NHTSA Owner Complaint Log
This page lists owner-reported complaints filed with NHTSA for the 2019 Lexus RX. Complaints are unverified consumer reports submitted to NHTSA and do not by themselves prove a defect or defect rate.
My Lexus 2019 RX450H is making a squishing squeaking brake actuator noise when pressing the brake pedal. I located well publicized TSB’s documenting the awareness by Toyota/Lexus of the problem. (TSB: MC-10245447-9999) Brake Squawk/Knock Noise. I checked with my local Lexus dealership to see if my vehicle had been repaired. It had Not!. Temecula Valley Lexus, 42081 Dealer Drive, Temecula, Ca. 92591. (951)365-6834. I scheduled an appointment for repair on 1/26/26. They rejected making the repair because my vehicle DID NOT have any codes in the computer OR any lights on the dash. They did in fact, confirm the brake actuator noise. I reject that concept because WHO could know ahead of time if a part experiences a (1st time) total failure without throwing codes and turning on dashboard lights? As I read the symptoms of severe problems experienced in a higher level of failure by brake actuators, I would not want to have to experience a crash accident in which the actuator would operate correctly in a hard stop condition or have ABS failure and brake lockup occur. Personal harm and property damage should not have to be an indicator if I can have the manufacturer cover a repair. Please advise What I should do to increase the response in the direction of getting this known problem repaired. Sincerely, [XXX] [XXX] [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
I was dropping my husband off at cardiac rehab. I was stopped and had my foot on the brake suddenly the car launched forward into the back of a car parked in front of me. The other driver got out of the car and said he wasn't hurt. We were able to drive the car home stopping 3 times to pull pieces of the car off that were dragging on the road. We took the car to a mechanic at the end of the road. He scanned the codes for us. Pre-collision 2 ClAll ; Intuitive Park Assist ClAll Front Sensor Comm. Circuit ; Front Recognition Camera ClAll ; Lane Keeping Assist ClAll Front Radar Sensor Incorrect Axial Gap ; BCM B124B Light Source Sensor Open/Short. The insurance adjustor totaled the car and they picked it up today. No warning lights showed up on the dash until after the accident.
While driving roof glass just exploded. Sounded like gun being fired. No one was around us and glass blew outward. Almost had an accident from the explosion.
Brakes making honking sound when applied even when stopped. My brake actuator is failing and has been confirmed by the LEXUS Dealership. Noise and stiff breaks has been going on for a while. There is no recall, but Lexus put out LSB to dealerships May 15, 2023 and revisions earlier. This information should be provided to owners IMMEDIATELY, if the actuator fails, this is a safety issue.
Brake actuator defective
For some unknown reason the car will suddenly display multiple warning lights, check engine, service needed, traction control off/inactive then the car will go into reduced engine power mode which prevents acceleration and causes the car to feel like it’s going to stall or shut off. This has happened several times on major high speed highways causing other cars to slam on their breaks, drive recklessly to avoid my failing car. My daughter has been in the car several times when this happened and it took everything in my power to not collide with another vehicle. We have taken the car to the Lexus dealership every time and they have tried to fix the car over a dozen times but it keeps happening. They don’t seem to know the issue or cause, trying several different solutions. How can a car manufacturer not know what is wrong with their own car not how to fix it? What am I supposed to do, just keep driving it and pray an accident never happens? There is no warning when this happens and no way to keep the car from swiftly decelerating or keep the car from using “minimal engine power” which I’m to,d is to protect the engine but I’m more worried about us. The date below is for the most recent incident.
My vehicle has an intermittent check engine light along with an ICS malfunction code that pops up on the dashboard. The car doesn’t want to turn on after every gas fill up. After taking my car to a couple of independent shops, they determined that the issue with the car was the high pressure fuel pump. The suggested, I take it to the dealer as there was TSB bulletin on it. Furthermore I still had the letter from Lexus stating the issue. The local dealership refused to do the work and suggested other work after charging a huge amount for the car inspection. Lexus Corp wouldn’t help on the matter. There are 4 other complaints on this same issue and the dealers and the Plexus Corp are getting away with not calling this an actual recall because the government hasn’t stepped in.
Unintended acceleration causing accident March 2, 2023. Low speed frontal collision avoidance assist did not engage causing accident June 11, 2024. August 2023 I agreed to video and Lexus is not providing video of accident June 11th, 2024! Recall on fuel pump and software replaced but still BAD. Radio off. Heard cybercrimes of communications systems for video game data by Meta.
Driver side upper frame has cracks start from where driver sits back to rear passenger seat.
I’ve approximately the last 45 thousand miles, Lexus dealers have replaced the brake actuator and other components of the brake system on this vehicle at least 6 times due to noise issues. Lexus has stated that this is not a safety issue, I have an open ticket with Lexus, but yesterday I had to slam on the brakes and did not get nearly the response that should have come right away. There are no warning lights, as far as I can tell the traction control didn’t engage, and eventually the car suddenly started breaking much harder, but it was very delayed l, and disproportionate with any additional force applied. Lexus knows the brake system is an issue in this model to the point where they are covering the system for 10 years or 150,000 miles, but to say that it isn’t a safety issue seems short-sighted. This system works in conjunction with the radar sensors adaptive cruise control, and I’ve had. This car has also had a number of seeming electrical issues, with every door lock actuator replaced at least once, some of them more than once, and the infotainment system shuts down and reboots randomly or freezes.
Codes P219C & P0301 have continuously came on , these lights MIL have made the vehicle go from AWD to 2 wheel drive while in motion. [XXX] vehicle was taken to Bay Ridge Lexus. In brooklyn NY. They over looked this issue and stated it was something else. This has been confirmed via multiple diagnostics machines. Lexus of Edison has also decided not to cover this repair under warranty . The danger of going from 4 wheel drive into 2 wheel drive while in motion is extremely dangerous . These warning lights/MIL once activated has also caused the vehicles sonar to fail cause a serious safety issue. My vehicle is available to be inspected as needed. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
It’s the third time that the transmission has hard time to move out of from either 1st or 2nd gear. The car jerks then plunges forward then I could smell burning smell inside the car after 5-15?seconds of the incident. The car only has 33,000 miles.
While vehicle was being driven into the garage the vehicle suddenly accelerated on it own without driver pushing on gas pedal. Vehicles automatic emergency braking did not engage. Vehicle then struck steel shelving and wooden cupboards causing considerable damage to vehicle. The body shop where the vehicle was taken for repair experienced the same surging phenomena when they moved the vehicle. There were no system warning lamps visible prior to the episode. The vehicle has not been inspected by Lexus.
Lexus 2019 RX350 driving down hill with adaptive cruise control on tries to keep speed set at 72mph but as going down hill car gains speed so adaptive cruise control applies brakes to slow down. After 1-2 mile of downhill driving the car starts to vibrate severely until ADC is turned off allowing brakes to cool. Brakes seem normal as they slow car down without the vibration. Engaging ADC starts the whole process over again while going down hill. ADC does not cause vibration if on normal level or slightly downgrade sloping road.
The accident happened on 12/28/22. At 8:50 am, I was picking up a friend and was leaving the parking lot. My friend got in and and then opened the door to lock it again . My foot was on the gas pedal trying to leave the parking lot. The car jumped and went to the opposite side. I turned the steering wheel to keep it from hitting an object but could not get any control on the car. The car kept accelerating and I had to put the car back into park to stop the car. The acceleration was so much that the car jumped the curb and stopped by a wall. The air bags opened up.
AC doesn't work properly. We had several incidences when we were driving from Houston (TX) to Dallas (TX) in hot summer. The AC worked well when we left Houston, however it stopped blowing cool air when we were in the middle of nowhere on highway. The air became warmer and warmer, and there isn't anyway that we can adjust the air volume nor to cool it down. We took our car to Lexus dealer for diagnosis and nothing unusual was detected. The dealer suspect the cool air clogged in condenser that blocked cool air from entering the cabin. This is truly a safety concerns to all Lexus owners. When we drive on highway, in the middle of nowhere in hot summer, the car cannot provide a comfortable environment to driver and passengers, especially for babies or older passengers. It's a safety design flaw with Lexus RX350.
On december 28th , 2022, i was giving a ride to a collegue of mine. She opened the door and got in . I proceeded to get the car out of park and suddenly the car accelerated and i could not get the car under control. I turned the car so it would not hit the structure in front of me. I turned but still could not control the car and the only way was to put the car in to park . It jumped the curb and stopped. The Left side door opened in the process and was open as this happened.
The contact owns a 2019 Lexus RX350. The contact stated while driving 50 MPH, there was a delay during acceleration. There was no warning light illuminated. The vehicle was taken to the dealer to be diagnosed; however, no cause for the failure was found. The failure recurred several times and the vehicle was diagnosed several times however, no cause for the failure was found. The manufacturer was contacted and stated that the vehicle was not under recall. The failure mileage was 17,500.
The contact owns a 2019 Lexus RX350. The contact stated that while driving at 70 MPH with the cruise control activated; the forward collision avoidance feature failed to operate as needed. As a result, the contact hit an 18-wheeler vehicle that had unexpectedly merged into his lane. The contact depressed the brake pedal prior to the collision back of the vehicle at approximately 10 to 15 MPH. The air bags did not deploy upon impact. There were no injuries and a police report was not filed. The contact managed to still drive despite minor front-end damage to the vehicle. The contact took the vehicle to the dealer where it remained in their possession for three months; an investigator was sent to the dealer to inspect the vehicle. The investigation determined that there was no fault with the vehicle. The manufacturer was notified of the failure. The vehicle had yet to be repaired. The failure mileage was approximately 9,000. The contact stated that the radar assisted cruise control nor the brakes helped to slow the vehicle.
AC drain was clogged/malfunctioned which resulted in constant condensation in footwell. The problem persisted which resulted in black mold in the carpet and footwell insulation beneath the carpet. Lexus service advisers insisted the ac drainage line was clogged by a spiderweb but the picture they took didn’t show any spiderweb. After researching many Lexus forums, it seems like a common problem and many other people were given the same excuse of a spiderweb clogging the drainage line that also resulted in mold. Lexus doesn’t seem to want to take responsibility for the problem and rather blame a spider. They estimated $10,000 to replace the carpet and fix the problem. Other people on forums were also quoted $8,000-$15,000 as well.
I was hit by another car. Both cars were totaled. My seatbelt did not lock and my air bag did not deploy. I hit my head so hard against the steering wheel. I got a concussion and now I have daily headaches and my neck is killing me. The seat belt should have locked and the air bag should have deployed.
The Lexus Emblem where the front facing camera is located along with other sensors is causing Malfunction and Warning Lights in the Instrument Gauge Cluster. Then the Check Engine, Check Battery, 4WD Malfunction, 2WD Engaged, High Temp Coolant, ABS Light, Coolant Gauge erratically fluctuates, RPM Gauge stays at Zero. Please pull over to a safe location and Contact your dealer. The messages above are generated periodically. Please note and inform Lexus of the Issues.
Sudden acceleration while parking; collision with post & fence Failure of safety systems; including ‘collision avoidance’ breaking, ‘collision warning’ alarm, air bag failed to deploy. Repair shop to evaluate the physical damage to the car the week of 13December2021. Lexus has been notified; case number 211208000564. Lexus has not contacted me or the insurance company as of 10December2021. Mishawaka, Indiana Police Dept. case #2112070051
In heavy traffic the car stalled and by pumping the accelrator several times it activated..then at the stop light it did the same issue and heavy traffic was upon me blowing there horns..I carryed it to a lexus dealer and they could not determine the problem..I went online and found out that their is a recall on this vehicle..which was not taken care of by the car dealer before I purchased the vehicle in April 2021. Lives can be taken and I want this problem taken care of before it's our lives.
The navigation system randomly marks places of interest while there is no input from the driver. This in effect cancels the navigation information causing distraction when relying on that system. It also “marked” an address twenty miles from the destination and gave directions to that sight. Dealer cannot find cause and it has not been repaired.
The contact owns a 2019 Lexus RX350. The contact stated while his wife, was reversing in the driveway she hit the retaining wall. The driving alarms failed to engage and alert. The driver depressed the brake pedal however failed to respond and was depressed to the floorboard. The emergency brake failed to engage. The vehicle rolled down the hill and crashed into some unknown objects in the driveway. There were no reported injuries, air bag deployment, police report or fire. The vehicle was drivable. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was notified however, no further assistance was provided. The failure mileage was approximately 30,000.
Car stalled after 100+ miles driving. When gas pressed, the car thump/hump and irregular acceleration. Engine light on trac off. Stop car, engine off. Restart and can not accelerate (engine hump/thump/shake). Stop, rest, turn off AC/all other electronic, restart, very irregular acceleration, was able to drive with slow acceleration. Once past 25 mph, slow acceleration, no stall. Around 25 mph, car stalling. 2nd time into shop with same issue. 1st time Lexus indicated O-ring placed in reverse. New 2019 Lexus Rx350L with less than 4000 miles. 3rd time into shop. Local driving no issue.
The contact owns a 2019 Lexus RX350. The contact stated while parking the vehicle in a parking space, as he disengaged the brake pedal to complete the parking, the vehicle accelerated inadvertently and crashed into the pillar of a building. There were no warning lights illuminated. There were no injuries sustained. The vehicle was towed to a tow lot, then it was towed to an independent mechanic. A police report was filed. The dealer was not made aware of the failure. The manufacturer was made aware of the failure and a case was opened. The vehicle was not diagnosed nor repaired. The approximate failure mileage was 21,000.
Vehicle has been reported before for similar problems. This time, no power, severe stumbling, lurching. Check engine light comes on along with warning indicators that the electrical systems have failed. Lexus notified again and vehicle will be towed to dealership. This car is dangerous and life threatening as we were in traffic with no power. Dealer did not notify us of any problems prior to sale but this car qualifies as a lemon under AZ law. Additionally, if other Lexus owners are having this problem, they should immediately stop using the vehicle and park it before someone gets injured/killed.
Car does not speed when gas pressed, noise and car shakes. Engine check light on and screen showed trac off. Saw something with engine power issue. Parked car and called Lexus emergency. Car at dealership and it is being inspected.
Vehicle stumbles and loses power when accelerating. Loss of power when passing. Both situations caused driver to abort action for fear of not having the power to enter the roadway or pass vehicles. Lexus dealership acknowledged the problem with this vehicle and said the fuel pump was replaced because of a recall. Problem continues to exist and is dangerous to driver and others. Dealer further recommended we “ change our driving habits “ to remediate the problem. Not acceptable.
While driving 65-70mph on the freeway, the car started dinging and one of the first messages that we saw was "pre-collision system malfunction" & "hybrid system malfuncion" followed by every other "system malfunction" and the gas pedal was no longer supplying gas to the car. We were in the fast lane and had to work over 3 or 4 lanes to get to the emergency lane/exit. Since we were at an exit, we took the exit and coasted as far as we could before the car came to a halt. Another car stopped to help us get completely off the road since we weren't fully in the emergency lane. After a few minutes we were able to crank the car and drive it a few miles to the dealership. We left the car at the dealership and they called the next day to inform us that the fuel pump had to be replaced. I did not find this vehicle to be part of any recall or lawsuit but several other Lexus models are part of a recall/lawsuit due to a problem with the Denso fuel pump. I'm not sure if my car has this same fuel pump. There were no warning lamps, messages or other symptoms of the problem prior to the failure.
WE TOOK THE CAR IN BECAUSE WHEN THE CAR IS COLD IT DOES NOT SHIFT PROPERLY. THIS PROBLEM HAS BEEN HAPPENING AND GETTING WORSE SINCE IT WAS THERE FOR A RECALL ON THE TRANSMISSION SOFTWARE. DEALER REFUSES TO ACKNOWLEDGE THAT THERE IS A PROBLEM STATING THE DIAGNOSTIC COMPUTER SHOWS NO ERROR CODES, WHILE IGNORING THE FACT WHEN YOU SHIFT FROM PARK TO REVERSE OR DRIVE THE VEHICLE JERKS AND THE GEAR LEVER REQUIRES A LOT OF FORCE TO MOVE INTO DESIRED GEAR. DEALER REFUSES TO REPAIR THE CAR OR ACKNOWLEDGE THE PROBLEM, INSTEAD THEY TRIED TO SELL ME A $875 OIL CHANGE. DEALER IS ALSO REFUSING TO SEND ME ANY PAPERWORK THAT MY CAR WAS THERE EVEN AFTER CALLING MANY TIMES TO REQUEST IT, THEY PROMISE TO SEND IT THAT DAY AND NEVER SEND IT BECAUSE "THEY ARE BUSY". SERVICE ADVISOR DAVID ( TAOU) WAS VERY ARROGANT, CONDESCENDING AND DISHONEST.
VEHICLE VIBRATES WHEN BRAKING, BRAKE PEDAL PULSATES AS WELL WHEN BRAKING, SUCH DECREASING THE ABILITY FOR VEHICLE TO STOP APPROPRIATELY. EVERY 6,000 MILES ROTORS WARP BADLY AND NEED TO BE CHANGED WITH NORMAL DRIVING HABITS. LEXUS MUST RECALL THESE DEFECTIVE ROTORS AS THEY COULD AFFECT THE ABILITY FOR THE VEHICLE TO STOP
ON A RECENT 2500 MILE TRIP ON 3 SEPARATE OCCASIONS WHILE ON INTERSTATE HIWAYS AND PASSING A VEHICLE THE EMERGENCY BRAKING SYSTEM ENGAGED AUTOMATICALLY WITH WARNING LIGHTS AND BUZZERS GOING OFF. THIS OCCURRED WITH NO OTHER VEHICLES ANYWHERE AROUND FOR A QUARTER MILE OR MORE. ON ONE OCCASION IT OCCURRED WHILE DRIVING IN THE RIGHT LANE AND WITH NO VEHICLES IN SIGHT. LEXUS SPECIAL TECH INSPECTED THE VEHICLE AND HAD SOME CALIBRATIONS PERFORMED BUT THEY WILL NOT TELL ME WHAT THEY FOUND IN DATA CAPTURED BY THE VEHICLE COMPUTER OR WHAT ACTIONS THEY TOOK AND WHY THEY THINK THE VEHICLE IS SAFE TO DRIVE.
WHILE DRIVING, I'VE BEEN ENCOUNTERING HARD GEAR UPSHIFTS AND DOWNSHIFTS. I HAVE NOTIFIED MY LOCAL LEXUS DEALERSHIP AND THEY TOLD ME THAT IS NORMAL. THIS IS MY 3RD CAR I BOUGHT AND THE FIRST TIME I'VE ENCOUNTERED SUCH AN ISSUE. ISSUE USUALLY HAPPENS WHEN SPEEDS RANGE BETWEEN 50-70 MPH REGARDLESS OF WEATHER CONDITIONS. THIS IS A SAFETY CONCERN, GIVEN THAT THE CAR IS STILL PRETTY NEW.
CAR WAS PDI AND DELIVERED ON 06/14/20 WITH AN OPEN RECALL 20LA01 THAT WAS LIKELY TO BE A LIFE-THREATENING ISSUE. EVEN AFTER PDI WAS PERFORMED, THE BREAKS WERE DISFUNCTIONAL, SO HAD TO TAKE THE VEHICLE FOR SERVICE ON 06/19/20. LEXUS FOUND THAT ROTORS REQUIRED RESURFACING TO FIX THE BREAKS AND AT THE SAME TIME COMPLETED THE OPEN RECALL 20LA01. HOWEVER ON 07/03/20, THE CAR STALLED IN THE MIDDLE OF A HIGHWAY TRAFFIC PUTTING THE PAX THRU A LIFE-THREATENING SITUATION. CAR HAS BEEN FINALLY TOWED TO THE SERVICE AND I AM VERBALLY PROMISED THAT THE CAR HAS BEEN FIXED TO NOT PRESENT ANY LIFE THREATENING ISSUES. SINCE IT IS JUST 15 DAYS OF PURCHASE AND VEHICLE BEEN INTO THE SERVICE REVIEW FOR 3 TIMES WITH SOME LIFE-THREATENING OPEN ISSUES, I AM REQUESTING LEXUS TO EITHER REFUND OR PROVIDE ME A REPLACEMENT. *TR

WHILE DRIVING ON A CITY STREET THE WARNING LIGHTS CAME ON AND THE CAR STALLED. I WAS ALONE IN THE CAR AND IN DOWNTOWN CITY TRAFFIC WITH HONKING CARS BEHIND ME. I WAS ABLE TO RESTART THE CAR AFTER A FEW ATTEMPTS. A FEW DAYS AFTER THIS INCIDENT, WHILE DRIVING ON A VERY BUSY HWY AT 55MPH THE CAR ENGINE WAS SKIPPING AS IF ABOUT TO STALL. I HAD A PASSENGER IN MY CAR WHO EXPERIENCED THIS ROUGH RIDE AND MADE A COMMENT ABOUT IT. A FEW DAYS LATER, ON MARCH 8,2020, I RECEIVED AN EMAIL FROM LEXUS ENFORM SERVICES WITH A VEHICLE HEALTH REPORT INFORMING ME THAT MY VEHICLE REQUIRES ATTENTION DUE TO A SAFETY RECALL 20LA01. WHEN I LEASED MY VEHICLE, I WAS NEVER INFORMED OF THIS RECALL WHICH GOES BACK TO 01/13/2020. SAFETY IS A TOP CONCERN FOR ME AND I FULLY COMMUNICATED THIS TO THE SALESPERSON WHEN I LEASED MY CAR BUT I WAS NOT INFORMED OF THIS RECALL AT THAT TIME. I HAVE EXACTLY 2800 MILES ON MY CAR. I AM NOT ABLE TO USE IT BECAUSE THE SERVICE DEPARTMENT AT LEXUS DOES NOT HAVE A REMEDY AVAILABLE TO FIX THE LOW -PRESSURE FUEL PUMP AND THEY DO NOT KNOW WHEN THEY WILL HAVE IT. I WANT TO DRIVE A VEHICLE THAT IS SAFE NOT ONE THAT INCREASES THE RISK OF HAVING A CRASH.
