On my way home from a trip on Tuesday 4-14-26 at about 60 mph with cruise control on the truck downshifted and turned cruise control off and a small wrench came on display. I made my way to next town at reduced speed, wouldn't get out of 5th gear, found a ford dealer had them scan and check out. They found 2 codes P061B:00-24- pcm and P0722:00-68 They cleared codes and drove truck with no return of trouble. I resumed my trip home and same thing happened about 1 hour later, stopped for fuel, and returned to the road with no problem or wrench on display. Came home went to my local transmission shop and he informed me that this was a known problem and warranty has been extended to 10years or 150k miles. Went to my local Ford dealer and they didn't know anything about extended warranty coverage

When I come to a stop at a stop sign or waiting for traffic to pass to make a turn, and try to accelerate, my truck does not accelerate as usual. On the dash it shows that the gear shifts into 5th, from the get go, after having been at a full stop. At some point it then down shifts from 5th to 2nd. A wrench appears on my dash as well. A very loud fan noise also came from my engine area. This was highly dangerous, because as i was trying to turn left on a fast paced road, my truck could not / would not accelerate as normal and i was kind of stuck sideways on a two lane road. The same happened when i tried to turn off the road. I was making a left turn. Unfortunately i had to come to a full stop to let the other cars pass, so i could make my left. Once i tried to accelerate to make my left hand turn, it took quite a while for me to make the turn and i was kind of stuck sideways on the road. The gears were not shifting properly throughout my drive from my house to my destination and back. I stopped using it once i got back to my house.
In 1 week 2 unexpected shifts into neutral occurred. On the third occurrence the wrench light came on and the vehicle would not shift out of third gear. I pulled code P1702. I will be replacing the transmission molded lead frame this weekend. It is not safe to drive.
While driving at steady speeds around 50–65 mph, the vehicle intermittently feels like it shifts into neutral and the engine revs unexpectedly. After a moment, the transmission re-engages. This creates a safety concern, especially at highway speeds. Issue appears consistent with reported problems involving OSS (Output Shaft Speed) sensor or transmission lead frame signal loss in 2015–2017 Ford F-150 vehicles currently under NHTSA investigation.
Multiple front brake line ruptures on 2016 F150 putting safety at risk. 7/26/2023 front passenger side brake line rupture while driving requiring new brake line. 4/6/2026 front driver side brake line rupture while driving requiring new brake line, replaced front passenger side brake line again because it was cracked and about to rupture again. Ford is using defective brake lines, brake lines should not be considered a routine maintenance item like a car battery. This is putting my family at risk and other drivers.
Transmission is going down in gears at high speeds and a wrench is showing up
Saturday 4/4 I took the truck to get a tire repaired. It would not initially start. 5 minutes later it finally did. Then a few blocks away the trucks internal lights started flashing and it jumped from drive into a lower gear. I thought my drive quit working. I pulled over and had reverse and low so I attempted to put in drive, still wouldn't go. After about 5 minutes I was able to get the truck back into drive and get it home where it sits today. I just bought it 1 1/2 months ago. It happened to my son also on Thursday 4/2 but I just found out about it. He was going highway speeds when lights started flashing and truck jerked from drive into a lower gear. THIS SHOULD BE A RECALL. THIS IS VERY DANGEROUS. I want to know what you are going to do about it? I just paid $22,000 for this truck. I shouldn't have to deal with this. My son looked it up and says it is P0707 lead frame failure or output shaft speed sensor. Whatever the case, it would be nice to know what can be done about it other than switching to manual dirive.
Odometer Fraud. The contact purchased a 2016 Ford F-150. The contact stated that while purchasing a warranty for the vehicle, it was discovered that there was a mileage discrepancy. The vehicle was a dealer sale. At the time of purchase, the mileage was 149,000 and at the time of purchasing the warranty, it was discovered that the mileage was 146,000.
Oil pan leaks. The F150 just crossed 70,000 miles. At just under 60,000 miles the truck had what the Ford Service tech described as a weap from the oil pan. He said it was a costly repair so just to watch it. At 65,000 it was a steady drip when the oil was warm. Now at 70,000 the Ford dealer is recommending a replacement. What I have learned is this is a common failure on 2015 to 2017 F150s with the 2.7L engine. I suspect this was happening earlier but the Ford Dealer was relucant to bring it up as it would have at the time been covered under warranty. I have also now learned Ford is on its 3rd TSB for resolving this common leak at the oil pan. Hopefully this will warrant a recall response from Ford. From research this is a very common failure and engine damage will occur if the leaking oil pan is not repaired. Please investigate as I suspect there are numerous complaints. Thank you.
Rear backup camera failed and is just showing a blue screen or black screen with squiggly lines.
Truck was repaired under recall 20S31 to repair the master cylinder issue in November 2020. Just the other day the master cylinder went out again and my brakes went out while driving. There clearly was an inadequate repair and Ford so far is not open to covering the expensive repair costs to this critical safety issue.
While driving at highway speed, in traffic, the transmission down-shifted without warning almost causing me to lose control and almost rear ended by another vehicle. The vehicle then lost power and would barely make it uphill. After parking safely and shutting it off, the check engine light came on and it would not crank and would not show what gear it was in. According to the Ford dealer, where I had it towed for repair, it suffered a lead frame failure and pcm failure and cost over $2300.00 to repair plus a $240.00 tow bill.
There currently is a NHTSA inspection investigation going for this issue. I only have 75k miles on this truck. The shifting is off. Sometimes it goes out of gear. I had to stop and restart everything to get it home. Getting on the freeway it went to first gear and I had to pull over. Made a weird noise in shifting. The shift illumination goes on and off. The shifting was acting definitely not right.
My Ford F150 randomly won't start.It also randomly down shifts while driving.There have been times I've been driving 50mph and the transmission starts down shifting on its own.
The truck out of nowhere downshifted when I was going down the highway at 60 and locked up the back tires, and then it went into limp mode and woundnt drive very fast. The mechanic shop said that it was probably that the TRS and the 6R80 transmission were currently being investigated for these safety concerns and i couldnt agree more. I could have hit someone or even got hurt my self.
Today I was driving my truck at 55mph on a local highway and all of the sudden I felt/heard/experienced a massive clunk out of nowhere - almost like the truck tried to downshift into 1st or something similarly jarring. It only lasted a second, and then I got a dash light on that said Service AdvanceTrac. Based on the service codes I'm seeing (two of them that are both UO100 codes which indicate loss of communication), I did some research and found out about the OSS sensor issue and think that's what is going on with my truck too. Unfortunately I'm not sure if I'm going to be able to get a Ford dealership to do anything about it since there isn't an official recall, and I'm afraid to keep driving it as this is likely going to keep happening - but fixing it myself is going to be expensive I think. It is available for inspection upon request. I felt that my safety was and continues to be at risk - downshifting into 1st gear, or whatever that was, while driving high speeds is dangerous and could potentially significantly damage my vehicle and myself. I have not taken it anywhere yet to be reproduced or confirmed - just plugged my scanner in and did a bit of research. No one has inspected it.
The drivers side door latch froze and stopped working, the door wouldn’t close and the lock would glitch when trying to lock or unlock it, once the door did close it reopened while driving, so it never really closed all the way, and it started working just fine once the weather warmed up. There are recalls on this issue but not for my vehicle specifically.

There was no warning light illuminated. The vehicle was towed to an independent mechanic, who replaced the electric power-assisted steering (EPAS) system. The manufacturer was not made aware of the failure. The failure mileage was approximately 81,000.
I’m reporting a serious transmission issue with my 2016 F-150. While driving, the 'wrench' light suddenly pops up on my dash and the truck starts shifting erratically. Sometimes it feels like it’s searching for a gear or jerking unexpectedly. I had the codes scanned and found P0706, P0707, and P1921. These are all related to the Transmission Range Sensor (TRS) signal. I know the NHTSA is currently investigating this exact failure under EA26001 because it causes the truck to lose track of what gear it’s in. Even though my tires haven't locked up yet, the erratic shifting is making the truck unpredictable and dangerous to drive in traffic. It’s clearly the same Lead Frame defect that is being investigated for a recall.
Date of occurrence 3-11-2026 2016 F-150 5.0 6R80- Vehicle down shifted to first gear sporadically locking rear wheels up around 30-35 mph. Wrench light came on saying see manual. Shortly after the check engine light came on. I came to a stop and the truck wouldn’t shift properly. When I would put it in reverse it would slam into gear. The dash would not show if I was in park, drive or reverse. I shut the vehicle off and it would not restart or turn over. I was only a mile or two from home. Was able to get it home in neutral with the truck off with a short tow. The truck started fine and acted fine the following day. I thought it was just a glitch so I drove it around in the driveway. And once the truck and trans got up to full operation temps the same problem occurred. Thankfully it was still in the driveway so that’s where it sits now. It is very unsafe and unstable to drive. 130,000 miles. I was able to hook my code reader up to it and now I’m getting codes P0707/P0706 all Transmission range sensor codes. P0707(1/ 6) Stored Transmission Range Sensor 'A' Circuit Low DTC Severity 2 of 3: This fault may pose damage to components and should be repaired as soon as possible.//// P0706(2/6) Stored Transmission Range Sensor 'A' Circuit Range/Performance/// P0707(4/6) Pending Transmission Range Sensor 'A' Circuit Low The vehicle is still at my residence I have not moved it since the occurrence. Has not been inspected by dealer or mechanic. Will have to arrange a flatbed tow to a facility so there is no risk of injury or causing any more possible damage if any exists.
2016 F150 5.0L 6 speed transmission. Lead Frame failure, Output Shaft Speed Sensor failure/lost communication with PCM. Truck had poor shifting, poor driving, eventually locked in 5th gear and went into limp mode. Only downshifted into 3rd When slowing to a stop. Continued limp mode. Dealer replaced lead frame after diagnosis of the failures.
Transmission conductor plate (lead frame) failed and had to be replaced this week. Vehicle has only 54,000 miles on it. Total cost to repair was $1,167..
My truck is equipped with a 6R 80 transmission and as I was pulling off an offramp onto interstate 295 my transmission shifted into first gear and my truck skidded and came to a stop sideways with oncoming, traffic, barely avoiding me. The rear end of my truck literally locked up.

going down the road the dash quite showing what gear i was in and then the truck downshifted to first or second gear while i was traveling 45 MPH. Light then went off and could see gears again. Came back on within a mile. I limpt the truck home where it is parked until i can get it into the shop to be repaired. codes were p070600-26, p07000-26, p170200-2e, p070600-27, p070700-27, p170200-af. codes will just go away if the wrench dash light goes off and truck will drive normal. Wrench randomly comes on and downshift will happen before i am able to slow down.

The rear window made a loud popping noise. It buckled and cracked. The window appeared to be sliding down. A lot of the edge pieces where the seal is broke off. Within an hour a large hole appeared as if it was hit by something. The Ford dealership in Milford, CT told us to call our insurance company and get it fixed by Safelight. We talked to our insurance company by phone. No one inspected it. No warnings or other problems.

The contact stated that while stopped at a traffic light, the light changed, the accelerator pedal was depressed, and the vehicle rolled backwards unexpectedly while in drive(D). The vehicle was stopped and shifted to park(P) and then shifted into drive(D) and responded normally. While driving and close to the residence, the vehicle started to lose motive power. The vehicle responded as if it were shifted into neutral(N). The contact arrived at the residence and turned off the vehicle, and the vehicle was difficult to restart. The "WRENCH" symbol was displayed. A handheld scanner was used to retrieve DTC: P0706 and P0707 for transmission range sensor failures. The vehicle was not taken to a local dealer or independent mechanic. The vehicle was not diagnosed or repaired. The manufacturer was not informed of the failure. The failure mileage was approximately 66,000.
Truck shifts down without warning causing the truck to skid and squeal tires. This could cause a crash is definitely causes damage to the transmission.
Truck down shifts randomly, causes a shutter, and has a hard time accelerating. I had a sensor replaced inside of my transmission along with new transmission fluid put in and it failed to fix my problem.
The transmission is clunks on random shifts through gears. Changing the shift selector results in a jolt and then it pops into gear. This truck has had new tranny wire lead replaced at ford dealership because of the same issue in the past

While driving at approximately 55 mph the vehicle began to behave strangely. The "D" light for drive and the gear indicator began to flash. Then the transmission abruptly down shifted to 2nd gear from 5th then lost all power before returning to somewhat normal operation for a short period of time before happening again. The issue occurred multiple times in a 5 mile trip. If there was anyone close behind me it could have resulted in a crash, or it could result in myself losing control of the vehicle. The vehicle has not yet been inspected by a mechanic. There were no warning lamps or any messages prior to, during, or after the issue.
My 2016 F150 with a 6R80 transmission will randomly fail to go into gear resulting in my vehicle going into neutral and rolling. It will randomly go into neutral while driving down the road which makes my car roll forward unexpectedly, it will randomly when you shift into park it will go into neutral and roll, it will also downshift randomly at random speeds, it has caused me on more than one occasion to almost roll into another vehicle due to it unexpectedly going out of gear without notice almost causing an accident. I believe it to be the lead frame in the transmission. It has been replaced twice with oem ford parts and has failed once within a week and the second time failing the next day, all work done at a licensed service shop, inspected and has failed completely regardless of parts being replaced,

The first sign of trouble with my 2016 Ford F-150 was that the blind spot sensor failure light came on while driving. Then the engine warning light came on and the gear indicator on the dash went blank. Also, while the truck was in forward driving motion, it lost throttle control. After stopping the truck and shutting it off, it would not restart. The check engine codes were 0706 and 0707. After clearing these codes, the truck started again, but repeated the same issues three more times in one week. The truck was taken to the Ford dealership and confirmed that the lead frame needs to be replaced.
while driving at about 50 mph, the truck will go from 6th to 1st, and lock up the rear end briefly. I'm the mechanic filing this for my customer, seems to me, its the lead frame failing in the trans, and they have done nothing about it, customer says ford worked on this transmission, the only updated part i found was 1 extra magnet. old part number for the lead frame, and a known failing lead frame #, I'm pretty sure you guys have a open case for these trucks.
Transmission will randomly shift gears. At speed sometimes it will shift into a lower gear causing rapid deceleration. From a start it will jump to 5th then back to 1st and stall the rear tires.

My truck has had multiple issues with randomly downshifting while on the freeway, i'm getting a code that leads back to the "Lead frame" wiring every now and then. The code comes and goes without having it cleared. I've had this issue before and have replaced it with the Ford Manufactured part. Its now doing it again but now more frequent not even a year and a half after replacement. I have been almost rear ended multiple times due to it locking up the rear tires and once it locked up the tire on ice and it caused me to slide into the median. There are multiple reports of this issue currently on many Ford Truck Forums. I am currently in the process of contacting Ford again as last time they said to just replace the wiring. My truck is equipped with the 6-speed transmission
I have a 6-speed (6R80) transmission. My V8-FORD F-150 2016, truck transmission has been experiencing SHIFTING issues for quite some time now. I reading now that all of these same problems that has been going on with my truck might be a recall. My truck downshifts very aggressively and I was told it does not need to be replaced but it could be something electrical. I have taken my truck to numerous mechanic shops and all of them have told me the same thing it could be something electrical but they cannot figure out what that might be without just replacing the entire Transmission which cost $8K to do, this is the only issue that has been wrong with my vehicle. I cannot drive over certain speeds on a highway and sense this might become apart of this recall and every issue that I have been experiencing, I would rather have it fixed if it is a recall instead of replacing an entire transmission if the issue is ALREADY known by FORD. Multiple times while driving my 2016 F-150 I would lose all power to my wheels. The RPMs would rise but no power would be transferred to the wheels. This happened one time while on the on ramp at 50+ MPH, a second time while drive down a city street at about 30 MPH, a third time while starting from a stop light, and a fourth time again when driving in the city at about 45 MPH. The check engine light would come on for a minute or two and then turn off. After the vehicle did this for the fourth time the check engine light stayed on. After reviewing there are two error codes the vehicle relayed that the mechanic said are related. During these 4 instances there were two time (highway on ramp and city street at 30 MPH when the loss of power to the wheels caused the vehicle directly behind the F-150 to have to swerve around to prevent a collision. P0722 (Output Shaft Speed Sensor Circuit No Signal) and P061A (Internal Control Module Torque Performance). The vehicle is available as well as the Scan results from the OBD2 sensor from 4 February 2026. The vehicle was not taken to an auto shop due to the check engine light not remaining lit prior to the 4 February incident at that point it remained lit and an OBD2 scanner was used to read the error.

Additionally, the transmission unexpectedly shifted upshifted and downshifted on several occasions. The vehicle was turned off and restarted to temporarily correct the failure. The dealer was contacted; however, the vehicle was not diagnosed or repaired. The manufacturer was not made aware of the failure. The approximate failure mileage was 40,000.

Was traveling down the road and my truck randomly downshifted into 1st gear and caused my rear wheels to lock up and i almost went off the road..occasionally 9n way back home gear shift selector was randomly cycling back and forth and truck would seem like was neutral and would suddenly go back into a random gear.
