Close the door and the rear view window shattered.
During snowy but manageable conditions, the brake system on my 2021 RAV4 Prime SE failed to respond, and the vehicle did not decelerate when I pressed the brake pedal. A “Pre-Collision System Malfunction” warning appeared shortly before impact. I was unable to steer away despite attempting an evasive turn. The vehicle collided with the car in front of me, and the airbags did not deploy. I was driving at a reduced speed due to snow. The vehicle ahead (a BMW SUV) began slowing. When I applied the brakes, my RAV4 Prime did not slow down at all—there was no deceleration, no ABS pulsing, and no traction control intervention. The vehicle continued moving forward as if the brakes were not engaging. At the same time, I saw a dashboard warning indicating a Pre-Collision System (PCS) malfunction. I attempted to turn the wheel to avoid the collision, but the vehicle did not respond to steering input quickly enough to prevent impact. Despite the collision, no airbags deployed. The vehicle has been regularly maintained (oil changes every 5,000 miles, new tires last year, all service intervals followed), and there were no warning lights or issues beforehand. Safety Concerns: •Complete or partial brake failure •Electronic braking or stability control malfunction •Pre-Collision System malfunction •Airbag non-deployment in a front impact •Potential systemic issue affecting Toyota hybrid/Prime models This incident caused significant fear and distress for both me and my child who was in the vehicle, and raises serious concerns about whether similar failures could occur for other owners.
The car has a grinding sound and a brake slipping feeling that occurs while traveling and braking over rough pavement. It does not happen consistently, but it does happen fairly regularly (every 2-4 weeks). Note, the grinding sound can range from subtle to quite loud, depending on bump/timing of braking. This has been happening since I purchased the car and continues to happen, which is why I put today's date as the incident date, and I have been bringing it up with the Toyota maintenance department at most maintenance appointments. Their shop manager has driven it and thought I was mistaking it for the ABS activating. I have driven it with him, ABS has activated, and when he pointed it out to me, I have let him know that what is happening is definitely something different than the ABS activating. I have tried to find a location to show them to replicate it with the service center, but it happens randomly on rough patches. For example, it happened on a spot near the dealership that I thought I could use to show them, but when I drove over the same area again and braked, nothing happened. So, I have not been able to replicate this with the service center, but it is most certainly happening. I have notified Toyota through their consumer contact system that this issue is going on and they said Toyota tracks issues like this. However, no resolution has been offered. Other Toyota Rav4 Prime owners of the same year have experienced this (see [XXX] and [XXX] ). Although the maintenance manager was stumped, he said at a recent visit that another Prime owner voiced the same type of thing happening. Toyota needs to investigate to determine what the issue is and remedy the issue. I am concerned this could lead to a larger issue and is a safety recall issue that warrants immediate attention. On July 24th, I experienced sudden unexpected acceleration with my 2021 Toyota Rav4 Prime XSE (41,600 miles) while driving in the electric mode. I was lining myself up to reverse into my garage and as I paused to switch gears to reverse, the car suddenly accelerated full-speed towards my neighbor's house. I slammed the brakes but that had little effect. The car slammed into the house, through the garage door and my car was only stopped when hitting an SUV parked in the garage, pushing it forward about 2 feet. No air bags were deployed, the collision detection system on the vehicle did not seem to work and tire marks from my braking were visible from the middle of the road. I have found a number of very similar incidents with Toyota cars described online. Cars were often turning into parking spaces at slow speeds and ended up accelerating into lamp posts or buildings. If there hadn't been a car parked in the garage, I can't imagine what might have happened as the car was probably going 35 mph+ upon impact. There were no warning lights and the collision shop informed me that there were no diagnostic codes when they downloaded the computer.
2021 Toyota Rav4 Prime, 69K miles ***** On [XXX] while driving at highway speeds, many warning messages began to scroll across the display. The gas pedal immediately became unresponsive. The power steering stopped working. The car was unable to be stopped with the brake pedal, and it had to be allowed to coast stop. ***** The car was then towed to Fred Anderson Toyota in Asheville, NC. The service advisor refused to scan the codes and said that all I needed to do was change the battery. I purchased a battery from Fred Anderson and changed it myself. The car turned on without any warning lights. ***** I started driving the car home. After approximately 45 minutes of driving, while at highway speed, the same dangerous situation occurred: Many warning messages began to scroll across the display. The gas pedal immediately became unresponsive. The power steering stopped working. I was unable to stop the car with the brake pedal, and it had to be allowed to coast stop. ***** Twice that same day, this vehicle lost all power and turned off power steering and braking without any prior warning. [XXX] that nobody was injured during either occurrence that day.
Upon entering a light controlled intersection at a relatively slow speed (probably less than 20 mph} on wet pavement, an oncoming car suddenly made a left turn directly in front of me. I applied the applied the breaks hard and slid into the passenger side of the turning vehicle. The ABS system did not function. The air bag did not deploy. At the moment of the collision my speed was reduced and I do not know the threshold for airbags to deploy, but this was a direct front end collision. I was able to brace myself and was not injured. I am not claiming the airbag malfunctioned as the speed at the moment of impact was not high. However, I have complained to the Toyota dealer service department almost since the car was new, that the ABS does not function. They have repeatedly told me that it is fully operational based only on the fact that the system registers as "on." Being "on" does not mean it is functioning. Questions: What component failed? ABS and possibly airbag How was my safety at risk? Non functioning ABS resulting in a potentially preventable accident Has the problem been confirmed? Yes, byt the dealer though they claim the sliding of the car is due to old tires. However, the tires are withing the legal limist for tread depth and I have complained about the problem since the car and the tires were nearly new. Has the vehicle been inspected? yes, byt the Toyota dealer and an independent Toyota crash inspector Were there any warning lights etc? No
I was driving along an expressway on a rainy night. The plug-in hybrid car was operating on battery power. Suddenly the motor went dead and it slowed to a stop. This was very dangerous because the expressway traffic had to swerve around me. I may have seen a hybrid system malfunction warning on the display, but I was so distraught that I didn't think to take a photo. After about 5 minutes, I was able to start the car and drive away. The Toyota dealer examined the car later and could find nothing wrong.
Spontaneous Shattering of Sunroof Glass while driving While driving westbound on [XXX] near Cambridge, Ohio at approximately 6:05 PM my wife and I heard what sounded like an explosion or shotgun blast on the roof of the car. We had a Thule cargo box on top, so my immediate reaction was it has blown open. I pulled off the road and opened the shade to the sunroof to see if the Thule box was damaged. It wasn't the box, it was the glass of the sunroof, which had spontaneously shattered. Pieces of glass rained down on us. Thankfully, the shade was closed, or, as the driver I would have been showered with glass while driving at 65 mph. Had we been in traffic when the explosive shattering of the glass occurred, this would have made it a dangerous event. Further, had we not had the shade closed, we would have been showered with glass fragments. There was no obvious cause. No cars were in front of us to have thrown up a rock. The Thule cargo box protected the glass from something falling from the sky. The only cause could be the glass spontaneously shattered. I had cleaned the car prior to our departure. I cleaned all of the glass surfaces, including the sunroof. There were no marks, visible damage, or scratches visible. I tried to take the car to a Toyota dealer in Oshkosh, WI, but they were unable to see me. When we returned to Maryland, I took it to Jim Coleman Toyota. I explained what had occurred and gave them photos. The information was sent to Toyota and they declined to cover the sunroof. I did a quick search after the incident and learned that this is not the first case of the sunroof glass of a 2021 RAV4 has spontaneously shattered. Toyota refuses to accept that the glass may have a manufacturing defect. The Toyota dealer estimates the cost of repair to be about $1900 and I have filed a claim against my USAA auto insurance. I was driving very slowly in my driveway. I turned to enter my garage. As I put my foot on the break, the car accelerated crashing into my house. There was sudden unexplained and unintended acceleration on my RAV4 prime.
My 2021 Toyota RAV4 Prime had traction control, braking issues and failure to warn in the event of a possible forward collision. On a few occasions the brakes seem to just slide and the car would keep moving. Last week I was in an accident and the emergency braking system, the forward collision warning system and all the air bags failed. The California Highway Patrol was surprised that even though the estimation collision speed was 60 mph the air bags did not deploy. I would have not had a chance to test out the air bags, emergency braking system or the forward collision warning prior to the accident.

I was pulling into a parking space at a slow speed and breaking when my car accelerated violently and caused me to go across a median and strike another parked vehicle. Police came and did a report. I took my car right away to dealer and they had me take it to their collision shop to be repaired. They said they would try to find the unintended acceleration problem after it was repaired. No warning lights or airbags.

When engaged, this system cannot tell the difference between traffic lanes/markings and groves on the road/lines separating different coasts of asphalt and will try to steer the vehicle following those lines. It constantly tries to drive the car into the medians
When driving over small bumps while slowing down will cause the rear brakes to lock
I am writing to register a complaint about the lack of resolution for the below recall number. the notice, from 3 months ago now, says not to use the electric charger for the vehicle in temperatures below 40 F. I expected a rapid rollout due to the approaching winter but instead Toyota is still saying that there is no solution. The notice specifically says there is a risk of vehicle fires. Toyota appears to know what part has to be replaced and should be required to do that in a timely manner so the car can be used as designed in a safe manner. I encourage NHTSA to look into this as soon as possible. I spoke with a Toyota representative today and was given the same vague timeline but also told it could take until Spring 2024 for a solution to be identified. Thank you for your assistance. When braking and hitting a slight bump in the road or on a wet road it actually like my car is accelerating, seems to be a possible issue with the regenerative brake system and the antilock brake system software working together. Luckily i have not hit anyone, but this is a major safety flaw. Has happened multiple times.
When pulling into a parking spot at less than 5 mph the car accelerated violently and couldn't be stopped while braking. A marble or granite barrier stopped the my car preventing me from going into a shallow ravine and potential on coming traffic. There have been multiple safety recalls on the braking system on the car and each have been executed in a timely fashion. I cannot determine if warning lights went off at the time due to how quickly the issue happened

I received the recall letter (23TB07) 7/12/2023 stating that my RAV4 had potential to start on fire. I have called several times and been told that on this date (11/2/2023), almost 5 months later, there is still no remedy and that there is no projected date for remedy.
Toyota issued a recall in July 2023 stating the the DC-DC conversion system may have been damaged in manufacture, potentially causing a short, and possibly leading to a fire if charged at less than 41 degrees. Toyota indicated that a fix would be available in the 4th quarter of 2023. We are nearing the end of the 4th quarter and fix is still not available. Living in upstate New York, nighttime temperatures frequently are less than 41 degrees. As a result I am reluctant to charge overnight. This defeats the purpose of a plug-in hybrid, resulting in higher fuel costs and excess vehicle pollution, not to mention additional anxiety due to potential fire! This is an unreasonable period of time to wait for such a potentially serious defect.

The manufacturer issued a recall 10 months ago warning of an electrical charging system issue. They still do not have replacement part available for dealers to repair/replace this. We paid $50,000 for a plug in vehicle that could potentially overheat and catch fire inside our garage. The manufacturer has not provided a repair in a reasonable amount of time.
Toyota issued a recall in July 2023 for risk of fire, charging the vehicle below 41 degrees ambient temperature. It has been 5 months to remedy the situation. Over 40,000 vehicles are affected. In Western New York we are experiencing colder temperatures, making it impossible to charge without a risk of fire. I am now having to purchase gas at an higher cost, instead of home charging. My multiple calls to Toyota corporate have not given me a timeline for a solution. If I want to trade my vehicle, I will get a greatly reduced value due to the open active recall.
There is a long-outstanding recall on the external charging system of this vehicle. Toyota delayed the original date of parts availability to 4th quarter 2023. There are no updates to the status of this recall or availability of the repair. I contacted Toyota corporate and they do not have the parts or an expected date and could not offer a loaner vehicle of the same model. Worse yet, if they provided a loaner vehicle, they wanted me to leave my vehicle at a dealership for the entire time of the loaner - with no announced date for the recall repair. I also emailed the Chairman of Toyota directly, but have received no response. Since the risk of this defect is reported to occur at temperatures of 41 or less, it is imperative that Toyota make the parts/repair available immediately, as cold weather has arrived in the US. I request that you immediately contact the manufacturer and issue an update to this recall - hopefully including Toyota actually repairing the affected vehicles, rather than continuing to delay and exposing people and property to risk. It is becoming apparent that this issue may be without a remedy and absent an immediate repair announcement from Toyota, I suggest that you require Toyota to repurchase all of the affected vehicles at full new vehicle replacement cost.

Went to unplug my charger from 2021 RAV4 prime in May 2023 which was who installed this 220 outlet. He confirms the 220 outlet which was installed previous yr is safe, correcoutside in NH. I found handle of charger was extremely hot to touch and it would not release from car. Locked and unlocked vehicle again with key fob but still it would not release. Also attempted to lock /unlock using door switch with no success. Due to the extreme heat of charger at both ends I needed to get something to protect hand from heat. I unplugged charger from the 220 outlet installed by electrician for this car but the charger handle would not release from car until it cooled which took between 5- 10 minutes. Brought back electrician t for job and working but reported the charger was so hot the heat affected the plug.I am lucky the house and car did not ignite. I used my level 1 charger for next 2 months as I was still afraid of fire from the April hot level 2 charger on both ends incident. Eventually with electrician’s knowledge & comfort with this 220 outlet also knowing we have 2 electric boxes for load of house I purchased a new level 2 charging system in June. All has been working as expected. July I happened to see in my Toyota app a notice there is a safety recall for a potential fire risk. I was not sent any notice and lucky I saw this in the Toyota app. I called Toyota to report my incident there was no concern to make a report of my incident they only stated this fire risk is when charging below 41 degrees and they are hoping for Q4th 2023 fix. There was no concern for what if temps had dropped, financial burden it placed having to purchase new charging station, bringing out electrician again or that I live in North where temps are below 41 months at a time.I had this issue in May when temps were below 41 and haven’t charged below this temp since it’s summer. I do not dare charge overnight when electric supply is more economical as temps can drop.
Car won't accelerate for about 1 to 2 seconds from stopped. Didn't have the issue before the 23TA01 recall performed at the Dealership.
On the driver's display, some Vehicle Settings menu pages do not display properly. The text either appears too dim to read or displays legibly but then disappears. The problem is inconsistent and difficult to replicate. This behavior prevents proper setting or adjustment; and may confuse the driver when vehicle is in operation.
Toyota dealerships stated that there is no fix for recalled issue and not to charge the vehicle with the charger and to park it outside my home due to a Potential fire concern We are going on almost a year on this recall and there is still no fix. Recall Code: 23TB07.
This is a rotating car seat. At first the seat would rotator from side to rear facing fine. After 5/6 weeks it didn't rotate all the way to rear facing without using the release button. Then when locked in the rear facing position when applying minimal force to the seat it rotates without the use of the release button. After removing the seat from my vehicle the seat was stuck on the side facing position and would not enter either the rear or front facing position. The contact stated that the car seat was located in the back seat on the passenger side. The manufacturer requested that the seat be sent back. The contact was refunded the purchase amount.
RE: RECALL # 23TA07 for 201 Toyota Rav Prime. An interim recall notice was issued a year ago stating "We are currently preparing a remedy. We will notify you again when remedy is available." Toyota Corporate has no information when this recall will be addressed. I paid over $5000 to get a plug- in hybrid and now cannot use the plug- in feature when temperature is below 40 degrees in the winter. The car is useless as designed.
Husband and I bought our 2021 RAV4 Prime from Toyota last Jan, brand new and I’ve had no complaints. On Dec 27, 2022 we were running errands and all of a sudden our sunroof EXPLODED. It sounded like a gun went off right in between us and then showered us with glass residue. There were no cars around us so we immediately pull into a parking lot and notice the glass exploded up and out. We took our car immediately to a Toyota dealership within half a mile—they contacted the main Toyota office with our situation and Toyota insisted it must’ve been from road debris hitting the sunroof and then the internal cabin pressure caused the glass to go up/out. However, we were driving with our sunroof tilted up and our windows down so that would’ve been impossible as internal pressure was equilibrated with outside the vehicle. Additionally the tension point was located directly under our roof rack so would’ve been impossible for debris to hit it at that point since it would’ve been protected by the roof rack itself. There was no other damage done to our car. Toyota is not helping us with this situation and we are having to use insurance. After reading other similar incidences, I was shocked how many others have experienced this on Toyota nation, Reddit, and NHTSA. It’s dangerous and is a safety concern for drivers, passengers and other vehicles on the road specifically the loud explosion sound itself, the risk for glass residue to impair vision of the driver and glass shards flying off the roof and hitting other vehicles on the road.
