Description: On July 7, 2025, I purchased this vehicle from AutoNation Spring, Texas. At the time of sale, the dealership did not disclose that there was an active safety recall on this vehicle. I only learned of the recall after the purchase, when I checked the VIN through NHTSA’s website. The recall remains open and unresolved as of today’s date. The Arizona AutoNation dealership has had possession of the vehicle for service since shortly after the sale, but no recall repair has been completed. I was never informed at the time of sale that this safety defect existed or that repairs were outstanding. I am concerned that the vehicle was sold in violation of federal and/or state consumer protection laws regarding safety recall disclosures. I request NHTSA’s review and investigation into whether this dealership and manufacturer have complied with all requirements for recall handling and notification. --- Key Points for NHTSA’s File: Date of purchase: July 7, 2025. Selling dealer: AutoNation Spring, Texas. Recall was active at time of sale. Recall was not disclosed to buyer. Vehicle remains in dealership possession with recall still open.
TRANSMISSION DOWNSHIFTS WHILE DRIVING TRANSMISSION STAYS IN FIRST GEAR TRANSMISSION BANGS WRENCH APPEARS ON DASH ALL CONTROL IS LOST
The truck drops from sixth gear to fourth gear randomly while at high speeds. I took it in and they told me it was not covered by the recall that was done a couple years ago for this very same reason. I took it into five star Ford in Plano Texas. They told me that recall expired. I took it in then and they supposed changed out faulty parts.
You will be driving down the road and it would downshift gears you guys have a recall on this already but my truck doesn't qualify for it apparently the downshifting is periodically it's not consistent
Truck has 91,200 miles and was well maintained. Truck has a loud rattle (like rocks in a can) on dry start up after sitting for several days. Rattle goes away after starting and is consistent with worn CAM PHASERS, as shown in many videos on the internet describing this known problem with the Ford 3.5L ecoboost engine. Ford issued a customer reimbursement program for repairing this known defect but it does not apply to this vehicle, even though it was built with the defective cam phasers and is showing signs they are destroying themselves, which can lead to catastrophic engine failure. Ford redesigned the cam phasers to a better version after learning of the originally defective design. The repair is very expensive $3k to $5k.
WRENCH displayed today, transmission was hard shifting trying to go in and out of gear
*transmission downshifted while traveling 15 mph *Unsafe condition traveling on roadway *Temporary loss of control *WRENCH appeared, shifted hard into first gear
The contact owns a 2013 Ford F-150. The contact stated that at the initial start, there was a loud rattling sound coming from underneath the hood of the vehicle. There were no warning lights illuminated. The first time the failure occurred, the rattling sound ceased after driving for a while. The failure occurred a second time but continued while driving. The contact stated that after inspecting the vehicle, the contact became aware that the cam phasers were causing the rattling sound. The dealer was contacted, and an appointment was scheduled. The vehicle was not repaired. The manufacturer was informed of the failure and opened a case. The manufacturer referred the contact to the NHTSA Hotline to report the failure. The failure mileage was approximately 63,000.
*transmission DOWNSHIFTS while driving at normal traffic speeds *Sudden stops in traffic *WRENCH appeared on dash, lost all digital numbers, I e. Speed...as soon as downshift

The incident happens while driving home from work. I was on the highway going around 60mph. The vehicle then shifted from 6th gear to 1st gear. My rear tires locked up and the vehicle started to fish tail as it was screeching to a halt. I was able to shift to neutral, maintain control and pull over to the side of the highway. I believe the transmission is the component that malfunctioned. My saftey and the saftey of others was put at risk by the vehicle locking the tires up at highway speeds. I believe if the circumstances had been different, this malfunction could have cause serious injury or death to myself as well as other vehicles around me. The problem has not been reproduced or confirmed by a dealer or independent inspector. the vehicle or component has not been inspected by the manufacturer, police or insurance representatives. Prior warnings or messages are unknown. There were no warning lamps on the vehicle when the incident happened.
driving at speeds of around 55mph it downshifted into 1st gear this happen at least 3 times. luckily no one was close behind when it happened .
Driving on the freeway in traffic, driving approximately 10mph. Started to see smoke come from hood, checked gauges and all gauges were normal, no warning lights on. Pulled over and turned off vehicle and flames started from under the hood. Entire vehicle burned.
I never got a notification regarding the recall for the transmission lead frame code. I just recently started having issues with this problem, where my truck will downshift to 1st gear while driving at highway speeds. This nearly threw me and my two young daughters off the road. I took it to the dealership, and they told me that they are unable to help me due to the 10-year mark being passed, even though my truck has less than 103,000 miles. So now I am out of a vehicle, and Ford is not willing to help.
My 2013 has the same transmission problem of an unexpected downshift to 1st gear while driving at highway speeds as the newer model year F150’s. I would like to see all F150’s with that model transmission (6R80) included into the recall. I believe this would include 2011 through 2018 model years.
Ford dealership Jack Kissee in Claremore, OK is telling me my truck is 6k miles out of warranty for Recall 19N01. I have owned my 2013 F150 for less than 2 years with no knowledge of any previous recalls or repairs needed by Ford. This has not been an ongoing issue and has only happened within the last 1,000 miles I have put on my truck. Wrench light and icon pop up on the dash and puts the truck into limp mode until I can get pulled over and get the truck shut off. Safety risk to everyone on the rode. Please let me know if there is a way this Recall 19N01 can be covered. Thank you.
F150 downshifted into first gear while driving down the highway at full speed. Speedometer stopped registering. Sudden and abrupt deceleration. Engine warning light on. Took the truck to the local Ford dealer, Malloy Ford Charlottesville, VA and Service department said the molded leadframe needed to be replaced per Customer Satifaction Program (CSP) 19N01, but Ford would not cover the cost. I believe that Ford needs to cover the cost. I never received a letter notification from Ford and had it in service at the same dealer in August 2022 and they did not notify me of the outstanding CSP and need to service per 19N01.
Complete, instant brake failure at only 105871 miles. My wife was backing out of the drive @5 mph, brake pedal pushed clear to floor with no warning. 3-4 minutes later, the brake warning light came on. No warning light ahead of time. Reservoir was very low, I added 12 oz. DOT3 fluid…..brakes seemed ok. Got up the next morning , brake low fluid light is on and all 12 oz of brake fluid are gone, not a drip anywhere to be seen. Called the dealer….basically because our VIN has not been listed in any recall, we are out of luck on any help with repairs. Incident occurred in our drive, my wife was very lucky. Ford has a serious problem. We are the original owner, 2013 with 105871 miles. Vehicle is very excellent condition. If this would have happened on the highway, serious injury or death to us or others would have been very likely. Taking to the dealer on Monday June 9 for a diagnosis and a serious talk. Hopefully after a confirmation of master cylinder failure and leakage into the brake booster (which Ford knows is an issue), they will be able to repair under an expired recall that should have included our 2013 with the 3.5 eco boost engine. Dealer’s service dept. agreed with me over the phone but added not to get my hopes up because Ford did not list every VIN in the recall that they should have included. Interestingly, back in May, Ford notified us by mail, that as still the original owner there had been issues with the master cylinder and booster. If we had a repair done, they would reimburse up to 700.00 if we could provide proof. We have had no issues, nor any repairs, so we threw the notice away. So, a month after the notification from Ford, here we are. I would think the lawsuits are out there somewhere, I would rather not be a part of any of that….this should get Fords prompt attention from a safety standpoint. So we’ll see what happens.

I have a 2013 F150 XLT. It is displaying the same backup camera issues that the recent recall has described, however it is not included in the recall. When I put the transmission in reverse, the backup camera display is either blank, shows a momentary blue screen, then blank or shows a distorted reversed image with no guidelines. I'm just reporting this information in an effort to expand Ford's recall to all years/models effected by this backup camera fault. Thanks.

I had recall 19N01 and had a local repair shop complete the repair JR's Transmission in Catskill, NY. My local dealer RC Lacy would not cover the recall. I had the repair completed on 11/2024. I would like to be reimbursed for the repair. I was going in for surgery for throat cancer at the time of the warranty.
The faulty output shaft speed sensor (OSS) causes a signal loss between the sensor and the powertrain control module (PCM), leading to a sudden downshift into first gear, regardless of the vehicle's speed. This sudden downshift can cause the rear tires to slide, potentially leading to a loss of control and an increased risk of a crasH. There is already a recall for the 2014 Models and my 2013 is having the exact same issue. This has been an ongoing problem since I bought the truck in December 2024
When putting truck in Reverse... Back-Up Camera goes Blank(Black-Out)
The contact owns a 2013 Ford F-150. The contact received notification of NHTSA Campaign Number: 19V075000 (POWER TRAIN). The contact stated that while driving 47 MPH, there was an abnormal sound coming from the engine. The contact was able to continue driving; however, the transmission shifted roughly and unexpectedly downshifted to first gear. The vehicle was taken to a local dealer where the vehicle was diagnosed and determined that the transmission leadframe was faulty and needed to be replaced. The contact was informed that DTC: P0720 and P0722 were retrieved. The dealer reprogrammed the powertrain control module and informed the contact that the Output Shaft Speed Sensor (OSS) needed to be replaced and was not covered under the recall. The contact referenced Customer Satisfaction Program: 24S37, as a possible cause for the failure. The manufacturer was not notified of the failure. The failure mileage was approximately 278,000.
My 2013 F150 had been acting strangely in showing a wrench on the LED screen and shutting off the electrical components on my dash. when this happens, it also sticks the transmission in between 3-5th gear. I am unable to shift down to 1st or 2nd gear, or up to 6th gear. After this happened a few times, it started to shift me in to Park and stop my wheels from turning. This happened to me 3 times while traveling down the highway at 80mph. All 3 times, it lasted only a few seconds, but it was enough to screech my brand new tires and leave marks on the ground as if I had slammed on my brakes. It returns to normal and continues down the road.
The contact owns a 2013 Ford F-150. The contact stated that while reversing out of a driveway, the brake pedal was depressed and went down to the floorboard. The contact used the parking brake to stop the vehicle. The brake warning light was illuminated. The vehicle was towed to the dealer who diagnosed that the brake master cylinder had leaked brake fluid into the brake booster pump. The contact was informed that the brake master cylinder and booster pump needed to be replaced. The dealer confirmed that the failure was related to NHTSA Campaign Number: 16V345000 (Service Brakes, Hydraulic); however, the VIN was not included. The vehicle was not repaired. The manufacturer was informed of the failure and confirmed that the VIN was not included in the recall. The manufacturer opened a case. The manufacturer referred the contact to the NHTSA Hotline to report the failure. The failure mileage was approximately 160,000.
Despite having my powertrain control module "reprogrammed" in 2020 at a Ford dealership located in Bozeman, Montana, pursuant to the recall issued by Ford associated with sudden gear downshift in F150s (2011-2013), my vehicle is now (May 2025) exhibiting the symptoms associated with the failure of the output speed sensor and molded leadframe, related to this same recall. I have been driving at highway speeds in surrounding traffic when my instrument cluster will suddenly show a wrench and my speedometer drops to zero, so I no longer know how fast I am going (four to five occasions over the last week). I am experiencing erratic and hard automatic shifting at all speeds (could create whiplash injury), and on one occasion my vehicle shifted suddenly from 5th gear down into 1st gear while I was driving close to 50 mph, locking the back tires and potentially causing engine damage. Obviously this could cause me to lose control of my vehicle and crash as the sudden downshift is abrupt and locked the rear wheels. Ford issued a "customer satisfaction program" extended warranty related to failure of this molded leadframe. That warranty would cover repairs within 10 years of service or 150,000 miles, whichever comes first. My truck is relatively low mileage (under 87,000 miles) for a 12 year old vehicle and based on what I have heard from both the Ford dealership in town and from my other mechanic shop, this is an issue they are seeing commonly. In my opinion, this extended coverage program is woefully inadequate. My vehicle poses a risk to me and my passengers' safety and to surrounding drivers and I should not be liable for the cost of these repairs because there is clearly a known and acknowledged problem. Ford has extended recalls to certain other years and models with the same issue, now having their repairs to make their vehicles safe covered while I am expected to pay out of pocket for Ford's bad and unsafe design.
The contact owns a 2013 Ford F-150. The contact stated while driving 65 MPH, the transmission unexpectedly downshifted to first gear. There were no warning lights illuminated. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer referred the contact to the NHTSA Hotline to report the failure. The failure mileage was unknown.
I was pulling an empty car trailer in Tow mode when my truck downshifted from 5th gear to 1st gear causing my trailer to fishtail and almost rear ended from a tractor trailer. Then continuing it shifted rear hard to the next gear. Since this first occurrence, it has happened multiple times, which is a major safety issue. I have to pull over immediately, turn off truck, and turn back on to reset the transmission. Until I figured that out, the issue was even more hazardous almost causing accidents for my family, including my grandchildren. Since then, they cannot ride with me. I found out that this is a known issue and covered to 150k miles for Molded Transmission Leadframe Extended Coverage. My issue is that I bought this truck at a Ford dealership in 2018 with 110k miles and have been in the dealership before and was never told about this safety issue and extended coverage. If it’s a known safety issue, why is it not covered beyond 150k miles or 10 years??????? I’m currently in the dealership waiting for an $180 diagnosis which I don’t feel that I should not have to pay. I’m fairly certain, they will charge MUCH more for their recommended fix, which should have all been covered by Ford for this known safety issue. Thanks for your help.

Traction Control Module failed -- (as diagnosed and confirmed by the dealer) I did a quick stop (someone cut in front of me), and felt the anti-locks 'kick in'. Later that day, the dashboard lit up with three warning lights and two messages stating that the anti-lock and brake systems needed to be serviced. I am not sure what risks are associated with this --- but... its the braking system. The repair cost estimate: Nearly $6,000.00
