AN OPEN SEAM IN THE CONSUMER'S 2000 MAZDA MPV'S FRONT EXHAUST MANIFOLD ALLOWED EXHAUST FUMES TO ENTER THE VEHICLE. THE CONSUMER'S CHILD HAD A SEVERE ASTHMA ATTACK AS A RESULT OF THIS FAILURE. *NM *NM
WHILE APPLYING THE BRAKE FROM 56-60 MPH THE BRAKE PEDAL GOT HARD, AND THE VEHICLE EXPERIENCED EXTENDED STOPPING DISTANCES BEFORE IT V WAS ABLE TO STOP. VEHICLE WAS TAKEN TO A REPAIR SHOP, AND MECHANIC DETERMINED THAT THEY COULD NOT FIND ANYTHING WRONG. DEALER WAS NOT NOTIFIED.*AK
VAN STARTED RUNNING ROUGH AT STOPLIGHTS AND AT LOW SPEED. CHECK ENGINE LIGHT CAME ON A DAY LATER. MECHANIC TOLD US THAT THE LEFT EXHAUST MANIFOLD WAS CRACKED AND NEEDED TO BE REPLACED. THIS FAILURE CAUSES A CO LEAK INTO THE PASSENGER CABIN. MECHANIC SAID THIS SHOULD NOT HAPPEN ON AN ENGINE WITH 83,000 MILES.*AK
ENGINE LIGHT STAYS ON AFTER BEING RESET 3 TIMES AND AFTER SENSOR WAS REPLACED.*AK
CONSUMER'S VEHICLE WAS HAVING THE SAME PROBLEMS AS STATED IN RECALL 99V170000 CONCERNING FUEL INJECTION SYSTEM. DEALERSHIP WOULD NOT REPAIR VEHICLE FREE OF CHARGE BECAUSE THE VIN WAS NOT INCLUDED.*AK
THE DASHBOARD WAS SHAKING WHILE DRIVING AT 55 MPH. REAR RIGHT TIRE WENT FLAT. THE TIRES WERE OEM TIRES CAME WITH MAZDA MPV 2000 PURCHASED 4/28/00. THE ODOMETER AT TIME OF THREAD SEPARATION WAS 35,000 MILES.*AK
CARBON MONOXIDE ENTERING THE PASSENGER COMPARTMENT. HORRIFIC ODORS. DEALER STATES DO NOT DRIVE VEHICLE ON SHORT TRIPS AND REFUSES CORRECTION OF IDENTIFIED ISSUE.*AK
CONSUMER WAS INFORMED BY THE DEALERSHIP THE REASON FOR THE ANTI FREEZE LEAK WAS DUE TO RUSTED HEATER CORE PIPE. THE PROBLEM WAS REPAIRED. *AK....*AK
WHILE PARKED IN THE DRIVEWAY CONSUMER NOTICED THAT THE RIGHT FRONT TIRE WAS FLAT. THE TIRE WAS TAKEN TO A TIRE DEALERSHIP, WHO INDICATED THAT THE TIRE NEEDED TO BE REPLACED BECAUSE IT HAD WORN OUT ON ONE SIDE, AND HAD A BIG GROVE IN IT. THE TIRE WAS REPLACED WITH A DIFFERENT MODEL TIRE WITH THE SAME GRADE. A WEEK LATER, THE SAME PROBLEM OCCURRED ON THE REAR OF DRIVER'S SIDE TIRE, AND THE OTHER TWO TIRES HAD STARTED WEARING THE SAME WAY. THE OTHER TIRES WERE TAKEN TO A TIRE RETAIL STORE, TECHNICIAN INDICATED THAT THE MANUFACTURER PUT THE WRONG SIZE TIRES ON THE VAN, AND THAT THEY WERE AWARE OF IT . THESE TIRES WERE NOT ADEQUATE FOR THE WEIGHT OF THE VAN. ALSO, DEALER INDICATED THAT THIS WAS A COMMON PROBLEM, AND MANUFACTURER WAS REPLACING THESE 92S TIRES WITH 94SA. *AK *NM
DRIVING ON HIGHWAY DOWN MOUNTAIN PASS ACCELERATOR STUCK, CAR WOULD NOT SLOW DOWN. COULD NOT TURN OFF CRUISE CONTROL. HAD TO USE BOTH FEET ON BRAKES AND USE TRUCK EMERGENCY TURNOUT TO STOP. ENGINE WAS RACING. *LA
WHILE DRIVING AT ANY SPEED, THE CHECK ENGINE LIGHT WOULD COME ON AND A STRONG SULFUR ODOR WOULD ENTERED THE VEHICLE. A MECHANIC DIAGNOSED THE PROBLEM AS A BAD CATALYTIC CONVERTER. *JB
ALTERNATOR FROZE, CAUSING THE BELT TO SLIP WHICH ALMOST RESULTED IN A FIRE. CONSUMER PULLED THE CAR OVER. LATER, HAD THE VEHICLE TOWED TO DEALERSHIP. *AK THE BURNED BELT COULD HAVE IGNITED IF CONSUMER CONTINUED TO DRIVE VEHICLE. *LA
AT ANY SPEED OR DRIVING CONDITIONS VEHICLE EMITS SULFUR ODOR, IE ROTTEN EGG SMELL, INTO CABIN. SMELLS LIKE SOMEBODY FARTED.
CONSUMER STATES WHILE DRIVING THE VEHICLE AT ANY SPEED, THE VEHICLE WOULD HAVE AN ODOR SMELL. DEALER NOTIFIED. PH
LEAK IN 2000 MPV THAT DEALER STATED WAS DUE TO "OUTSIDE INFLUENCE" AND WOULD NOT BE COVERED AS A MANUFACTURER'S DEFECT. DEALER TOLD US THAT THE LEAK (WHICH RUINED OUR FLOOR BOARDS, RUGS AND WIRING) WAS DUE TO PARKING THE CAR OUTSIDE, WHICH LED TO DIRT AND LEAVES GETTING INTO THE HOOD, (OR SOMEWHERE) CLOGGING THE DRAINAGE SYSTEM . WE DO PARK THE CAR OUTSIDE IN A CLEAR DRIVEWAY. (WE WERE NOT AWARE THAT YOU HAD TO OWN A GARAGE TO OWN A MAZDA). WE HAVE NEVER HAD AN ACCIDENT OR MADE ANY MODIFICATIONS TO THE CAR. IT JUST STARTED TO LEAK UNCONTROLLABLY. THEY ALSO REFUSED TO FIX THE SPARE TIRE HOLDER(WHICH IS UNDER THE BASE OF THE CAR) WHICH ACCORDING TO THE DEALER, BROKE ON THE FIRST USE.(WE SIMPLY FOLLOWED THE INSTRUCTIONS IN THE MANUAL WHEN REMOVING THE TIRE FROM THE HOLDER) WE CAN NOT PUT THE TIRE BACK, AND WERE TOLD THAT IT WOULD COST US 480. (AT LEAST) TO FIX IT. OUR CAR IS 6 MONTHS AND 5,000 MILES OUT OF WARRANTY AND WILL BE RUINED THE NEXT TIME IT RAINS AS WATER POURS INTO THE FRONT OF THE CAR INTO THE FLOORBOARDS, AFTER SOAKING THE WIRING. I SUPPOSE WE WILL HAVE TO MOVE AND BUY A GARAGE.... *JB
PROBLEMS WITH EXHAUST MANIFOLD IS CAUSING CARBON MONOZIDE FUMES TO ENTER THE PASSENGER AREA.*MR (NAR) *CB
MOLD IS STARTING TO GROW IN THE VAN UNDER THE MATS. THIS IS THE SECOND TIME IT HAS HAPPENED AND THE DEALER SAYS THAT IT IS NOT COVERED. THE WATER GETS IN WHEN IT DOES NOT DRAIN PROPERLY. *LA
WHILE DRIVING AT 70 MPH, THE LEFT FRONT TIRE TREAD SEPARATED AND THE RIGHT FRONT TIRE DEVELOPED A KNOT IN THE SIDEWALL. BOTH TIRES HAD OVER 50 % TREAD LEFT AT TIME OF FAILURE. THE SERVICE DEALERSHIP INDICATED THAT BOTH FAILURES WERE DUE TO A MANUFACTURING DEFECT. *AK THE VEHICLE EXPERIENCED THREE TIRE FAILURES. *JB *NM
THE CHECK ENGINE LIGHT ILLUMINATED ON THE DASHBOARD WHILE DRIVING. THE DEALER DETERMINED THAT THE OIL PUMP FAILED AND NEEDED TO REPLACED. *AK *NM
AFTER STARTING MY CAR THE GAS PEDAL IS STUCK AND NEEDS TO BE PRESSED HARD TO UNSTICK IT. THIS PROBLEM HAS OCCURRED 4-5 TIMES DURING THE LIFE OF THE CAR. IN EACH CASE THE DEALER HAS REPAIRED IT BUT CLAIMS THE PROBLEM IS DUE TO THE FUEL I AM USING. THE DEALER CLEANS THE THROTTLE BODY AND THE PROBLEM STOPS FOR ABOUT 10 THOUSAND MILES. THIS PROBLEM IS CLEARLY NOT MINE ALONE; COMPLAINT 10005259 DESCRIBES MY EXACT SAME CONDITION.*JB
WHILE DRIVING, THE VEHICLE EXPERIENCED SUDDEN ACCELERATION. THE VEHICLE WAS TAKEN TO AN INDEPENDENT SHOP, WHO FOUND THE CRUISE CONTROL CABLES WERE DEFECTIVE. *AK THE MECHANIC NOTICED THE CRUISE CONTROL WAS STUCK PREVENTING THE ENGINE SPEED TO RETURN TO IDLE. A MAZDA DEALERSHIP INSTALLED A NEW CABLE THAT WAS PURCHASED BY THE CONSUMER. VEHICLE OPERATED CORRECTLY SINCE THEN. *PH *JB
THE MANUAL SLIDING DOOR CONTAINED A VERTICAL LINE OF APPROXIMATELY 10 MAGNETS. A METAL BRACELET CAME IN CONTACT WITH THE MAGNETS WHICH CAUSED BURNS TO THE CHILD'S HAND. *JB IN LESS THAN A MONTH, THE MOTOR AND REGULATOR ON THE DRIVER SIDE WINDOW HAD BLOWN. *SCC *JB
WHILE DRIVING AND WITHOUT WARNING THE VEHICLE EXPERIENCED ELECTRICAL PROBLEMS. *NLM
THE VEHICLE WAS INVOLVED IN A COLLISION, AND THE AIR BAGS FAILED TO DEPLOY. *JB
I HAVE A MAZDA MPV 2000, THE CHECK ENGINE LIGHT CAME ON AND I TOOK IT IN FOR SERVICE ON 2/13/03. I WAS TOLD THAT THE MPV REQUIRED A NEW OXYGEN SENSOR BUT IT WOULD BE 3 TO 4 WEEKS DUE TO A BACK ORDER. I WAS TOLD BY THE DEALER THAT I COULD DRIVE THE CAR AND IT WOULD HARM NOTHING. ON FEB 20TH, I DROVE 3/4 MILE FROM HOME WHEN THE MPV DIED WITH THE CHECK ENGINE LIGHT, OIL LIGHT AND BATTERY LIGHT ALL ON. THE MPV WAS TOWED TO THE MAZDA DEALERSHIP, THEY CALLED ME AND SAID THE ENGINE WAS CEASED.*JB
CONSUMER RECEIVED A RECALL LETTER FROM MAZDA MANUFACTURER. DEALER WON'T HONORING THE RECALL LETTER. PLEASE PROVIDE FURTHER INFORMATION. PH
WHEN EXCELERATING ON THE INITIAL DEPRESSION OF THE PEDAL, CAR WILL NOT EXCELERATE, SEEMS 'STUCK'. FOR THE CAR TO MOVE YOU MUST DEPRESS THE PEDAL FURTHER WHICH RESULTS IN A SUDDEN HIGH SPEED EXCELERATION, IN A JUMPING FASHION. THIS IS TRUE IN DRIVE OR REVERSE. WHEN MAZDA DEALER SERVICE REP. WAS CONTACTED SHE SAID THERE HAD BEEN SEVERAL PROBLEMS REPORTED TO THAT DEALERSHIP OF THE SAME NATURE BUT NOT RECALLS OR SERVICE BULLETINS HAVE BEEN REPORTED.
CONSUMER STATED THAT WHEN PUTTING ON THE REAR SEAT BELT IT WOULD NOT BE TIGHT ENOUGH FOR IT TO FIT THE CONSUMER PROPERLY. DEALER WILL BE NOTIFIED. TS
DEFECTIVE PART. EXHAUST MANIFOLD FAILED AND IS OUT OF WARRANTY. DEALER WOULD NOT COVER REPAIR COST DUE TO BEING OUT OF WARRANTY. THIS TYPE OF PART SHOULD NOT HAVE FAILED IN THREE YEARS NO MATTER WHAT THE MILAGE IS. IT CAUSED DRIVER (ME) TO BREATH EXHAUST FUMES AND I BECAME LIGHT HEADED AND SICK. WEATHER WAS TOO COLD TO DRIVE WITH WINDOWS OPEN. MANUFACTURER SHOULD COVER THE COST OF THIS REPAIR.
CONSUMER STATES THERE IS A SULFUR SMELL IN THE CAB OF VEHICLE. MECHANIC STATES THAT THE WAY THE CATALYTIC CONVERTER IS BURNING THE GAS IS CAUSING THE SMELL. MECHANIC IS AWARE OF PROBLEM. PLEASE PROVIDE ANY FURTHER INFORMATION. PH *TS
WHILE DRIVING ENGINE PULLEY FELL OFF, CAUSING CONSUMER TO LOSE CONTROL OF VEHICLE. BOLT THAT HELD IT IN PLACE BROKE OFF. CONTACTED DEALER. *AK
BOTH FRONT TIRES (INSIDE EDGE) WORN OUT AROUND HALF THE CIRCUMFERENCE OF THE TIRE. THE FRONT TIRES WORN OUT PREMATURELY WITH THE TREADS SHOWING SEPARATIONS ABOUT AN INCH LONG FROM EDGES WHERE TREADS MEET THE SIDEWALL. THE STEEL BELTS COULD BE SEEN FROM THE ONE-INCH SEPARATION. CAR DEALER SERVICE MANAGER COULD NOT FIGURE OUT WHAT CAUSED THE PREMATURE WEAR.. THE VEHICLE TIRES HAVE BEEN ROTATED AND BALANCED ACCORDING TO MAINTENANCE SCHEDULE. CONTACTED TIRE MANUFACTURER AND REFERRED TO A LOCAL DUNLOP DEALER. TIRE DEALER CONCLUDED THAT TIRES WERE NOT ROTATED. MY BROTHER- IN- LAWS MPV 2000 WITH 25K MILES EXPERIENCED THE SAME TIRE WEAR (THE BRAND OF TIRES HE HAD WAS YOKOHAMA), BUT MUCH MORE SEVERE DAMAGE WITH LONGER TREAD SEPARATION CAUSING THE TIRES TO LOSE MOST OF THE AIR. MY COMMENT IS, COULD THIS BE A CAR PROBLEM (SUSPENSION), AND NOT THE TIRE? I AM THANKFUL THAT THESE TIRES DID NOT BLOW UP AT HIGH SPEEDS. WHO KNOWS HOW MANY MORE OUT THERE ARE NOT BEING REPORTED. WHILE DRIVING ON THE FREEWAY THE VAN SUDDENLY LOST POWER TO THE WHEELS, EVEN THOUGH THE ENGINE WAS STILL RUNNING. HAD IT TOWED TO A MAZDA DEALER WHO REPORTED THAT THE THRUST BEARING IN THE CRANKSHAFT HAD BROKEN. THE ENTIRE ENGINE HAD TO BE REPLACED.*AK
WHILE DRIVING ON THE INTERSTATE, MY MAZDA MPV LOST POWER TO THE DRIVE WHEELS, AND I HAD TO PULL OVER TO THE SIDE OF THE ROAD.
SLIDING DOOR OPENED ON THREE SEPARATE OCCASIONS WHILE DRIVING, ON ONE OCCASION THE DOOR WAS STUCK OPEN AND COULD ONLY BE CLOSED BY LIFTING AND PULLING THE DOOR, THE DEALER REPLACED MISSING SCREWS IN THE DOOR LATCH BUT THE PROBLEM RECURRED. *AK *NLM
THE CONSUMER STATES THE TETHER STRAPS WERE INCORRECTLY INSTALLED AT THE THIRD SEAT BY THE MAZDA DEALER. THE CONSUMER STATES AFTER GOING TO THE MANUFACTURER TO CONFIRM THE INSTALLATION WAS WRONG. THE CONSUMER WENT TO ANOTHER MAZDA DEALER FOR A SECOND OPINION THAT THE TETHER ANCHOR WAS INCORRECTED. THE CONSUMER ALSO STATES THE CHILD COULD HAVE BEEN SERVERLY HURT THE PROBLEM WAS NOT DETECTED. PLEASE ANY ADDITIONAL INFORMATION.
ON 3/9/02, LEFT HOME TO HEAD FOR BANK. ABOUT 2 MINUTES/3 STREETS DOWN, CAR WAS SMOKING AND BLACKISH OIL WAS GUSHING FROM ALL AROUND HOOD AREA. CAR BROKE DOWN AND COULD NOT RE-START.FIRE/SHERIFF DEPT CAME AND CAR WAS TOWED TO OAK TREE MAZDA WHERE CAR WAS SERVICED ON 1/29/02. ON 3/11/02 PETER POTMAN OF OAK TREE MAZDA CALLED AND TOLD ME ENGINE OIL WAS MIXED WITH COOLANT DUE TO OIL PUMP FAILURE. ENGINE LOCK IS CRACKED AND HAD TO REPLACE WITH NEW ENGINE. 10 DAYS LATER ON 3/21/02. PETER CALLED AND SAID CAR WAS READY TO PICK-UP.THEY TESTED EVERTHING AND CAR WAS "WONDERFUL". I PICK UP CAR 2:30 PM AND HEADED HOME AND AT 4:00 LEFT HOME TO GO TO BANK BEFORE PICK UP DAUGHTER FROM SCHOOL. WITHIN MINUTES CAR WAS SMOKING AGAIN AND SMELL LIKE IT WAS BURNING. CALLED PETER AGAI AND HE SENT FOR THE TOW TRUCK TO TOW IT BACK TO THEM. NEXT DAY ON 3/22/02, PETER TOLD ME THEY COULD FIND NO NEW OIL LEAKAGE. SMOKING/BURNING CAME "POSSIBLY" FROM RESIDUE OIL FROM PREVIOUS (3/9/02) INCIDENT. THEY'D TRY TO DETAIL CLEAN ENGINE AGAIN TO MAKE SURE NO MORE RESIDUE OIL IS LEFT TO BURN. N 3/25/02, PETER OFFERED TO DRIVE MY CAR HOME (60 MILES ONE WAY) AND BACK THE NEXT MORNING (120 MILES ROUND TRIP) TO MAKE SURE. I SAID OK AND ON 3./26/02, PETER CALLED AND SAID HE WASN'T COMFORTABLE WITH GIVING ME CAR BACK AS HE COULD SMELL BURNING OIL FROM INSIDE OF THE CAR AS THE HE WAS DRIVING. HE'D DRIVE MY CAR HOME AGAIN 3/26/02 EVENING AND WOULD LET ME KNOW TOMORROW (3/27/02). I'VE COMMUNICATED WITH MAZDA'S DAREN (X1179) SEVERAL TIMES RE MY CONCERNS ABOUT SAFETY BECAUSE TWICE THE CAR BEGAN SMOKING/STEAMING WITH NO WARNING LIGHTS ON WHATSOEVER. NLM

COMPLAINT RECEIVED E-MAIL. WE ARE HAVING PROBLEMS WITH THE FRONT DOOR NOT UNLOCKING. WE HAD TO FIX THE PASSENGER'S SIDE ABOUT A MONTH AGO. NOW, THE DRIVER'S SIDE CAN NOT BE OPENED UNLESS USING A KEY ON THE OUTSIDE OR HOLD THE UNLOCKED BUTTON ON THE INSIDE. I HOPE WE NEVER GET INTO A WRECK BECAUSE NO ONE COULD GET US OUT OF THE CAR. *AK *TT
BELT RUNOUT WITH ONE TIRE. THIS MAY BE RELATED TO RECALL - 00V113001. MAZDA CLAIMS TIRES MEET 94 LOAD RATING. HOWEVER, THE EARLY FAILURE OF A TIRE LEADS ME TO THINK THAT THE TIRES ARE TOO SMALL AND DO NOT MEET THE 94 LOAD RATING. ALSO, THE LABELS ISSUED BY MAZDA SAY TO USE A REPLACEMENT TIRE THAT IS A 205/65 15 94S AND INFLATE IT TO 35 PSI. THIS IS WRONG. ONLY 3 TIRES ARE AVAILABLE IN THAT SIZE WITH A LOAD RATING OF 94. THEY ALL REQUIRE YOU TO INFLATE THE TIRES TO 44 PSI OR GREATER. THIS IS A SERIOUS SAFETY ISSUE. MAZDA IS SUGGESTING OWNERS UNDER-INFLATE THE TIRES BY 9 TO 16 PSI. THE STANDARD TIRE RATING FOR A 205/65 15 IS A 1992. *AK (TIRESIZE: 205/65 15)
TL*THE CONTACT OWNS A 2000 MAZDA MPV. UPON PURCHASING THE VEHICLE, THE CONTACT NOTICED THAT IT WAS NOT LEVELED AND WAS LEANING TOWARDS THE PASSENGER SIDE. THE VEHICLE WOULD ALSO PULL TOWARDS THE RIGHT WHILE DRIVING MORE THAN 25 MPH. BETWEEN 60-70 MPH, THE VEHICLE WOULD VIBRATE VIOLENTLY AND COULD BE FELT WHEN HOLDING THE STEERING WHEEL. IN ADDITION, THE PASSENGER SIDE DOOR, CD PLAYER, AND LIGHTER ALL FAILED. THE REARVIEW MIRROR BECAME FOGGY AND NEEDED TO BE REPLACED, AS WELL AS THE LOWER BALL BEARINGS. THE LIFTGATE FAILED AND THE TRUNK WOULD FALL DOWN ON TOP OF HER. THE MANUFACTURER WAS NOTIFIED, BUT THERE WERE NO RECALLS FOR HER VIN THAT PERTAINED TO THE FAILURES. THE CONTACT FOUND SEVERAL TECHNICAL SERVICE BULLETINS REGARDING THESE FAILURES; HOWEVER, THE MANUFACTURER WOULD NOT TAKE ANY RESPONSIBILITY FOR REPAIRING THE VEHICLE. THE FAILURE MILEAGE WAS UNKNOWN AND THE CURRENT MILEAGE WAS 53,000.
NHTSA 01V382000 MANUFACTURER CAMPAIGN # 0520A 12/20/2001 REMEDY AVAILABLE DX BRAKE LINES AND VALVES. AS VEHICLE IS IN OPERATION FRONT SUSPENSION SHAKES VERY BAD ABOUT 35MPH TO 50MPH DOES NOT FEEL SAFE TO DRIVE ON HWY.
CLOCKING NOISE WHEN BRAKES APPLIED AT LOW SPEED, MOVING FORWARD OR BACKWARDS. NO RECALL OR DESIGN FOR THE BRAKES ATTACHMENT PAD CLIPS WHICH ARE CAUSING THE CLOCKING NOISE.
ENGINE STALLS INTERMITTENTLY AND HESITATES WHEN APPLYING ACCELERATOR PEDAL. VEHICLE HAS BEEN TO DEALER SHIP ON SEVERAL OCCASIONS. DEALER HAS BEEN UNABLE TO CORRECT PROBLEM. FEEL FREE TO PROVIDE ANY FURTHER INFORMATION.*AK

TREAD SEPARATION AND EXCESSIVE TREAD WEAR DISCOVERED ON BOTH FRONT TIRES DURING ROUTINE TIRE ROTATION MAINTENANCE. THE RUBBER HAD PARTED IN A COUPLE OF PLACES ON BOTH TIRES REVEALING THE TIRES' STEEL BELTS UNDERNEATH. TIRES WERE IMMEDIATELY REPLACED - WE ARE STILL IN POSSESSION OF THE FAULTY TIRES.*AK (DOT NUMBER: FD9LNHL289 TIRESIZE: P205/65R15)
FRONT AIRBAGS DID NOT WORK AT ALL. BACK RIGHT SEATBELT SHOULDER HARNESS DID NOT WORK. VEHICLE WAS HIT FROM BEHIND AND THEN PUSHED INTO ANOTHER VEHICLE THAT WAS IN FRONT. THE VEHICLE THAT HIT MY VAN WAS TRAVELING AT APPROXIMATELY 40 MILES PER HOUR WHEN MY VEHICLE WAS HIT MY DAUGHTER WHO WAS SITTING IN THE 3RD ROW SEAT WEARING A LAP/SHOULDERBELT FLEW FORWARD ENOUGH TO HIT HER HEAD AND FACE ON THE BACK OF THE SECOND ROW SEAT. IT SEEMS THAT THE SHOULDER PART OF THE SEATBELT DID NOT LOCK AT ALL DURING IMPACT WHICH ALLOWED HER THAT MUCH MOVEMENT. IN ADDITION, THE PASSENGER AND DRIVERS SIDE AIRBAGS DID NOT DEPLOY DURING THE FRONTAL IMPACT WHEN MY VEHICLE WAS PUSHED INTO THE VEHICLE IN FRONT OF ME WHICH PUSHED THE WHOLE FRONT OF THE VAN INTO THE ENGINE COMPARTMENT.
CONSUMER STATED WHEN THE BRAKES WERE APPLIED AT 65MPH WHILE EXITING OFF A RAMP, THERE WAS EXTENDED STOPPING DISTANCE WHICH RESULTED IN A COLLISION.*AK *SLC
DUE POSSIBLY TO POOR ALIGNMENT, INCORRECT COIL SPRINGS, ALL FOUR TIRES WORN DANGEROUSLY AND HAD TO BE REPLACED. MAZDA ISSUED "BULLETINS" ON THESE PROBLEMS TO DEALER BUT DID NOT NOTIFY OWNERS. THERE WAS ALSO A "BULLETIN" ON A BRAKE PROBLEM, NOT REPORTED TO OWNER.*AK (DOT NUMBER: FD9L TIRESIZE: 65R15)
