I WAS AT A PARKING LOT AND I WAS MAKING MY WAY INTO A SLOT WHEN I FIRST HAD TO REVERSE THEM WHEN I CHANGED THE GEAR THE DRIVE AND RELEASED THE BREAKS TO MOVE FORWARD INTO THE SLOT I SUDDENLY ACCELERATED AND HAD TO PRESS ON THE BREAKS HARDER THAN EVER BECAUSE I THOUGHT AT FIRST I WAS RELEASING IT TOO FAST. BUT EVENTUALLY I SCREECHED INTO A FULL STOP AND AVOIDED HITTING THE CARS PARKED AND WORSE GOING OVER THE STREET ONTO INCOMING TRAFFIC. I NOTICED TIRE MARKS THAT MY BREAKING HAD MADE AT THE LOT.
Recently saw an article that NHTSA was looking into "Ghost Breaking" with Tesla cars. This is something we have experienced with our Model S on a few occasions in 2018 and 1029. Haven't been driving that car that much now during the pandemic. Problem reported to Tesla: While driving on US 101 in Bay Area (driving from Milpitas to South SanFrancisco), and the car on Adaptive Cruise Control, the car would suddenly break and resume driving again. This not only scares the driver, but could be very problematic in a crowded lane. Telsa has looked into it when we complained and there was not definitive answer or fix. Hope the recent considerations by NHTSA will result in a through root cause and corrective action. Thanks!
I DROVE TO WORK ON MAY 2, 2019, AND WAS TRYING TO PARK IN MY ALLOTTED PARKING STALL ON THE 3RD LEVEL OF MY OFFICE PARKING GARAGE. MY PARKING STALL WAS ALONG THE OUTER EDGE OF THE GARAGE AND THE ONLY BARRIER TO THE OUTSIDE WAS A SET OF THREE (ABOUT " DIAMETER) CABLES STRUNG BETWEEN OUTSIDE COLUMNS OF THE GARAGE, BESIDES A THIN PERFORATED CORRUGATED METAL SHEET OUTSIDE THE CABLES, ACTING AS A RAIN STOP. I WAS TRYING TO SLOWLY TURN RIGHT FROM THE PARKING AISLE TO PARK BETWEEN TWO PICKUP TRUCKS ON EITHER SIDE OF MY STALL. WHEN I HAD TURNED PARTWAY INTO MY STALL, MY TESLA SUDDENLY ACCELERATED INTO MY PARKING STALL AT HIGH SPEED, HIT THE OUTSIDE CABLES, REBOUNDED BACKWARDS AND STOPPED ABOUT 10 FEET FROM THE OUTSIDE CABLES. THE CABLES SAVED ME FROM PLUNGING 3 FLOORS DOWN FROM THE GARAGE. THERE WAS EXTENSIVE DAMAGE TO THE HOOD AND FRONT PORTIONS OF MY TESLA. THERE WAS ALSO SOME DAMAGE TO THE METAL CORRUGATED SHEETS OUTSIDE THE CABLES AND A SLIGHT DENT ON THE CORNER OF THE VEHICLE PARKED TO MY RIGHT. I HAVE BEEN PARKING IN MY OFFICE PARKING STALL FOR ABOUT THE PAST SIX MONTHS WITHOUT ANY ISSUE LIKE THIS. I WANT TO KNOW WHY MY CAR ACCELERATED LIKE THAT WITHOUT WARNING AND WHY THE COLLISION WARNING SYSTEM WAS NOT TRIGGERED BY THE OUTSIDE CORRUGATED METAL SHEET. ALSO, THERE WAS AN ISSUE WITH MY TESLA THE PREVIOUS EVENING. WHEN I TRIED TO START MY TESLA AFTER WORK, I GOT THE 'SYSTEMS ARE POWERING..' MESSAGE ON THE SCREEN AND MY CAR WOULD NOT START EVEN AFTER A LONG WAIT. THEN, I HAD TO GOOGLE TO FIND A WAY TO REBOOT THE CAR COMPUTER, BY PRESSING TWO BUTTONS ON THE STEERING WHEEL. ON THE DAY OF THE ACCIDENT, I USED THE INTERMITTENT RAIN SENSING WIPERS FOR THE FIRST TIME, AS THERE WAS A LIGHT RAIN. MY CAR IS STILL IN THE BODY SHOP WAITING FOR SOME BACK ORDERED PARTS. I HAVE ASKED TESLA TO DO A DIAGNOSTICS TO SEE IF THERE WERE ANY SOFTWARE BUGS IN THE SYSTEM.
I ENTERED THE STATIONARY CAR AND TURNED IT ON. THE WINDSHIELD AND ALL INTERIOR GLASS IMMEDIATELY FOGGED MAKING VISIBILITY NEARLY ZERO. THE INSIDE OF THE WINDSHIELD BECAME EXTREMELY WET. WIPING THE MOISTURE ONLY RESULTED IN IMMEDIATE FOGGING OF THE WINDSHIELD. I SET THE FRONT AND REAR DEFROSTERS ON MAXIMUM AND ONLY A 8 INCH SEMI CIRCLE AT THE BOTTOM OF THE WINDSHIELD IN-FRONT OF THE DRIVER AND PASSENGER WOULD DEFOG. I RESET THE COMPUTER AND NOTHING CHANGED. AFTER 20 MINUTES I CALLED THE EMERGENCY SERVICE NUMBER FOR TESLA. THEY COULD NOT HELP ME BECAUSE I HAD ALREADY DONE EVERYTHING. THE THE TESLA PARKED NEXT TO ME HAD A SIMILAR EXPERIENCE ONLY RESETTING THE COMPUTER CORRECTED THE PROBLEM. I PREVIOUSLY OWNED THE SAME MODEL TESLA AND HAD THE SAME PROBLEM SEVERAL TIMES AND THE SERVICE DEPARTMENT LOCATED IN CINCINNATI COULD NOT FIND A PROBLEM OR SOLUTION.
The contact owns a 2018 Tesla Model S. The contact stated while driving at various speeds, the instrument cluster and home screen was intermittently inoperable. The contact called a service center on several occasions and was informed that the failure was linked to a software update and a full onboard computer folder. Despite the repair, the failure persisted. The manufacturer was not notified of the failure. The vehicle was not repaired. The failure mileage was approximately 20,000.
TL* THE CONTACT OWNS A 2018 TESLA MODEL S. WHILE ATTEMPTING TO MAKE A LEFT TURN FROM A PARKING SPACE, THE VEHICLE ACCELERATED BACKWARDS IN A TIGHT CIRCLE. THE VEHICLE STRUCK SOME TREES ON THE PASSENGER SIDE AND CRASHED INTO ANOTHER VEHICLE THAT WAS PARALLEL PARKED ON THE SAME STREET. THERE WAS NO WARNING INDICATOR ILLUMINATED. A POLICE REPORT WAS FILED. THERE WERE NO INJURIES. THE VEHICLE WAS DAMAGED AND TOWED TO A TOW YARD. THE CONTACT CALLED THE MANUFACTURER AND WAS CONNECTED TO TESLA ROADSIDE ASSISTANCE. THE CONTACT REQUESTED THE DEALER. TESLA (831-264-6896, LOCATED AT 1901 DEL MONTE BLVD, SEASIDE, CA 93955) WAS NOTIFIED, BUT DID NOT ASSIST. THE CONTACT DID NOT KNOW IF THE VEHICLE WAS TOTALED. THE CAUSE OF THE FAILURE WAS NOT DETERMINED. THE FAILURE MILEAGE WAS 4,757.
MY CAR WAS FULLY ENGAGED IN AUTO PARK AND IT HIT A CAR. I QUICKLY BRAKED TO STOP A WORSE COLLISION. I PATIENTLY WAITED FOR TESLA TO REVIEW THEIR LOGS. THEY INSIST THE AUTO PARK WAS WORKING WITHOUT FAULT."THE ENGINEERS HAVE INDEED SAID THE AUTOPARK FEATURE WAS OPERATING WITHOUT FAULT." I KEEP QUESTIONING THEM. "IF THE VEHICLE COMES INTO CONTACT WITH ANOTHER VEHICLE DOES NOT MEAN THE AUTOPARK FEATURE IS OPERATING WITH FAULT,' SAYS TESLA. I REPLY, 1. WOULD YOU CLARIFY IT AND SAY THAT IF THE VEHICLE COMES INTO CONTACT WITH ANOTHER VEHICLE WITH THE AUTO PARK ACTIVATED AND THE DRIVER NOT GRABBING THE WHEEL TO INTERRUPT THE PROCESS THAT THE AUTO PARK FEATURE IS OPERATING WITHOUT FAULT? 2. I REPEAT MY QUESTION AGAIN. DO THE ENGINEERS HAVE ANY REASON OR PROOF TO BELIEVE I ACTIVATED THE AUTO PARK INCORRECTLY OR IN ANYWAY INTERRUPTED THE PROGRAM TO CAUSE THE ACCIDENT? I UNDERSTAND THEY DON'T WANT TO PAY FOR REPAIRS BUT I WANT THEM TO EITHER ADMIT THE FEATURE WAS AT FAULT OR CONVINCE ME I DID SOMETHING WRONG TO CAUSE THE ACCIDENT TO HELP PREVENT ANOTHER ACCIDENT. I THINK OTHER TESLA DRIVERS SHOULD BE AWARE OF HOW POORLY THIS WORKS. ALSO, I HAD BROUGHT THE CAR IN BEFOREHAND TELLING THEM THAT I DIDN'T BELIEVE THE AUTO PARK WORKED CORRECTLY. A TESLA SALESPERSON ALSO GOT IN THE CAR WITH ME AND AGREED THAT THE AUTOPARK DID NOT SEEM TO WORK. THIS PROCEEDED THE ACCIDENT. ONCE AGAIN A REPORT THAT THE ENGINEERS SEE NO FAULT.

THIS DOES NOT APPEAR TO HAVE BEEN CAUSED BY IMPACT DAMAGE, IT HAS SIMPLY SNAPPED OFF. CLOSE UP PICTURE SHOWS THAT THE SUSPENSION LINKS AND LOWER ARM HAVE SNAPPED. THIS 2018 CAR IS STILL FITTED WITH THE OLD STYLE FRAGILE LINKS THAT ARE SUPPOSED TO HAVE BEEN REPLACED WITH STRONGER ONES AS NOTIFIED IN TSB : HTTPS://STATIC.NHTSA.GOV/ODI/TSBS/2019/MC-10153421-9999.PDF THERE SEEM TO BE MANY CARS ON THE ROAD THAT ARE STILL FITTED WITH THESE WEAK PARTS. ALSO THE CAST ALUMINIUM LOWER SUSPENSION ARM HAS SNAPPED. MANY SIMILAR FAILURES HAVE BEEN REPORTED. I INTEND TO INFORM THE MEDIA AND MOTORING ORGANISATIONS ABOUT THESE FAILURES AND THAT NHTSA IS FAILING TO TAKE ANY ACTION. I AM NOT AN OWNER. I AM A CONCERNED CITIZEN. BACKUP LIGHTS ARE INADEQUATELY BRIGHT AT NIGHT, IN DARK SITUATIONS ALLOWING NO ABILITY TO MAKE OUT GROUND FEATURES WHEN BACKING UP. TESLA HAS UPDATED FIRMWARE AND REPLACED CAMERA WITH NO IMPROVEMENT. SAFETY DEFECT OF BEING UNABLE TO SEE BEHIND CAR WHEN BACKING UP AT NIGHT NOT FIXED, SEEMS TO BE A DESIGN PROBLEM WITH THE CAR.
CAR WAS STOPPED PRIOR TO ENTERING CAR GARAGE. CAR SUDDENLY REVERSED THEN SPED UP AUTONOMOUSLY AND CRASHED INTO THE HOME. SUSPECT CAR ELECTRONICS COMPROMISED AND ENDANGERED DRIVER. REQUESTING INVESTIGATION IF CAR DEFECT VS USER ERROR. THANK YOU !

--- System: Autopilot, emergency braking, collision detection. Vehicle IS available for inspection if requested. --- How safety was put at risk: In 2018, while driving on the interstate at about 70-80 mph with Autopilot enabled the car suddenly braked for no reason. By our estimates the car's velocity changed by about -20 mph in a very short time. There was no debris in the road, obstruction or reason for the car to do this. If there had been a car behind us it would have resulted in an accident. --- Has problem been confirmed by service center: Yes --- Has vehicle been inspected by manufacturer: Yes --- When we contacted Tesla they initially denied anything happened but when we took it to another service center a different engineer confirmed that something happened but would not provide the logs. After 6 months of fighting with them trying to get more information we gave up and had to stop using Autopilot due to safety concerns. --- Were there any warnings: No. It was abrupt and frightening. --- Additional: I have email correspondences with the service center and invoices if requested. However due to their extensive nature I'd rather speak to someone at the NHTSA before gathering the documents.
I WAS DRIVING HOME ON THE FREEWAY. ON MY RIGHT WAS AN EXIT RAMP AND IN THE LEFT LANE THERE WAS NOT ANOTHER VEHICLE. SUDDENLY THE WARNING BUZZER WENT OFF AND THE CAR PULLED SHARPLY TO THE RIGHT. I QUICKLY CORRECTED TO THE RIGHT. IT SCARED ME. IN ADDITION I CAME HOME AND DROVE INTO MY GARAGE LIKE I ALWAYS DO. THE BUZZER WENT OFF AGAIN TELLING SOMETHING WAS IN THE WAY AND SHIFTED ITSELF INTO PARK. I GOT OUT OF CAR AND CHECKED TO SEE IF ANYTHING WAS IN THE WAY. THERE WAS NOT. I CALLED TESLA STATING THESE THINGS WERE SAFETY ISSUES. THEY WANTED ME TO DRIVE THE CAR IN TO THE SERVICE CENTER. I TOLD THEM MY 3 AND 6 YEAR OLD GRANDKIDS WHERE STAYING WITH ME AND I DID NOT FEEL SAFE DOING THAT. AFTER CALLING CONTINUOUSLY FOR 2 DAYS, A LOANER WAS BROUGHT TO MY HOUSE AND MY TESLA WAS DRIVEN (NOT TOWED) TO COSTA MESA SERVICE CENTER.

THE INSURANCE COMPANY TOWED THE VEHICLE TO A COLLISION REPAIR FACILITY. THE DEALER (SAN DIEGO MIRAMAR TESLA, 9250 TRADE PL, SAN DIEGO, CA 92121, (858) 271-5100) WAS NOTIFIED OF THE CRASH AND THE CONTACT WAS WAITING FOR FEEDBACK FROM THE INSURANCE COMPANY. THE DEALER AND THE MANUFACTURER WERE WAITING TO RETRIEVE THE INFORMATION FROM THE ELECTRONIC DATA RECORDER TO INVESTIGATE THE CAUSE OF THE FAILURE. THE APPROXIMATE FAILURE MILEAGE WAS 49. *BF*JB
THERE HAS BEEN SO MANY MALFUNCTIONS, DEATHS AND FIRES CAUSED BY TESLA'S AUTO PILOT FEATURE. SO MANY GOVERNMENT INVESTIGATIONS ARE GOING ON, AND THE RESULT OF SOME INVESTIGATIONS THAT HAVE BEEN COMPLETED SOUNDS ALARMING. TESLA'S AUTO PILOT FEATURE IS UNDER INTENSE GOVERNMENT SCRUTINY. TODAY, ELON ANNOUNCED THAT ALL TESLA CARS WILL BE FULLY AUTOMATED DRIVERLESS IN AUGUST. ELON HAS BECOME DESPERATE, INSANE AND DANGEROUS. HE HAS NO REGARD FOR PUBLIC SAFETY. IS HE CRAZY? ARE YOU KIDDING ME? PLEASE STOP THIS DANGEROUS AND FRAUDULENT MAN, AND PUT HIM IN PRISON FOREVER. HE IS IRRATIONAL, CRUEL AND LETHAL. HOWEVER, SADDAM HOSSEIN DID SO AND KILLED SO MANY IRANIAN SOLDIERS DURING THE 8 YEAR WAR BETWEEN IRAN AND IRAQ. I SAW PICTURES OF SO MANY IRANIAN SEVERELY INJURED AND BURNED SOLDIERS BEING AIR LIFTED TO GERMANY FROM THE WAR ZONE, BECAUSE IRAN DIDN'T HAVE THE MEDICAL CAPABILITY TO HANDLE THIS SITUATION. NOTHING CAN BE DONE ANYWAYS. ONCE YOU ARE EXPOSED TO MUSTARD GAS, YOU'RE FINISHED. IT'S JUST THE MATTER OF QUICK DEATH, OR VERY PAINFUL PROLONGED DEATH. ELON NOW HAS TURNED TO CHEMICAL WEAPONS OUT OF DESPERATION JUST LIKE SADDAM HOSSEIN AND BASHAR ASSAD. THANKS.

NO INJURIES WERE REPORTED. A POLICE REPORT WAS TAKEN AT THE SCENE. THE VEHICLE WAS TOWED AWAY. THE CAUSE OF THE FAILURE WAS NOT DETERMINED. THE LOCAL DEALER TESLA OF HONOLULU (1450 ALA MOANA BLVD, HONOLULU HI) WAS NOTIFIED OF THE FAILURE. THE CONTACT STATED THAT AFTER THE VEHICLE WAS REPAIRED THE SAME FAILURE HAD OCCURRED WHILE PARKING CAUSING THE DRIVER TO LOSE CONTROL OF THE VEHICLE AND CRASH INTO A CONCRETE WALL. DURING THE CRASH THE DRIVER SUSTAINED A CHEST INJURY DUE TO THE AIR BAG DEPLOYING WHICH REQUIRED MEDICAL TREATMENT. NO POLICE REPORT WAS TAKEN. THE VEHICLE WAS TOWED AWAY. THE CAUSE OF THE FAILURE WAS NOT DETERMINED. THE CONTACT ALSO STATED THAT WHILE DRIVING THE INSTRUMENT DISPLAY PAD HAD SUDDENLY BLACKED OUT ON MULTIPLE OCCASIONS AND THE DOORS HAD FAILED TO OPEN ON SEVERAL OCCASIONS. THE CAUSE OF THE FAILURES WERE NOT DETERMINED. THE FAILURE MILEAGE WAS 7,410. OWNER IS EXTREMELY SCARED TO DRIVE AGAIN. CAR IS SITTING IN THE GARAGE. WORRIED ABOUT ANYONE DRIVING THE CAR MAY GET INTO A FATAL ACCIDENT.*DT*JB
TL* THE CONTACT OWNS A 2018 TESLA MODEL S. WHILE ATTEMPTING TO PARK THE VEHICLE, IT INDEPENDENTLY ACCELERATED AND CRASHED INTO A WALL. THE AIR BAGS DEPLOYED. A POLICE REPORT WAS NOT FILED. THE CONTACT SOUGHT MEDICAL ATTENTION TEN DAYS LATER FOR A STERNAL FRACTURE. THE TESLA BUENA PARK DEALER (6692 AUTO CENTER DR., BUENA PARK, CA 90621, (714) 735-5696) WAS CONTACTED AND THE VEHICLE WAS TOWED TO A TESLA APPROVED FACILITY TO BE DIAGNOSED. THE CONTACT WAS INFORMED THAT THE ON BOARD DIAGNOSTIC TESTING REPORTED THAT THE ACCELERATOR PEDAL WAS DEPRESSED AT THE TIME OF THE CRASH. THE CONTACT WAS ALSO INFORMED THAT THE AUTOMATED FEATURES, INCLUDING THE CRUISE CONTROL WERE UNABLE TO HAVE INDUCED THE ACCELERATED RESPONSE. THE VEHICLE WAS NOT REPAIRED. THE MANUFACTURER WAS ALSO NOTIFIED OF THE FAILURE. THE APPROXIMATE FAILURE MILEAGE WAS 214.
