CERTIFIED LTR REQUESTING NHTSA INITIATE A DEFECT INVESTIGATION RE SELECTED FORD MOTOR COMPANY VEHICLES LEFT FRONT INNER BEARINGS FOR ADVANCED FATIGUE CRACKS. *TS COMPLAINANT WAS RECENTLY ASKED TO CONSULT IN THE MATTER OF A FATAL HIGH SPEED HEAD ON TWO-VEHICLE COLLISION. WHILE DRIVING WESTBOUND 2001 FORD EXPEDITION VEERED INTO THE ONCOMING LANE AND STRUCK AN EASTBOUND 2000 FORD F250 LONG BED TRUCK TOWING A 5TH WHEEL TRAVEL TRAILER. THE RESULTING COLLISION KILLED THE THREE OCCUPANTS IN THE 2001 EXPEDITION AND THE TWO OCCUPANTS IN THE 2000 FORD F250 PICKUP TRUCK. THE PICK UP TRUCK AND TRAILER WERE COMPLETELY ENGULFED IN FLAMES UPON OFFICER'S ARRIVAL. METALLURGICAL ANALYSIS SHOWED THAT THE LEFT FRONT INNER BEARING FAILED ON THE EXPLORER AS A RESULT OF PROGRESSIVE FATIGUE. THE BEARING FAILURE OCCURRED AT APPROXIMATELY 30,000 MILES. PLEASE REFERENCE ODI# 10139931 TO OBTAIN INFORMATION ON THE 2001 FORD EXPEDITION. *NM
VEHICLE SUFFERED TWO TIRE FAILURES.*MR. THE TIRE TREAD SEPERATED. *TS *JB
WHILE TRAVELING ON THE HIGHWAY AND WITHOUT PRIOR WARNING, THE WINDSHIELD WIPERS STOPPED WHILE IN USE. *AK. THE WIPER MOTOR WAS REPLACED UNDER RECALL PREVIOUSLY, HOWEVER THE SECOND TIME THE COVER AND SWITCH ASSEMBLY WAS REPLACED AT THE CONSUMER EXPENSE. ALSO THE PINION SEAL WAS REPLACED DUE TO A LEAK. WHILE DRIVING THE ABS LIGHT ILLUMINATED ON THE DASHBOARD AND THE BRAKE PEDAL WENT ALL THE WAY DOWN TO THE FLOOR. *NLM
THE SEAT BELT WOULD NOT RETRACT PROPERLY. *NLM
WHILE DRIVING THE VEHICLE STALLED. IT WAS LATER DETERMINED THAT THE SPARK PLUGS WERE DEFECTIVE. *NLM
WHILE DRIVING 60 MPH THE LEFT REAR TREAD SEPARATED FROM THE TIRE. *NLM
WHITE SMOKE ENTERED INSIDE THE VEHICLE, THE CONSUMER SHUT THE VEHICLE OFF AND RESTARTED IT, HOWEVER THE SMOKE APPEARED AGAIN, THE VEHICLE WAS TOWED TO THE DEALER. *JB
AT A COLD START, THE WINDSHIELD WIPERS CAME ON AUTOMATICALLY. *JB
THERE WERE NO BRAKES IF THE VEHICLE WAS TURNED SHORT. *JB
VEHICLE SUFFERED TIRE TREAD SEPARATION.*MR WHILE DRIVING THE TREAD SEPARATED FROM THE TIRE. THE CONSUMER SENT THE TIRE TO THE MANUFACTURER. *PH *NLM
THE VEHICLE BED DID NOT HOLD THE CAB AS DESIGNED, THE STRUCTURE OF THE VEHICLE WAS WEAK.*JB
THE VEHICLE WOULD PULL TO THE LEFT OR RIGHT WHEN THE BRAKES WERE APPLIED, DEALER STATED THEY WERE AWARE OF THE PROBLEM, HOWEVER THERE WAS NO REMEDY . TS *JB
WHILE DRIVING THE VEHICLE HAS A SQUEAKY NOISE THE DEALER STATED THAT BALL JOINTS ARE WORN DOWN. PLEASE PROVIDE ADDITIONAL INFORMATION. TS
CONSUMER STATES THE VEHICLE CONSTANTLY PULLS TO THE LEFT. TOOK TO DEAELR AND DEALER COULD NOT DUPLICATED, AFTER WHICH CONSUMER TOOK VEHCLE BACK OUT ON ROAD. AS A RESULT CONSUMER LOSS CONTROL OF VEHICLE AND LANDED IN A DITCH. PLEASE PROVIDE ANY FURTHER INFORMATION. TS
CONSUMER WAS TOWING A TRAILER AND APPLIED BRAKES THE TRUCK SHUDDERED VIOLENTLY AND THE TRAILER ALMOST "JACK KNIFED". VEHICLE WAS CHECKED BY DEALER WHO DETERMINED THAT THE FRONT ROTORS WERE WARPING EXCESSIVELY DUE TO WATER BEING SPLASHED AND THE HEAT GENERATED WHEN BRAKES ARE APPLIED. FIRST TIME THE ROTORS HAD TO BE TURNED WAS AT 38K MILES. THE SECOND TIME WAS AT 58K MILES. TS
WHILE DRIVING VEHICLE WILL PULL TO THE RIGHT RESULTING IN CONSUMER LOSING CONTROL OF VEHICLE AND VEHICLE RUNNING INTO A DITCH. DEALERSHIP CAN NOT FIX PROBLEM. PLEASE PROVIDE MORE DETAILS. TS
THE CLUTCH DISENGAGED WITH THE TRANSMISSION WHILE DRIVING. TS CONSUMER STATED THE CLUTCH HAD CRACKED.*JB
TIRE BLEW UP WHILE DRIVING 60 MPH NO WARNING NO NAILS OR PUNCTURE INDICATIONS IN TIRE.
IMPROPER HOOD LATCH BOLT NOT EQUIPPED WITH SERRATED LOCKING WASHER WORKED LOOSE AND RELEASED HOOD ON FREEWAY. HOOD RIPPED OFF IT'S RETAINING STRUTS AND SMASHED INTO THE WINDSHIELD. NLM
TIRE MFG., GENERAL; MS LT23585R16 GRABBER AW; THE TIRE DECENTEGRATED WHILE BEING DRIVEN 65 MPH ON A EIGHT LANE SECTION HIGHWAY. PLEASE DESCRIBE DETAILS. NLM
CONSUMER STATES WHILE SHIFTING DOWN FROM 2ND TO 1ST THE GEAR GOES INTO REVERSE. PLEASE PROVIDE ANY ADDITIONAL INFORMATION. TS
HAD JUST COMPLETED DRIVING 280 MILES ON I-10 AND TEMPERATURE WAS APPROX. 100 DEGREES. TIRES WERE AT 80 PSI AND HAD BEEN CHECKED PRIOR TO TRIP. EXITED OFF I-10 ONTO COUNTY ROAD AND THEN ONTO ANOTHER SECONDARY ROAD. HAD JUST ACCELERATED TO 45 MPH WHEN I HEARD LOUD BANGING NOISE AND AND FELT BACK END VIBRATE. PULLED OFF ROAD AND STOPPED WITHIN 100 YARDS. FOUND THAT RIGHT REAR TIRE TREAD HAD COMPLETELY SEPARATED FROM MOUNTED TIRE AND IT WAS NOW WRAPPED AROUND AXLE IN ONE PIECE. THE INTERIOR TIRE WAS DESTROYED AND WHEEL WAS RESTING ON SHREDDED SIDEWALL AND RIM. TIRES ARE 10 PLY, E RATED. INSPECTION OF ENTIRE SEPARATED TREAD REVEALED NO PUNCTURES OR OTHER DAMAGE. VEHICLE OFTEN USED TO PULL HORSE TRAILER. BELIEVE HAD TIRE TREAD SEPARATION OCCURRED AT HIGHWAY SPEEDS, COLLISION/ROLLOVER COULD HAVE RESULTED DUE TO RAPID TIRE DEFLATION ONTO RIM AND TREAD WRAPPING AROUND AXLE. AM STATE POLICE OFFICER AND HAVE INVESTIGATED MULTIPLE INJURY/FATALITY COLLISIONS INVOLVING FIRESTONE WILDERNESS TIRES. TIRE SEPARATION IN THIS INCIDENT APPEARS TO BE SIMILAR.*AK
TREAD SEPARATED AND DAMAGED REAR FENDER. TIRE DID NOT DEFLATE, OTHER TIRES SHOW SIGNS OF RIPPLING IN SIDEWALL AND APPEAR TO BE DEFECTIVE. TIRES STILL HAVE VERY GOOD TREAD DEPTH. DEALERSHIP HAS ROTATED AND MAINTAINED TIRES ON SCHEDULE. THIS TIRE WAS NOT COVERED ON RECENT RECALL. HOW CAN I RECEIVE DUE PROCESS ON THIS MATTER.*AK

FIRESTONE, STEEL TEX TIRE, LT265/75R16SIZE, DOT# VNW81XM040. ORIGINAL EQUIPMENT ON A 2000, FORD, F250. WHILE DRIVING AT HIGHWAY SPEEDS VALVE STEM SEPARATED. FORD AND FIRESTONE WERE NOTIFIED.*AK *JB
BRAKES- DON'T KNOW WHAT PART WOULD BE CONSIDERED DEFECTIVE. HAVE HAD IT IN ATLEAST 9 SEPARATE OCCASIONS TO ANDERSON FORD IN GRAND ISLAND, NE 120 DIERS AVE 68803. THEY HAVE ADJUSTED THE CAMBER ON THE BRAKES, REPLACED PADS, AND HAVE HAD TO ADJUST THE ALIGNMENT. HAVEN'T HAD IT IN SINCE DEC. 27, 2001, ALTHOUGH BRAKES ARE PULLING AGAIN. (ANDERSON FORD SAID THEY WOULD NOT WARRANTY AGAIN). TIRE- THE ENTIRE SIDEWALL HAS BLOWN OUT WITH NO HOLES IN THE TREAD AREA, CAUSED ME TO LOOSE CONTROL AND GO INTO DITCH, HAVE MINOR FINDOR AND PAINT DAMAGE DID NOT REPORT. WORKER AT GARRET TIRE AT 3004 W. OLD POTASH HWY, GRAND ISLAND 68803 SAID THAT THE VALVE STEM HAD A CUT IN IT AND CAUSED THE PRESSURE TO RUN LOW. SAID THE SIDEWALL WAS BASICALLY RUNNING ON THE ASPHALT AND THAT IS WHAT CAUSED THE SIDE OF THE TIRE TO COMPLETELY BLOW OUT. WAS NOT LOW WHEN I STARTED MY TRAVEL. I HAVE PICTURES AND ALSO HAVE THE TIRE AND WITNESSES.*AK
SIDEWALL BUBBLE; TIRE REPLACED BEFORE CATASTROPHIC FAILURE. *AK
THE METAL BRAKE LINE FROM THE MASTER CYLINDER UP TO THE FRONT WHEELS HAS A HOLE, WHICH WAS WORN INTO FROM RUBBING AGAINST THE MOVING STEERING WHEEL SHAFT. CAUSING A LOSS OF BRAKING FLUID. CONSUMER STATES THAT THIS LINE SHOULD HAVE BEEN SECURED TO THE FIREWALL, TO PREVENT SITUATIONS LIKE THIS FROM OCCURING. ALSO THE BRAKE LINE WAS REPLACED BY THE DEALER UNDER WARRANTY. *AK *SCC
DRIVING DOWN A TWO LANE HIGHWAY, WHEN WE FELT A BAD VIBRATION IN THE TRUCK, WAS SLOWING DOWN WHEN THE TRUCK MADE A LOUD NOISE AND VEERED SHARPLY TO THE ONCOMING TRAFFICS LANE. WE GOT THE TRUCK UNDER CONTROL AND PULLED OFF AT THE FIRST AVAILABLE PLACE.THE TIRE DID NOT LOSE ANY AIR BUT IT DID LOSE ALL OF ITS RUBBER LIKE IT WAS A RETREAD TIRE. WE WERE VERY LUCKY THAT WE WERE NOT TOWING HORSES AT THE TIME AND THAT THERE WAS NO ONCOMING TRAFFIC RIGHT AT THAT MOMENT.*AK
I HAD A FLAT SO I HAD ALL TIRES CHANGED AND TECNICIAN FOUND A LARGE BUBBLE ON THE INSIDE OF LEFT FRONT TIRE WHICH CAUSED SIDEWALL TO BE VERY WEAK. FIRESTONE REFUSED TO HONOR WARRANTY STATING IT WAS CAUSED BY ROAD HAZARD. NOT TRUE. TIRE DEFECT. WOULD HAVE CAUSED MAJOR CRASH IF LEFT ON FRONT AND FAILED AT HIGHWAY SPEED. MR

ORIGINAL TIRES ON A FORD F250 2000, R4X2 L265/75R16 DOTVNW81XM349. TOWING AND TRAILING AT 65 MPH, THE LEFT REAR TIRE TREAD HAD COME COMPLETELY APART. THE CONSUMER KEPT CONTROL AND HAD COME TO A COMPLETE STOP. *AK THE CONSUMER STATES THAT THEY DO NOT AGREE W/FIRESTONE IN REFERENCE TO THE 8 PLY TIRES AND IF A FORCE GREAT ENOUGH TO BREAK THE STEEL BELTS, THEY FEEL THAT THE 2 PLY SIDEWALLS WOULD HAVE ALSO BEEN DAMAGED. *SCC
WHILE DRIVING AT 70 MPH, VEHICLE STALLED. THE DEALERSHIP INDICATED THAT THE CAMP POSITION SENOR HAD GONE BAD WHICH CAUSED THE PROBLEM. THE DEALERSHIP REPLACED THE PART, BUT THE PROBLEM RECURRED. TS
VEHICLE WAS GOING 10MPH WHEN HYDRO BOOSTER LINE BLEWOUT. *AK
I WAS DRIVING HOME ON THE INTERSTATE WHEN TRAFFIC STOPPED AHEAD OF ME. IT WAS RAINING PRETTY HARD AND I WAS NOT ABLE TO STOP AND I HIT A MINIVAN THAT WAS IN FRONT OF ME. (THERE WERE 40 - 50 OTHER VEHICLES INVOLVED IN THIS INCIDENT, BUT MY PIECE OF THE GREATER WRECK ONLY INVOLVED MYSELF AND THE MINIVAN.) I DO NOT KNOW HOW FAST I WAS GOING WHEN I MADE CONTACT, BUT IT WAS FAST ENOUGH TO CAUSE $16,000 OF DAMAGE TO THE FRONT END OF MY TRUCK. THE IMPACT MOSTLY WENT ABOVE THE BUMPER, PUSHING THE GRILL INTO THE FRONT OF THE ENGINE CAUSING EVERYHING IN FRONT OF THE ENGINE TO BE REPLACED ALONG WITH REPLACEMENT OF THE MOTOR AND TRANSMISSION MOUNTS. NEITHER THE DRIVER OR PASSANGER AIRBAG DEPLOYED. I WAS WEARING MY SEATBELT, SO I WAS NOT AT ALL INJURED, BUT I WAS VERY SURPRISED THAT A SIGNIFICANT INPACT LIKE I EXPERIENCED DID NOT SET OFF THE AIRBAG. ARE THERE ANY GENERAL GUIDELINES FOR WHEN THE AIRBAGS SHOULD DEPLOY?*AK
WHILE DRIVING AT HIGHWAY SPEEDS, THE FIRESTONE STEELTEX AT TIRE TREAD COMPLETELY SEPARATED FROM RIGHT REAR TIRE, TIRE DID NOT DEFLATE. THIS CAUSED FENDER TO CURL UP AND MUDFLAPS TO BE TORN OUT. DEALER HAS NOT SEEN VEHICLE. *AK *YD
WHILE VEHICLE IS IN IDLE AND BRAKES ARE APPLIED STEERING WHEEL WILL LOCK UP. CONTACTED DEALER, AND DEALER WAS NOT WILLING TO DO ANYTHING.*AK
TURNING LOCKS WHEN BRAKES APPLIED AT LOW SPEEDS. NEARLY IMPOSSIBLE TO PARK THE VEHICLE BECAUSE STEERING IS IMPEDED. PS PUMP REPLACED TWICE WITHOUT ANY CORRECTION. BELIEVE PROBLEM LIES IN THE FACT THAT THE PS PUMP SERVES AS A BRAKE ASSIST PUMP AND IS NOT PUMPING SUFFICIENTLY TO OPERATE BOTH THE BRAKES AND THE STEERING. CONDITION APPEARS TO BE RAMPANT AS NUMEROUS COMPLAINTS ARE NOTED ON NHTSA'S WEB PAGE. SHOULD BE INVESTIGATED, POSSIBLE MFG. DEFECT.
WHILE TRAVELING ON EXPRESSWAY CONSUMER APPLIED BRAKES AND PEDAL WENT ALL THE WAY TO THE FLOOR, ALSO THERE WAS SMOKE COMING FROM PASSENGERS SIDE WHEEL. DEALERSHIP IS AWARE OF PROBLEM. *AK *YH
WHILE PARKED WITH KEY ON IN ACCESSORY POSITION WINDSHIELD WIPERS CAME ON BY THEMSELVES. VEHICLE STARTED SMOKING BETWEEN DASH AND UNDERNEATH HOOD, AND COULD HEAR A SIZZLING NOISE. SMELLED RUBBER BURNING. COULD NOT TURN WIPERS OFF UNTIL PULLING FUSE FROM FUSE BLOCK. *AK
VEHICLE WAS AT DEALERSHIP FOR WARRANTY WORK. WHILE PARKED DRIVER'S POWER SEAT SHORTED OUT AND CAUGHT FIRE, CAUSING $ 17000.00 DAMAGE.*AK
WHILE PROCEEDING THROUGH AN INTERSECTION, MAKING A LEFT HAND TURN, WITH ONCOMING TRAFFIC ALSO MAKING A LEFT HAND TURN, HALFWAY THROUGH THE INTERSECTION MY TRUCK STOPPED TURNING. LUCKILY, I AM YOUNG AND IN GOOD SHAPE, I COULD "MUSCLE IT" THROUGH THE REST OF THE TURN TO AVOID THE ONCOMING TRAFFIC. IF IT WEREN'T FOR MY QUICK THINKING AND "LUCKY" CONDITIONS, THIS COULD HAVE EASILY RESULTED IN A FATALITY. NO SAFETY DEFEC TLISTED. *AK
WHILE DRIVING, THE CABLE THAT HOLDS SPARE TIRE BROKE AND TIRE FELL OFF, CONSUMER CONTACTED DEALER, AND THEY TOLD CONSUMER TO CONTACT MANUFACTURER. *AK *SLC
WHILE TRAVELING AT 50-55 MPH AND AS A RESULT OF A COLLISION HEAD-ON WITH TWO WHEEL TRAILER AIR BAGS DID NOT DEPLOY. *AK A VEHICLE THAT WAS BEING TOWED, CAME OFF THE TRAILER AND HIT CONSUMERS VEHICLE HEAD ON, CONSUMERS WIFE HIT THE STEERING WHEEL WITH HER FACE. *JB
I NOTICE CRACKING ON SIDE OF TIRES WHERE THE SIDEWALL AND TREAD MEET. HAD TIRES CERTIFIED SAFE TO OPERATE BY CERTIFIED FIRESTONE DEALER PER FIRESTONE. AT HIGHWAY SPEED OF 70 MPH LEFT FRONT TIRE TREAD SEPERATED WITHOUT WARNING. I WAS ABLE TO CONTROL VEHICLE AND COME TO COMPLETE STOP WITHOUT FURTHER INCIDENT. TIRE WAS REPLACED BY FIRESTONE DEALER SAME DAY, I WAS CHARGED FOR THE MOUNTING AND BALANCING. ESTIMATED DAMAGE TO FENDERWELL OF VEHICLE TO INCLUDE BENT METAL AND TORN MUD FLAP ESTIMATED DAMAGE 250.00. FIRESTONE DENIED REPLACEMENT OF OTHER TIRES. I HAVE SINCE REPLACED THEM WITH ANOTHER BRAND.*AK (TIRESIZE: 245/75/16)

MY FORD TRUCK WAS IMPACTED AT 55 MILES AN HOUR HEAD-ON. I KNOW OF AT LEAST FOUR OTHER PEOPLE PERSONALLY, WHOSE FORD AIRBAGS DID NOT BEEN EMPLOYED ON THEIR TRUCKS DURING COLLISION. YET I SEE NO REPORTS ON THE NTSA AS FAR AS AS COMPLAINTS GO. I KNOW THAT THERE'S A PROBLEM AND I HAVE A FEELING THAT YOU DO TOO WAND IT'S NOT BEEN REPORTED. I INTEND ON CONTACTING MY CONGRESSMAN ABOUT THE NHTSA'S LACK OF RECOGNITION IN THIS PROBLEM. MY CAREER WAS ENDED BY A 55 MILES AN HOUR HEAD-ON IMPACT IN MY FORD TRUCK. HAVE SEEN NO SUCH REPORT OR COMPLAINTS ABOUT AIRBAGS ON YOUR WEB SITE.*AK
CONSUMERS VEHICLE WAS INVOLVED IN A HEAD ON COLLISION AT 55 MPH, HOWEVER THE AIR BAG DID NOT DEPLOY, AS A RESULT THE CONSUMER SUSTAINED A BROKEN NECK AND CLOSED HEAD INJURIES. NLM
TRUCK SHAKES AND BUCKS OUT OF CONTROL WHEN MAKING UNNORMAL STOPS. THE PROBLEM WAS THAT THE Z-RELAYS WHERE NOT INSTALLED. ANY ONE OF THESE TRUCK ON THE ROAD PULLING A TRAILER WITHOUT THIS RELAY STANDS A CHANGE OF LOSING CONTROL.
