I slowed down to turn into the complex, and my vehicle had Sudden Unintended Acceleration while I am turning into a parking location. It jumped over the curbs and hit a tree. Airbags got deployed, and the car was damaged badly. Insurance deemed it a total loss. I saved videos from the crash from the USB. I can forward or upload them.
On April 10th I received a notification from the car of a "Front passenger safety restraint system fault" and was asked to request service, which I did right away. Thus far I have been unable to receive any information from Tesla on this matter and they have not allowed me to service the car. This seems like a safety hazard from the description and I would hope Tesla would both share more information on this issue and help car owners who have received the warning. Right now it's unclear if the car is safe to drive.
On a multi day drive across the country I had multiple episodes of automatic breaking while on autopilot and using Traffic aware cruise control. In all instances there were no vehicles in either lane ahead of my vehicle. Braking episodes did not result in an accident but made the features essentially unusable.
When I use the autopilot system. The car applied automatic emergency braking abruptly even there is nothing in the front. This could result in the rear end accident. I recorded a video since it kept applying AEB. But your system does not let me upload it.
I had the car for exactly one week. I was driving the vehicle in the street, and my vehicle jerked and stopped in the middle of the intersection and a warning sign came on “rear motor disabled”. Then stated it two or three more times, stating the vehicle is safe to drive under 50 miles, and no steel grades. I do not feel safe driving this vehicle at all at this time, and feel that Tesla needs to replace my vehicle as the vehicle is a week old, and the motor isn’t working. This is a huge safety concern as it could’ve caused a major injury accident, especially if I was on the freeway.
The incident happened on 4/9/2023 at 2:30 pm. I drove in I-10 from Phoenix to California with Cruise Control and Autosteer ON. The speed was set to the highway speed limit. There were three other passengers with me. This seems to be related to the known issue of "Phantom Breaking" in Teslas. I was driving on a 4-lane road with no traffic, then the car suddenly reduced its speed from 65 mph to almost 30 mph, causing a sudden jolt to everyone in the car. There was nothing in front or reason for the car to break. No incoming traffic. It does not seem to have detected a "ghost" obstacle as an emergency. As there was no beeping or emergency message on display. No warning of any type. Luckily, no car was following me; otherwise, it could have caused a rear-end collision.
I was using the advanced cruise control (autopilot without steering assistance turned on) on a four lane highway with light traffic on a clear partly cloudy late afternoon. The speed limit of the highway was 75mph which was my set speed. For no reason at all my system slammed on the brakes, hard. Tesla refers to this as "phantom braking" according to online reports. This almost caused an accident as the car behind me had to very quickly swerve out of my lane. This is unacceptable behavior for an ADAS system to push the brakes so hard without warning or reason.
The car completely stalled on the freeway. Giving less than 20 secs of warning that the vechicle will shut down. I got over from the fast lane to middle lane and car shut down. I had to call 911 and tow truck. I towed it to tesla and they said the rear unit failed… car was just purchased new a few months before this.
I was driving on free way with Autopilot (Adaptive Cruise Control) set at 79 mph. Without warning, the Model Y suddenly quickly decelerated. There was no vehicle or barrier in front of me. I was able to quickly response and started to step on the Accelerator and noticed the speed dropped to about 70 MPH. There was no sound alert during this process. It was about 8:20 in the morning and I was driving east towards the sun. My daugher was with me in the vehicle and experienced this scare. There was no vehicle closely behind me, otherwise this may cause collision. The vehicle functioned normal afterwards.
At highway speed, using the autopilot feature for speed control only, the car applies severe braking when there is nothing obstructing the road or requiring brakes. This has happened dozens of times over the first several thousand miles we have owned the new car. We filed a report with Tesla and they said this is not a problem and is expected behavior. We are concerned this will occur with a vehicle behind us resulting in being rear ended.
I recently took a multi-day cross country trip to the southwest mostly along the interstate 40 but also along US and state highways and multiple times each day I experienced what is often called “phantom braking” while at posted highway speeds (typically 65 to 75 mph). These occurrences happened 3 to 5 times each day while I was either using adaptive cruise control or autopilot features on my vehicle. At times this occurred soon after passing another vehicle or semi truck, but sometimes it happened without any other vehicles in sight of my vehicle. The phantom braking occurred without warning and caused a rapid deceleration of my vehicle, at times risking an accident due to the vehicle behind me having to abruptly brake even though there was no indication of any sort of road hazard to alert them of a need to brake. Due to this random, unwarranted braking risk I was forced to keep my foot hovering over the accelerator so that when it occurred I could quickly press the accelerator to override the phantom braking. I tried to monitor the road conditions when the phantom braking would occur and the only seeming commonality may have been when it was a bright and sunny day with slight natural undulations in the roadway, but I cannot be absolute sure of those observed conditions being causal or just coincidental to the events. What does seem to be consistent was I did not seem to experience phantom braking during overcast conditions.
Drivers side mirror glass vibrates while driving causing blurry image, and has done so since vehicle was new. Tesla service center indicated that it is working as designed. However the passenger side mirror doesn’t vibrate at all and the image is perfect. This is an active problem on my vehicle that Tesla cannot repair. Tesla needs to develop a fix for the drivers side mirror. According to various Internet discussion boards, this appears to be a common problem.
Vehicle steering lost power steering and the steering is stuck in straight position only while driving. Vehicle can travel only straight line forward or backward and cannot turn. Powering off and on did not help resolve the issue. Had to get the car towed to Tesla service.
The car has been giving me fake forward collision warnings. Even in parking lots
It is known as phantom braking that many other people have reported. I bought this car for about two months and has drived for about 2000 miles. It has happend 3 threes to me. In all three cases, the car was on adaptive cruise control at about 70 miles per hour. There was no car within 200 yards. The car suddenly braked and the speed went down to 30 miles per hour in a couple of seconds. I had to intervene and cancel the adaptive cruise control.
My 2023 Tesla Model Y Extended Range (purchased 3/13) is experiencing phantom braking when autopilot (adaptive cruise control) is engaged. At random times and with no apparent obstacles ahead, the vehicle performs aggressive braking without warning. This has occurred at various times of day on divided freeway. This is consistent with what other Tesla drivers have described consistently for several years. I reached out to Tesla for assistance, and they advised to "when engaging Autopilot, make sure the vehicle distance is set above a 4. It can get a little breaky below that" effectively acknowledging this as a known issue. They could not provide guidance on a planned fix. This is a significant safety issue as it creates the risk of being hit from behind if another vehicle is following when the emergency braking occurs. I'm reporting since this clearly is a known safety issue that evidently has gone unresolved for quite some time, and which was not disclosed to me at time of purchase, despite a pattern of issues.
Air Bags malfunctioning, if fact, not even working on this 2023 Tesla Model Y.
on 30th MAR 2023, when my wife drove on to our drive way with gentle speed, car rammed into closed garage door and broke the door and rammed into living room wall with breaking room entry door . there is neither emergency breaking /stop nor collision warning . it damaged wall , garage door and more living room property damage with addition to damage to the car. i am really worried about the safety of the car..
Emergency bracking occured on multiple occasions during trip. When there was nothing to brake for, the brakes were automatically applied, sometimes severely. The car is available for inspection if requested. With unexpected hard braking, we could put someone following behind in danger of a rearend collision and possible injury to us and them. The problem has not been confirmed by a dealer or independent service center. The vehicle/components have not been inspected. There was no warning prior to this unexpected braking.
Driving eastbound on I-70 in Nebraska March 29 I had about 4 incidents of sudden braking with no obstacles or even any cars or trucks in front. All oncoming traffic was far across the median. In each of these incidents, there was a dark patch across the highway, probably from recently applied asphalt. One of these was severe enough that there would have been an accident if there were any closely following cars. In that particular incident there was a small concrete bridge that had an obvious dark black surface - probably from resurfacing. I purchased "full self driving" for the car a few days ago. The car was in auto-steering mode and was capable of changing lanes initiated by the turn signal, although there was no lane change at the time of these incidents. I have not reported this other than through this web site.
Phantom/False Forward Collision Warning 1. March 28, 2023 While driving on Alder Woods Dr, Fairfax, VA at 6:21PM Forward Collision Warning activated automatically for a parked car on the street 2. March 28, 2023 While driving on Alder Woods Dr., Fairfax, VA at 6:22PM Forward Collision Warning activated automatically for another parked car on the street - same trip as above at 1., but additional incident 3. On March 25, 2023 on Alder Woods Dr., Fairfax VA Forward Collision Warned activated automatically for yet another parked car on the street. When forward collision warning activates the screen turns red, there are high sounding alarms inside the car, it distracts the passenger by creating an unsafe conditions for the wrong reasons.
I have seen this happen on several occassions during my trip from Austin,TX to Lubbock, TX. I out the car on Cruise Control and the car has applied emergency brakes suddenly several times even when there is no cars or other obstacles around. Even after I tried disabling the "Automatic Emergency Braking" feature, I am still facing this issue on several occasions. I have taken 2 trips so far in the past 2 months from Austin to Lubbock (about 360 miles one way) and I ahve decided the car is not at all reliable for me. I am even considering trading-in the car for another brand. This is real safety hazard.
New Tesla Model Y brake failed and accelerated resulted in a serious crash within 2 hrs of taking delivery. I believe it is acceleration functioning and brake due to auto pilot feature. Vehicle will be available for inspection after 4 weeks. After taking delivery of new Tesla Y, we drove to temple. While parking car accelerated and lost control of the vehicle as brakes were not working. It hit the temple wall and when we tried to backup to get the car out of the crashed wall, it got even worse. It reversed in lightning speed and hit another car, run over the bushes and huge palm tree across the street and finally stopped. This terrifying experience caused huge mental trauma, multiple property damage, and financial hardship that we are dealing with right now. Fortunately, there were no injuries. We lost all the trust and confidence in Tesla. We are NOT confident of driving due to huge safety concern. It is a very important safety issue considering the customers at large. Tesla service has not worked on the diagnosis. We tried reaching out Tesla service to investigate the failure. There is no response from them and they keep cancelling the service appointment. On top of it, there is no appointment available in body shop for a month. This incident was reported to police. One observation was made during the incident where Auto pilot beta was displaying on the dash board monitor without invoking any self driving options.
Water intrusion rear tail lights. According to Tesla service it is within Tesla specs.
Auto wiper not functioning properly heavy rain with low speed wiping and full blast with light rain
Took delivery of new Tesla Model Y on Friday, March 24th. While driving it to my residence (205 miles away) noticed that the alignment was off and vehicle pulled to the left at highway speeds on flat, level sections of roadway. I was not informed of the 100 miles or 7 days to report issues with vehicle. I scheduled service with Tesla with odometer reading of 348. Tesla service verified the issue and corrected it but charged me $275 for an issue that was present from factory.
Phantom braking has occurred on 4 different occasions nearly causing a rear end collision during one of them. On all occasions it occurred with no other car in front of us. Tesla Service was contacted through their app for service and we were told this is a known issue and to simply apply pressure to the accelerator. No warning lights or other was given before the incident. The problem has not been reproduced or confirmed by Tesla or independent service center?
The contact owns a 2023 Tesla Model Y. The contact stated that while driving approximately 62 MPH with the autopilot mode activated, the vehicle experienced phantom braking. There were no warning lights illuminated. The manufacturer was not made aware of the failure. The vehicle was not diagnosed nor repaired. The approximate failure mileage was 3,000.
This is the second instance of this issue happening, first at 1800 miles ODO, and second at 4690. Both instances, when driving, the vehicle suddenly and violently shuddered to a stop. The first instance happened as I pulled out from a stop sign to take an unprotected left onto a street. In the middle of the intersection, the car shuddered to a stop violently and a lot of warnings and alerts came up on screen. Cars moving both directions had to suddenly stop to not hit my car dead in the intersection. The gear stalk inputs to put the car in drive or reverse and allow function of the vehicle did not work. I pushed the park button on the right stalk and then moved it into drive, which it allowed. I was then able to limp the car out of the intersection (with power vastly reduced and speed reduced) and await a tow. The second instance I had pulled into a parking lot at around the same speed (~15 mph) in which the car again shuddered violently to a stop with no ability to override. Traffic behind me had to brake as to not hit my vehicle which stopped in the middle of the lot. Again the car was inoperable until this time locking and unlocking the car from outside of it and reentering to limp it to a parking spot for tow. Safety of myself and others were put severely at risk suddenly and without warning from the car stopping itself. If this were to happen in other, less fortunate scenarios, risk to human lives is great. Problem has been confirmed by Tesla service center, but not reproduced while in their possession. The vehicle was inspected by Tesla service after the first incident and was explained to have found no faults with any hardware, but found a corrupt line of code in the firmware (no update or firmware change was noted to be available). Warning messages and alerts populated at time of incident and no sooner (no warning is given prior to issue). Alerts include power reduced, rear motor disabled, vehicle shutting down, automatic emergency braking disabled and more.
Tesla model y 2023 phantom braked on the intestate. The emergency braking system and adaptive cruise are not correct. I almost got hit because my tesla auto pilot braked on the interstate for no reason.
Took 1st long trip with the Tesla. I had "phantom braking" happen 5 times while on the trip. Once on the way to the destination. Then, it happened 4 times on the return trip home in a span of 2 hours. This was in the first autopilot mode (Traffic-Aware Cruise Control), not the Autosteer version. Each time it happened on an open road in a rural area at a speed of around 70mph. This puts my vehicle and others at risk where someone that is close behind me would be surprised with a sudden decrease in speed and cause an incident. It also is nerve-wracking to the driver and passengers of the vehicle. This problem is known within the Tesla community and was reported to Tesla. The vehicle was "inspected" by looking at incidents in the cars software log. The Customer Service agent could not confirm any incidents and asked that I file a "bug report" through the car immediately after it happens again. There are no symptoms or warning of the problem - it happens at random.
Component/System Failure: Brakes spuriously slam on when traveling at freeway speeds. Safety concerns: People behind me possibly slamming into me, in addition, my terrified reactions to an unexpected/unseen concern. Vehicle has not been confirmed by dealer or independent service Vehicle has not been inspected my manufacture, police, or insurance representative there is no warning as to when they may happen. first appeared in March 2023 and has happened about five times throughout the various months since then,
While driving South on I75 near Tampa Florida using Autopilot the car suddenly decelrated to 10mph. There were no cars next to me and we were passing through a 6 lane area where the road splits. No distinguishing features, no cars on either side and we were heading slightly up hill. It seems like the cameras could only see the sky. Fortunately there was no car directly behind me. Very scary.
New car with less than 1800 miles. Multiple times where The car will frequently and unexpectedly slam on the brakes while in autopilot on the highway. Almost caused an accident twice and I was terrified. Did not know about this problem but this can cause an accident.
While driving acceleration pedal got loose and fell on the floor or the car.
I got the delivery of the car VIN [XXX] on March 10 7 PM and got the car out and drive to home from Kennesaw tesla delivery center. On my way home the car stopped right after crossing the light signal at the intersection of Medlock bridge rd. and parson rd. And it does not move, I had to turn on the emergency lights to save myself from accident and the cop had come after some time. We were lucky to save ourselves without an accident as the car stopped all of the sudden and there was no car behind. I think this is a problem with the car and needs to be inspected and nobody should be in this situation in the future. I want to raise this as a safety concern please for Model Y in the very first hour after the delivery of the car. INFORMATION Redacted PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6).
The contact owns a 2023 Tesla Model Y. The contact stated while driving 40 MPH with the Autonomous Self Driving feature activated, the vehicle abruptly stopped without warning. The contact was then able to accelerate, and the vehicle started operating normally. The vehicle was not diagnosed. The dealer was contacted, and the dealer stated that should the failure recur, the vehicle needed to be diagnosed. The manufacturer was contacted, and the manufacturer stated that the vehicle needed to be diagnosed. The failure mileage was 1,250.
We had 2 phantom breaking occurrences while in Autopilot traveling at 75 MPH within 10 minutes. The first time we were passing a pickup with trailer on the right and the in the second a pickup was passing us on the left. in both cases there were no other vehicles or objects on the road in front of us. If there was a vehicle following close behind us there could have been an accident. We did not come to a complete stop but I am not sure if that was because of Autopilot or my reaction. There were no other warnings.
Vehicle brakes suddenly while driving even when there are no cars or obstacles of any kind in front or even in the distance. This has led to several events where I was almost rear-ended because of how strongly and violently the car was breaking on the highway. Last example was on March 5th 2023 at 1:19 p.m. while driving on interstate 90 in Ohio going westbound, there were no cars in front or in the distance in the lane. The car suddenly applied the brakes in a very strong fashion in the middle of the highway. There were at least five examples just like this one which happened prior.
While on the highway i lost all power steering and acceleration and breaking power. no blinkers or access to my cars screen would not put car in park while on the side of the highway. i had to cross 4 lanes with no power, blinkers, minimal breaking. The control computer completely broke and stopped working. this was a week after delivery. Telsa towed my car for free and fixed the issue. But did not give anything for almost killing me on the highway. what repair notes say they fixed/replaced. Steering Colum control module bolt Rc Airbag harness Faulty SCCM,CAN
I was driving on East on I-8 and was East of Yuma, AZ. I was using the enhanced auto pilot feature. I was traveling 70 miles per hour (5 below the speed limit) There were no autos in front of me and nothing on the shoulder. There was a semi truck behind me (I am not sure how far back.). My car suddenly put on the brakes and rapidly slowed down. (I am not what speed) The de-acceleration was so severe that my coffee cup too flew onto the floor. The semi truck was able to move to the right lane but blew his horn loudly. It was extremely scary and dangerous for both of us. I have done some research and this seems to be a problem that many Tesla owners are experiencing. This issue needs to be fixed immediately.
The adaptive cruise control system routinely applies the brakes when there is no lead traffic on a completely open highway. I’ve had multiple instances where the car decelerated from 75 to 45 mph before I could react and press the accelerator to correct the state. During these events, the car does not indicate any type of emergency warning (no forward collision warning, no emergency braking warning). This poses enough of a safety risk to cars behind me that I will no longer use Tesla’s adaptive cruise control feature.
