This page combines three types of NHTSA data: recall campaigns (official manufacturer or government actions), owner complaints (unverified consumer reports), and crash test ratings (where available). A vehicle with many complaints is not necessarily less reliable — complaint volume correlates with sales volume and vehicle age. Recalls indicate identified defects, not overall quality. To compare this model year with others, use the year navigation in the sidebar or return to the model overview page.

I am reporting a serious safety issue involving my vehicle where the braking system did not respond as expected during an emergency. While traveling southbound on I-575 in Georgia, I experienced a sudden tire blowout. As I moved to the shoulder and applied the brakes firmly, the vehicle did not slow down as expected. Despite pressing the brake pedal continuously and with significant force, the vehicle continued traveling for approximately 1–2 miles before finally coming to a stop near Exit 11. This was a highly dangerous situation, as the brakes appeared to be ineffective during a critical moment. Fortunately, there was no crash or injury. After the incident, the vehicle was towed to a Toyota service center (Cherokee Toyota) where the damaged tire was repaired/replaced. I was informed that no other mechanical issues were found. However, I remain concerned that the braking system may have a defect or intermittent issue that was not detected during a standard inspection. Given the severity of the incident, I request a thorough investigation into the braking system performance under such conditions. A failure or significant delay in braking response presents a major safety risk.

The contact owns a 2024 Toyota Rav4 Prime. The contact stated that while driving at an undisclosed speed, several unknown warning lights illuminated. The contact stated that while turning off the vehicle, the check engine warning light and the malfunction warning light illuminated. The vehicle was towed to the dealer, where it was diagnosed and determined that rusted wiring cable had caused the failure. The vehicle was not repaired. The contact was informed that the part needed to be ordered to repair the vehicle. The manufacturer was not made aware of the failure. The approximate failure mileage was 8,000.
The car was under warranty and experienced a transaxle failure while on a highway between Yuma, AZ and Tucson, AZ. The dealer who inspected and repaired the car determined the cause to be a flawed cooler replacement by a body shop in which transmission fluid was not properly replaced subsequent to a left front fender bender accident, which voided the warranty. (Fluids were dry on inspection; no leaks/issues identified at our residence or during "Toyota Care" X2 following the body shop repair.) The car worked flawlessly for one year after the body shop repair until the transaxle failed. Issue: There were no warning messages to indicate a problem with the fluid level until immediately before the catastrophic failure; we were able to get the car to the side of the road right before the wheels locked but had we not, a serious crash could have occurred. Toyota tells us there is no alert for fluids since they have designed the transaxle as a "closed system". However, conditions exist that could compromise this closed system so consumers (and service dealerships) should have a way to proactively identify a fluid level problem so it can be remedied to avoid potential safety hazards related to an abrupt failure.

the back glass spontaneously shattered
Road debris damaged AC condenser that cools batteries of RAV4 Prime. A quick google search yields many other cases over multiple model years highlighting a design failure on the part of Toyota. Not only is the repair costly, if not quickly remedied could lead to damaged hybrid batteries and possibly even fire.
I was driving on a busy road at rush-hour. I was behind a school bus at a light. I stopped the car the way I always do but the car kept inching up very very slowly. I could not stop the car. It just kept moving until I hit the back of the bus!!! there was no damage to the bus, but my vehicle has a lot of damage on it. It was almost new 2024 RAV4 prime!!! I have since read that there have been a lot of recalls on this vehicle for brake problems. I could not drive the car and had to have it towed to the dealership Of course they say they know nothing of anything like this and were very uncooperative. I have been leasing the car with the agreement to buy it outright at the end of the term and now I am afraid of this car! I am a senior citizen, and I drive my grandchildren all over.. please help!!
Hazard (fishtailing risk): In rain with LTA ON, small but critical driver steering corrections near center are overridden by EPS/LTA, which apply centering torque into steering dual compliance creating a real steering wheel deadband canceling the driver’s turning of the wheel across the deadband; then the steered wheels snap abruptly to the steering wheel position. The significantly delayed response and snap overcorrection, caused cause fishtailing (causing a miss with a car, then a semi), just like the car suddenly had loose ball joints. Conditions: 40–50 mph, intermittent heavy rain, wipers high, spray, occasional false lane detections. Dealer response: Acknowledged the general behaviour. Refused diagnostics; advised turning LTA OFF or lowering sensitivity. Reference: Technical appendix in Toyota.txt (mechanism, thresholds, dual‑compliance explanation, SAE 2019‑01‑1516). SAE Technical Paper 2019-01-1516 (Fu et al., "Control Strategy to Reduce Torque Oscillation of the Electric Power Steering System") Observer credentials: Retired 39-year integrated circuit design engineer; 23 US patents (8 involving nonlinear feedback/deadband control).
The electronic dashboard with speedometer, and information regarding mechanical functions and fuel has gone blank. This recall has been issued however, Toyota does not yet have a fix. They are estimating sometime in December, however no specific date for a fix. I have followed up with the Toyota dealer who stated that I should file a complaint regarding the delay in a fix.

Moonroof spontaneously exploded while driving on the highway. The moonroof was closed, no one was trying to open it. All windows were closed, no one was trying to open them. My husband was driving and differently we heard a loud explosion which sounded like a shotgun going off and thought someone was shooting at us. I looked behind the car and could see glass flying everywhere. We safely pulled over and the entire moon roof had randomly and spontaneous exploded. No collision. Nothing fell on us etc. A quick Google search with show you that this is not uncommon for my car's make and model. Something needs to be done! It was terrifying and could have killed is all in the car from the panic hat ensure the
While driving at no more than 5 mph, looking for a parking space, the car suddenly accelerated dramatically. I vigorously applied pressure to the brake but the car continued to go forward, sideswiping a parked pickup truck and crashing through a metal fence and coming up against a rock wall. Fortunately, there were no pedestrians walking through the parking lot and no one was injured. I don’t know if the problem has been reproduced or confirmed by a dealer or independent service center. The vehicle has been inspected by an inspector hired by the manufacturer and also inspected by my auto insurance representative. There were no warning lamps, messages, or symptons of this sudden unintended acceleration prior to the failure.
The driver was pulling into a friend's carport. After crossing a small curb her foot was taken off the gas and was moving to the brake. The car was going slightly uphill as slow as a car can go with no gas pedal depressed. The car then accelerated as if someone pressed the gas to the floor. In a matter of 8 feet it gained enough speed and momentum that when it hit our friend's parked car the 4 door SUV was pushed forward 4 feet. The gas pedal was never pushed by the driver. No failed component was ever identified by multiple inspections by the Toyota dealership, the insurance company, the body shop and the 3rd party company hired by Toyota to download the ACM data and the Vehicle Control History. We had filed a complaint with the Toyota Brand Engagement center hoping Toyota would make a proper investigation. A police report was made also. No one was injured but the homeowner was standing a few feet away. It could have been much worse. There were no warning messages, the emergency braking did not engage, and the airbags did not deploy. Earlier in the day the car's proximity sensors were beeping with no indication of what was setting them off. It took 3 cycles of park to drive to stop the beeps. The driver even walked around the car to try to identify what was setting it off. After the crash the passenger side rear window went down by itself and the rear liftgate went up. It took 7 weeks for Toyota Corp Legal to respond saying they found nothing wrong. To this day they have not shared the Vehicle Control History raw data with us. Their response made no mention of them looking into a failure of the ADAS system with regard to the Cruise Control or any other sensors that send signals to the throttle. I am attaching their response and various other inspects and reports. Also a ring video of the crash poorly converted to a gif. No option to attach a video is in the instructions.
My girlfriend's father was driving at normal driving speed on the highway when the tire blew out for no reason. There are multiple blown out sections of the tire on both the internal and external sides of the tire that would appear to be a structural failure. This was definitely not a puncture due to the failures completely around the entire circumference on both sides of the tire. The tire was inflated as per spec. This caused vehicle instability and was a dangerous situation so he had to quickly pull to the side of the highway. The tire is available for inspection. I have not yet taken the car or tire to the shop for inspection. There were no warning lamps or other symptoms prior to the blowout. The tire size is: 235 / 55 R 19 101V
Windshield cracked by itself while pulling out from home garage. No visible signs of hit by foreign object. Cannot feel the crack with finger tips from inside or outside. Crack seems started from defroster.
Front driver and passenger electric seats have issues going back and forth and apparently to be bumpy not smooth like they should be
I was stopped at a red light at an intersection, when a car pulled up next to me, and the driver rolled down the window to tell me my vehicle was on fire. I got out of my vehicle while the driver of the other car called 911. Brookline Police & Fire came, firefighters extinguished the fire, and one of the emergency services responders had a wrecker come & take the car away. The car was towed to Watertown, then collected by Safety Insurance, which got a repair estimate of $95,000. The car was deemed a total loss and is in the possession of the insurance company. When the vehicle caught on fire, it had 3,627.4 miles on it. I did not see any warning lamps, messages or other symptoms of the problem prior to the fire. My safety and the safety of others was at risk due to the sudden and unexplained fire.
