i was involved in a collision on US30 3 miles west of Rainer Oregon. where I struck another Ford truck while cruising at 55 mph. The vehicle I struck pulled in front of me, my vehicle was severely damaged in the front end and the steering wheel and lower dash panel injured me.,
The Belt in the tire separated while driving. This caused a "Bubble" to form causing my truck to shake violently and required changing to the spare tire. If this happened on the highway I could have lost control and caused an accident. I took my truck to a tire installer who documented the above finding and noted the tire was properly inflated and there were no impact marks on the tread or sidewalls. I am currently working with the supplier but they are being unreasonable with the requirements to have it inspected. No other reps have been contacted at this time. No ( its a 24 year old truck, I don't have any tire related warning lamps) LT235/85 R16
My fiancé & I were driving down a 35mph road & I told her my steering wheel was a little crooked & I don't remember hitting anything to knock out of alignment. We got to my friends house to move a car trailer for him up his steep driveway. I hooked up to the trailer pulled it out, moved it around to line up to back up driveway & making the last adjustment all of a sudden I was turning my steering wheel but the tires were not turning, jumped out to find the output shaft of the steering box was cracked in half diagonally & the steering arm was laying on the street. When I got under the truck & looked at the part that broke you could see it was fractured for a long time. There was only about an 1/8" on one side that was raw metal from just finally breaking. Thank God we are alive if it would have happened going fast probably would have killed someone.
I was driving on the interstate going highway speed of 65 mph and the accelerator was stuck open and I could not slow down. The engine stayed at a high rpm. I had to press with a lot of force to slow down and come to a stop with the engine still at high rpm and had trouble stopping. Luckily, where I live there's not much traffic, and I made it to a shop with this issue. Upon investigating, the accelerator cable became frayed where it enters the sheath and gets stuck there. The Ford dealer didn't have a replacement in stock or any nationwide. So I went to a junk yard to find one and found the same vehicle that had the same frayed accelerator cable. I located a newer, second vehicle with less mileage with same cable that matched without any fraying. In the meantime, I ordered a new accelerator cable through Ford, but it was backordered several months and tied up in shipping. With this happening on these two same vehicles, I thought I should report it. Thank you.
The contact owns a 1999 Ford F-250. The contact stated that the vehicle stalled while driving at various speeds. The contact stated that the check engine light illuminated for approximately 30 seconds. The contact would turn off and restart the vehicle and the vehicle would return to normal operation. The contact stated he took his vehicle to several independent mechanics and was informed that the camshaft sensor was faulty and needed to be replaced. The vehicle was not repaired. The contact stated that similar vehicles were included in NHTSA Campaign number 07V553000 (Engine and Engine Cooling) however, the VIN was not included in the recall. The manufacturer was notified of the failure. The failure mileage was 300,000.
TL THE CONTACT OWNS A 1999 FORD F-250 EQUIPPED TRAILERED TO 2019 FOREST RIVER EAST TO WEST TRAVEL TRAILER EQUIPPED WITH TBC BRAND LLC TIRES, TIRE LINE: TRAILER KING, TIRE SIZE: ST225/75R15, DOT NUMBER: 1ADB4HFT1). THE CONTACT STATED THAT WHILE DRIVING APPROXIMATELY 60 MPH, THE PASSENGER'S SIDE FRONT TIRE EXPERIENCED A BLOWOUT. THE CONTACT PULLED OVER TO THE SIDE OF THE ROADWAY AND PLACED THE SPARE ONTO THE VEHICLE. THE CONTACT STATED THAT ABOUT 15 MINUTES LATER, APPROXIMATELY 10 MILES LATER, THE FRONT DRIVER'S SIDE TIRE ALSO EXPERIENCED A BLOWOUT. THE TIRES WERE REPLACED WITH NEW TIRES. THE CONTACT CONTACTED CAMPERS INN RV OF MACON - BYRON (125 PEACHTREE PKWY, BYRON, GA 31008, (478) 956-6111) AND WAS INFORMED BY THE DEALER THAT THE TIRES WERE NOT REPAIRABLE. THE MANUFACTURER WAS NOT MADE AWARE OF THE FAILURE. THE APPROXIMATE FAILURE MILEAGES WERE 221,000 ON THE VEHICLE AND 1,000 ON THE TIRES ON THE CAMPER. THE VIN WAS NOT AVAILABLE.

THE CONTACT PULLED THE VEHICLE OVER AND DISCOVERED THAT THE REAR STRAP CONNECTED TO THE FUEL TANK DETACHED FROM THE VEHICLE. THE CONTACT WIRED THE TANK BACK INTO PLACE AND DROVE HOME. THE CONTACT ASSOCIATED THE FAILURE WITH NHTSA CAMPAIGN NUMBER: 11V385000 (FUEL SYSTEM, GASOLINE). THE CONTACT INFORMED THE MANUFACTURER OF THE FAILURE AND WAS INFORMED THAT THE VEHICLE WAS NOT INCLUDED IN THE RECALL; THEREFORE, HE WOULD BE RESPONSIBLE FOR THE REPAIRS. A DEALER WAS NOT NOTIFIED OF THE FAILURE. THE VEHICLE WAS NOT REPAIRED. THE FAILURE MILEAGE WAS UNKNOWN.

THE VEHICLE WAS NOT DIAGNOSED OR REPAIRED. THE FAILURE MILEAGE WAS UNKNOWN.

TL* THE CONTACT OWNS A 1999 FORD F-250. WHEN THE CONTACT EXITED AND RETURNED TO THE VEHICLE, IT MADE AN ABNORMAL SOUND WHEN IT WAS RESTARTED. SUDDENLY, A FIRE ERUPTED UNDER THE HOOD, WHICH SPREAD TO THE CABIN OF THE VEHICLE. THE DRIVER FELL AND HURT HER KNEE WHILE ATTEMPTING TO EXIT THE VEHICLE QUICKLY. MEDICAL TREATMENT WAS REQUIRED. THE FIRE WAS EXTINGUISHED BY THE FIRE DEPARTMENT. THERE WAS EXTENSIVE DAMAGE TO THE FRONT END OF THE VEHICLE. THE INSURANCE COMPANY WAS MADE AWARE OF THE FAILURE. THE MANUFACTURER WAS NOTIFIED. THE DEALER WAS NOT CONTACTED. THE CAUSE OF THE FAILURE WAS NOT DETERMINED. THE VIN WAS NOT AVAILABLE. THE APPROXIMATE FAILURE MILEAGE WAS 300,000.

FILLED THE GAS TANK FULL THE DROVE DOWN THE STREET AT 45 MPH. ABOUT 1 MILE INTO THE DRIVE THE REAR FUEL TANK STRAP BROKE IN HALF AND THE REAR END OF THE FUEL TANK FELL TO THE GROUND SPILLING MOST OF THE FUEL. THE STRAP LOOK CORRODED.
BLOWS SPARK PLUGS OUT & BREAKS COIL PACKS FROM SITTING STILL AT INITIAL TAKE OFF. HAS HAPPENED 4 PREVIOUS TIMES
I WAS GOING DOWN THE FREEWAY @ 65MPH AND I NOTICED MY STEERING WAS NOT RESPONDING IN THE NORMAL WAY. I HAD MADE IT TO A EXIT AND STOPPED AT LIGHT TRIED TO MAKE RIGHT TURN TRUCK CONTINUED STRAIGHT, AND I WAS STOPPED IN RUSH HOUR TRAFFIC IN CA. GOT OUT AND SAW MY SECTOR SHAFT HAD BROKEN OFF MY STEERING GEAR BOX.
I WAS HEADING BACK TO COLLEGE IN BROOKINGS SD, WHEN I WAS STOPPED WAITING FOR A TRAIN TO PASS. AFTER THE TRAIN PASSES I PUT IT FROM PARK INTO DRIVE AND RIGHT WHEN I STARTED MOVING I HEAR A LOUD POP. AFTER I DRIVE ANOTHER BLOCK TO A GAS STATION TO LOOK UNDER THE HOOD I CAME TO THE CONCLUSION THAT I HAD BLOWN A SPARK PLUG. AND IT TOOK OUT THE IGNITION COIL WITH IT AND STRIPED THE THREADS ON THE HEAD. ALONG WITH THAT THERE WAS GAS EVERYWHERE ON THE DRIVERS SIDE OF THE ENGINE BLOCK. THAT'S NOT ONLY SUCKS BECAUSE I HAD TO PAY TO GET IT FIXED, I'M IN COLLEGE I DON'T HAVE MONEY TO PAY FOR THIS. BUT IF THAT GAS WOULD HAVE HIT I SPARK MY TRUCK COULD HAVE BLOWN UP. RUSTED THREW FUEL TANK STRAPS NO CRUISE CONTROL NO AIR CONDITIONING
REAR FUEL TANK STRAP FAILED, CAUSING FUEL TANK TO DRAG ON GROUND WHILE VEHICLE WAS IN MOTION. SEE SAFETY RECALL 11S21 WHICH RECALLED FUEL TANK STRAPS ON OTHER 1999 FORD TRUCKS INCLUDING F-150 AND F-250 LIGHT DUTY BUT NOT F-250 SUPER DUTY. RECALL SHOULD BE EXTENDED TO 1999 F-250 SUPER DUTY.

THIS IS MY 2ND REPLACEMENT OF A THROTTLE POSITION SENSOR. VERY EXPENSIVE REPAIR! $500-600 (BASICALLY A NEW PEDAL ASSEMBLY.) WHAT IS DISTURBINGLY UNSAFE IS THE DRIVER WILL LOSE ALL THROTTLE RESPONSE ANYTIME AND UNKNOWINGLY RANDOM TIMING. COULD BE ON THE FREEWAY (WITH TRAFFIC ON YOUR BUTT! COULD BE AT A LIGHT TURNING LEFT! COULD BE CROSSING A INTERSECTION OR RAILROAD CROSSING THAT YOU WERE STOPPED AT! IT IS VERY UNNERVING TO STEP ON THE PEDAL AND HAVE NO RESPONSE WHEN YOUR VERY OWN SAFETY IS AT STAKE! TL* THE CONTACT OWNS A 1999 FORD F-250. AFTER THE VEHICLE WAS PLACED INTO THE PARK POSITION, IT INDEPENDENTLY ROLLED BACKWARDS WITHOUT WARNING. THE PASSENGER WAS ABLE TO STOP THE VEHICLE. THE CONTACT DISCOVERED THAT THE VEHICLE INADVERTENTLY SWITCHED INTO THE REVERSE POSITION. AS A RESULT, THE CONTACT WAS KNOCKED DOWN AND SUSTAINED INJURIES THAT REQUIRED MEDICAL ATTENTION. A POLICE REPORT WAS FILED. THE VEHICLE WAS NOT DIAGNOSED OR REPAIRED. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE. SPARK PLUG NUMBERS 2 AND 7 BLEW OUT OF THE CYLINDER HEAD. NUMBER 2 BLEW OUT WHILE DRIVING DOWN THE HIGHWAY AND NUMBER 7 BLEW OUT STARTING OFF AT A GREEN LIGHT ON A MAIN STREET. THERE WAS A STRONG ODOR OF GAS WHEN THIS OCCURRED AND SERIOUS VIBRATION FROM THE ENGINE MISFIRING. THIS CREATED A POTENTIAL EXPLOSION HAZARD SINCE THE AIR/FUEL MIXTURE IS DESIGNED TO IGNITE FROM AN IGNITION SOURCE. THIS JEOPARDIZED MY SAFETY AS WELL AS THE SAFETY OF OTHERS AROUND MY VEHICLE. THIS IS A KNOWN PROBLEM TO FORD AND NEEDS TO BE ADDRESSED FOR ALL VEHICLES MANUFACTURED THAT ARE AFFECTED BY THIS PROBLEM. MILEAGE HAS NOTHING TO DO WITH THIS AS OWNERS OF THESE SPECIFIC VEHICLES WITH ALUMINUM HEADS HAVE HAD THIS ISSUE AT LESS THAN 100,000 MILES.

SPARK PLUG BLEW OUT OF HEAD WHILE DRIVING. ENGINE HAS 92000 MILES AND ORIGINAL SPARK PLUGS.

THE VEHICLE WAS TOWED TO A DEALER WHERE IT WAS DIAGNOSED THAT THE BRAKE LINE WAS SEVERELY CORRODED AND NEEDED TO BE REPLACED. THE VEHICLE WAS NOT REPAIRED. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE AND STATED THAT THE VIN WAS NOT INCLUDED IN NHTSA CAMPAIGN NUMBER: 11V385000 (FUEL SYSTEM, GASOLINE). THE FAILURE MILEAGE WAS APPROXIMATELY 180,000. TL* THE CONTACT OWNS A 1999 FORD F-250. WHILE ATTEMPTING TO STOP THE VEHICLE, THE BRAKE PEDAL EXTENDED TO THE FLOORBOARD AND THE VEHICLE FAILED TO STOP. THE CONTACT DISCOVERED BRAKE FLUID WAS LEAKING UNDER THE VEHICLE. THE VEHICLE WAS TAKEN TO THE DEALER WHO DIAGNOSED THAT THE BRAKE LINES WERE CORRODED, CAUSING BRAKE FLUID TO LEAK FROM THE VEHICLE. THE VEHICLE WAS NOT REPAIRED. THE MANUFACTURER WAS NOT NOTIFIED OF THE FAILURE. THE VIN WAS NOT AVAILABLE. THE FAILURE MILEAGE WAS 180,000.
#6 SPARK PLUG BLEW OUT OF THE HEAD. I HAD IT REPAIRED TWO DAYS LATER. TWO DAYS AFTER THAT, THE #3 SPARK PLUG BLEW OUT OF THE HEAD WHILE I WAS DRIVING AT 25MPH. DURING BOTH INCIDENTS GASOLINE FUMES AND EXHAUST FILLED BOTH THE ENGINE COMPARTMENT AND THE PASSENGER COMPARTMENT.
THIS ENGINE WITH ALUMINUM HEADS BLEW OUT SPARK PLUG NUMBERS 2 AND 7. FIRST OCCURRENCE ON AUG 11 2015 AND THE NEXT JAN 14, 2016. THE FIRST INCIDENT OCCURRED DRIVING DOWN A HIGHWAY AND THE NEXT OCCURRED STARTING FROM A STOP LIGHT. THERE WAS A LAG IN POWER AND THEN A LOUD BANGING NOISE AND A STRONG ODOR OF GAS. THIS CREATED AN EXPLOSION HAZARD AS WELL AS PUT MY SAFETY IN JEOPARDY AS WELL AS THAT OF THOSE AROUND ME. SINCE THE AIR/FUEL MIXTURE IS BEING BLOWN OUT OF THE ENGINE ON THE COMPRESSION STROKE AND THE COIL FIRING NORMALLY THIS CREATED A DEADLY SITUATION. THIS PROBLEM IS KNOWN TO FORD AND THE COMPANY IS REFUSING TO DO ANYTHING ABOUT IT FOR ALL VEHICLES WITH THESE PARTICULAR ENGINES THAT ARE HAVING THESE PROBLEMS. THIS SHOULDN'T EVEN HAPPEN WITH AN ENGINE. IT'S A MANUFACTURE/DESIGN FAULT. IT NEEDS TO BE ADDRESSED AND FIXED. THE MILEAGE IS NOT THE ISSUE AS THESE OCCURRENCES ARE HAPPENING ON THE NEWER TRUCKS/SUVS AS WELL WITH WAY FEWER MILES. THAT SHOWS IT'S A MANUFACTURE/DESIGN FLAW AND A VERY SERIOUS ONE.
I WAS DRIVING DOWN INTERSTATE 75 AND SUDDENLY I LOST POWER AND HAD A EXTREMELY LOUD PIPING NOISE COMING FROM UNDER THE HOOD . I STOPPED IMMEDIATELY AND CHECKED UNDER THE HOOD I COULD NOT TELL WITH THE ENGINE OFF SO I RESTARTED IT AND INSPECTED IT AND FOUND THAT MY NUMBER 2 SPARK PLUG HAD BLOWN OUT OF THE CYLINDER HEAD AFTER BUYING A NEW COIL PACK AND SPARK PLUG SET I DISCOVERED THAT ALL THE THREADS HAD BLOWN OUT OF THE HEAD WITH THE SPARK PLUG AND AFTER CHANGING ONE OF THE OTHER SPARK PLUGS I FOUND THAT IT WAS BARELY HOLDING IN .I THINK THAT THE HEADS DESIGN WAS POOR AND WEAK AND IS S SAFETY HAZARD.

THE VEHICLE WAS NOT REPAIRED. THE MANUFACTURER WAS NOTIFIED OF THE FAILURE. THE APPROXIMATE FAILURE MILEAGE WAS 107,000.

THE VEHICLE WAS TAKEN TO AN INDEPENDENT MECHANIC FOR DIAGNOSTIC TESTING AND THE MECHANIC STATED THAT THE FUEL LINE STRAP NEEDED TO BE REPLACED. THE VEHICLE WAS NOT REPAIRED. THE MANUFACTURER WAS NOTIFIED OF THE FAILURE. THE APPROXIMATE FAILURE MILEAGE WAS 83,000.
A MAJOR LOSS OF POWER WHILE TOWING A LOAD. THE LOSS WAS SIGNIFICANT ENOUGH THE FIRST TIME THAT THE TRUCK COULD NOT SHIFT INTO HIGHER GEARS. COULD ONLY MANAGE 55 MPH MAX. WHEN I ARRIVED HOME, TOOK IT TO MY LOCAL DEALER TO HAVE IT REPAIRED. THEY HAD IT FOR 10 DAYS AND WERE UNABLE TO LOCATE ANY PROBLEM. THEY REPROGRAMMED IT, AGAIN, AND GAVE IT BACK WITH A SIGNIFICANT BILL. THE PROBLEM PERSISTS. THE NEXT TIME I USED IT TO HAUL A LOAD, IT DID IT AGAIN, ONLY THIS TIME I WAS ATTEMPTING TO PASS. COULD NOT COMPLETE PASS AND FORTUNATELY WAS ABLE TO GET BACK IN BEHIND THE VEHICLE I WAS TRYING TO PASS AND AVOID A HEAD ON COLLISION. IT'S DANGEROUS! THE PROBLEM IS SUBTLY WORSENING. SINCE THAT LAST EPISODE I DID SOME RESEARCH OF MY OWN, BESIDES CONTACTING FORD CORPORATE DIRECT NUMEROUS TIMES AND THE DEALER, I DISCOVERED THERE IS A HISTORY OF ALL SORTS OF TROUBLE WITH THEIR V 10. I DISCOVERED LAST NIGHT THAT MANY OTHERS HAVE EXPERIENCED WHAT I AM ATTEMPTING TO CORRECT, ALL WITH THE SAME RESULTS, NO REPAIR. ALL FORD WOULD DO IS TO CONTINUE TELLING ME OVER AND OVER TO JUST GO TO MY DEALER AS THEY KNOW ALL. THEY DO NOT! I AM STILL IN CONTACT WITH MY DEALER REGARDING THIS PROBLEM WITH NO OPTIONS AVAILABLE. I AM ABOUT TO REPLACE A FEW PARTS MYSELF IN AN ATTEMPT TO CORRECT IT MYSELF. PER MY RESEARCH, IT BECAME VERY APPARENT THAT FORD'S COMPUTER FOR ENGINE OPERATION AND DIAGNOSIS IS FLAWED AS THERE WERE NEVER ANY NUMBER INDICATIONS REVEALED TO INDICATE THE SOURCE OF THE PROBLEMS, PER NUMEROUS OTHERS AS WELL AS MYSELF. IT IS NOW OBVIOUS THAT FORD IS WELL AWARE OF THIS PROBLEM BUT IS COVERING IT UP INSTEAD OF ADDRESSING IT. THIS PROBLEM ALSO APPEARS TO BE HAPPENING TO VEHICLES MUCH NEWER THAN MINE. THIS VEHICLE IS DANGEROUS WHEN UNDER LOAD. THERE MAY ALREADY HAVE BEEN ACCIDENTS CAUSED BY THIS CHRONIC PROBLEM, I DO NOT KNOW. BUT I DO KNOW THAT THIS VEHICLE IS A DISASTER WAITING TO HAPPEN. *TR

THE VEHICLE WAS TOWED TO AN INDEPENDENT MECHANIC, WHO DIAGNOSED THAT THE FILLER NECK AND THE FAILED BOLT NEEDED TO BE REPLACED. THE VEHICLE WAS NOT REPAIRED. THE MANUFACTURE WAS MADE AWARE OF THE PROBLEM. THE APPROXIMATE FAILURE MILEAGE WAS 250,000
HEARD A LOUD POP FROM UNDER THE HOOD WHILE DRIVING AT HIGHWAY SPEED. SOUNDED LIKE IT WAS RIGHT IN FRONT OF THE GLOBE BOX AREA. THE ENGINE EJECTED A SPARK PLUG WHICH IS A VERY COMMON FAILURE FOR TRITON ENGINES WITH ALUMINUM HEADS - THREADS FAILED THAT HOLD THE SPARK PLUG IN PLACE. I DID NOT HEAR ANY WARNING SOUNDS THAT A FAILURE WAS IMMINENT. THE PASSENGER SIDE OF THE ENGINE, SPARK PLUG CLOSEST TO THE FIREWALL WAS BLOWN OUT WHICH IS ALSO VERY COMMON. I SMELLED GASOLINE AND EXHAUST AND FORTUNATELY THIS FAILURE DID NOT RESULT IN A FIRE. *JS
I FILED A CLAIM WITH MICHELIN NORTH AMERICA, INC (MICHELIN), THAT WAS DENIED, FOR DAMAGES IN EXCESS OF $3,000.00 INCURRED BY MY PERSONAL VEHICLE RESULTING FROM THE COMPLETE TREAD DETACHMENT OF A UNIROYAL LAREDO TIRE (DOT# BFW8FEUU4107), AFTER 13,586 MILES. DETACHMENT OCCURRED AT HIGHWAY SPEED (65MPH) WITH PROPER INFLATION PRESSURE, CHECKED PRIOR TO BEGINNING OF TRAVEL. PRIOR TO COMPLETE TREAD DETACHMENT, THE SUBJECT TIRE WAS REPAIRED FOR DAMAGE INCURRED BY A COMMON ROAD HAZARD (I.E., A NAIL PUNCTURE). THIS, THE TIRE?S ONLY, REPAIR WAS PERFORMED BY A REPUTABLE SERVICE CENTER IN ACCORDANCE WITH GUIDELINES SET BY US DEPARTMENT OF TRANSPORTATION. MICHELIN DOES NOT CALL INTO QUESTION TIRE REPAIR AS THE CAUSAL FACTOR. RATHER, MICHELIN ASSERTS THAT THEIR INSPECTION OF THE TIRE INDICATES TREAD DETACHMENT RESULTED FROM TIRE DAMAGE CAUSED BY THE PUNCTURE. AND, ?EVIDENCE OF THIS IS FOUND WHEN TRACING THE STEEL CORD PATH FROM THE PUNCTURE/REPAIR TO THE INITIAL DETACHED AREA?. MICHELIN FEELS THEY ARE NOT RESPONSIBLE FOR ANY DAMAGES. I CHALLENGE MICHELIN?S DENIAL BASED ON (1) MICHELIN?S ADMISSION THAT THE ?INITIAL DETACHED AREA? OCCURRED AT SOME DISTANCE FROM THE REPAIR/PUNCTURE; (2) MICHELIN HAS A HISTORY OF TIRE RECALLS PERTAINING TO DEFECTS AND POTENTIAL TREAD DETACHMENT OF SIMILAR TIRES; AND (3) THE SUBJECT TIRE WAS MANUFACTURED DURING A PERIOD BOUNDED BY TWO LAREDO TIRE RECALLS, DUE TO THE POTENTIAL FOR TREAD DETACHMENT: ?SEPTEMBER 2004, UNIROYAL VOLUNTARY RECALL OF 46,000 TIRES DUE TO POSSIBLE TREAD/BELT DETACHMENT. ?JULY 2012, NATIONAL HIGHWAY TRAFFIC SAFETY ADMINISTRATION (NHTSA #12T-019) RECALL OF APPROXIMATELY 800,000 UNIROYAL TIRES, DUE TO POSSIBLE TREAD BELT SEPARATION. FURTHER, A MODICUM OF RESEARCH CLEARLY INDICATES THAT INSUFFICIENT TIRE BONDING AND OTHER MANUFACTURER DEFECTS ARE THE MOST FREQUENT CAUSE OF TREAD DETACHMENT. MY TRUCK WAS PARKED AT WORK. WHEN I GOT IN THE TRUCK TO LEAVE I WENT TO START IT AND I HAD HEARD A LOUD BANG FROM UNDER THE HOOD FOLLOWED BY A CLATTERING SOUND. THE FAILURE CAUSED DAMAGE TO THE FRONT PASSENGER SIDE DOOR AND FENDER. THE CONTACT REMOVED THE FAILED TIRE AND INSTALLED THE SPARE. THE VEHICLE FAILURE MILEAGE WAS 137,000. THE TIRE FAILURE MILEAGE WAS UNKNOWN. THE DOT NUMBER WAS UNAVAILABLE.
