THIS IS A PETITION FOR AN INVESTIGATION INTO LOW-SPEED SURGING IN THE 2006-2010 TOYOTA COROLLA WITH ETCS-I, IN WHICH THE BRAKES FAIL TO STOP THE VEHICLE IN TIME TO PREVENT A CRASH. THIS REQUEST IS BASED ON FIRST-HAND EXPERIENCE IN WHICH MULTIPLE LOW-SPEED SURGE EVENTS THAT OCCURRED WHILE DRIVING OUR 2010 COROLLA. THE LATEST INCIDENT RESULTED IN A CRASH ON JUNE 8, 2014. IN ADDITION TO THE EVIDENCE FROM OUR CRASH INCIDENT, WE ARE PROVIDING EVIDENCE THAT MANY OTHER COROLLA OWNERS ARE EXPERIENCING SIMILARLY UNSAFE SCENARIOS THAT ARE LEADING TO CRASHES. MY WIFE USUALLY DRIVES THE CAR AND HAS COMPLAINED SEVERAL TIMES ABOUT THE RIDE AND STEERING OF THE CAR. ON THIS DAY I WAS DRIVING ON A FREEWAY AT 60 MPH WHEN I NOTICED THAT CAR DOES FEEL LOOSE AND CAN BECOME HARD TO CONTROL. I AM A SENIOR MECHANICAL ENGINEER AND WORK ON LOADS AND DYNAMICS IN AEROSPACE SYSTEMS. THE CAR FEELS LIKE IT HAS A POOR STEERING SYSTEM, AND BOUNCES BACK AND FORTH IN A DEAD BAND AND BECOMES HARD TO CONTROL. THIS COULD BE AN EXPLANATION WHY THE FRONT TIRES WEAR AT AN EXCESSIVELY HIGH RATE. *TR

2010 TOYOTA COROLLA. CONSUMER WRITES IN REGARDS TO STEERING AND OTHER VEHICLE PROBLEMS. *SMD THE CONSUMER STATED THE SKID LIGHT WOULD COME ON AT ANY TIME, EVEN IF SHE WAS DRIVING SLOWLY. WHEN GOING AROUND A CORNER, CONTROLLING THE STEERING WAS SOMEWHAT OF A PROBLEM. IF IT WERE SNOWING, OR RAINING, THE REAR END WOULD SLIDE OUT TO THE POINT, THE SKID LIGHT WOULD COME ON. THERE WERE TIMES, WHEN THE VEHICLE WOULD NOT START. *JB THE CONSUMER ALSO STATED SHE HEARD A CLICKING SOUND FROM THE VEHICLE, THEN ALL OF A SUDDEN, THE VEHICLE TOOK OFF. THE CONTACT SUFFERED INJURIES TO THE BACK. A POLICE REPORT WAS FILED. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE. THE VEHICLE WAS TAKEN TO THE DEALER FOR FURTHER INSPECTION. THE FAILURE AND CURRENT MILEAGE WAS 58,948.
TRUNK FOR 2010 COROLLA DID NOT STAY OPEN WHEN WIFE OPENED TRUNK TO ACCESS ITEMS. LID EDGE HIT BRIDGE OF HER NOSE, CAUSING A SMALL CUT, AND SWELLING. DEVELOPED TO "RACCOON EYES" THE SAME EVENING THAT LASTED A FEW DAYS. CAR WAS PARKED ON SLIGHT INCLINE. EVEN ON LEVEL SURFACES, LID CLOSES UNEXPECTEDLY. *TR

WHILE DRIVING ON ROUTE 30 EAST IN PA, I EXPERIENCED A SCENARIO WHERE I WAS ON CRUISE CONTROL AND PRESSED THE BRAKE. THE CAR CONTINUED TO ACCELERATE AT IT'S SPEED AND SUBSEQUENT PRESSINGS OF THE BRAKES DID NOT SLOW THE CAR. I SHIFTED THE CAR INTO NEUTRAL AND THE BRAKES STILL WERE NOT SLOWING THE CAR ENOUGH. I HAD TO DRIVE INTO THE LEFT SHOULDER AND GRASS BEYOND THAT IN ORDER TO SLOW THE CAR DOWN ENOUGH TO LET OTHER CARS PASS SO I COULD GO INTO THE RIGHT LANE. THE NEXT BUSINESS I TOOK MY CAR TO THE DEALER AND AFTER DRIVING 7 MILES AND LOOKING FOR ERROR CODES, REPORTED THAT THEY "COULD NOT REPRODUCE THE ISSUE". MY CAR ALREADY HAD ALL THE FIXES FOR THE UNINTENDED ACCELERATION ISSUE THAT TOYOTA HAD A NUMBER OF RECALLS FOR DONE IN DECEMBER 2013. THIS ISSUE NEEDS TO BE FURTHER INVESTIGATED BY TOYOTA BECAUSE IF THE REPORTED FIXES DO NOT WORK, THEN THERE COULD BE A SIGNIFICANT NUMBER OF ACCIDENTS/FATALITIES YET TO COME. IN CALL TODAY TO TOYOTA, THEY INDICATED PARTS, WHICH ORIGINALLY WERE EXPECTED TO BE AVAILABLE BY MID OR LATE SUMMER, ARE NOT AVAILABLE, AND THEY DO NOT KNOW WHEN PARTS WILL BE AVAILABLE. IT SEEMS TO ME THAT 5 1/2 MONTHS SINCE RECALL IS TOO LONG TO CORRECT THIS SAFETY CONDITION. *TR

I WAS DRIVING AND THE PERSON IN FRONT OF ME HAD HIT THEIR BRAKES DUE TO THE CARS IN FRONT OF THEM HITTING THE BRAKES DUE TO A RED LIGHT. I THEN HIT MY BRAKES AS WELL AND THE CAR DIDN'T STOP, I PUMPED THE BRAKES A FEW TIMES AND STILL HAD NO BRAKES. MY HUSBAND HAD PANICKED AND PULLED THE EMERGENCY BRAKE AND THE CAR STILL DID NOT STOP. MY CAR THEN HIT THE VEHICLE IN FRONT OF US WHICH IN TURN CAUSE A CHAIN REACTION FOR THE OTHER 3 VEHICLES INVOLVED. IF MY BRAKES WOULD NOT HAVE FAILED, I WOULD HAVE BEEN ABLE TO STOP IN TIME BEFORE SMASHING INTO THE BACK OF THE VEHICLE IN FRONT OF ME. MY CAR IS NOW A "TOTAL LOSS" AND I AM OUT OF A VEHICLE. THE CAR SUDDENLY STOPPED AND SOON CAUGHT FIRE BY ITSELF WHEN I WAS DRIVING ON 101 & LAWRENCE EXPRESS, CALIFORNIA. REPORT SHOWN BY INSURANCE COMPANY SAID "FOUND CYL #1 ROD IS BENT AND HAS PUNCTURED THE ENG. BLOCK CAUSING OIL TO LEAK ALSO THE BLOCK IMPACTED THE AC COMPRESSOR POSSIBLY CAUSING THE VEH TO CATCH FIRE MELTING THE AC COMPRESSOR HARNESS,RF WHEEL SPEED HARNESS,FRONT BUMPER,WHEEL AND WHEEL LINER.NO OTHER FAILURES SEEN OR DEMO" THIS HAPPENED A WEEK AFTER I HAD MY CAR INSPECTED AND MAINTAINED. TRIED TO REACH TOYOTA, BUT THEY ARE NOT RESPONDING. *JS
AT HIGHWAY SPEEDS (55-70 MPH) VEHICLE CAN'T BE DRIVEN ON A STRAIGHT LINE. AS SOON AS YOU THINK IT IS GOING STRAIGHT, IT VEERS EITHER TO THE RIGHT OR LEFT FORCING CONSTANT CORRECTIONS OF THE STEERING WHEEL. SOMETIMES YOU FIND YOURSELF MICRO CORRECTING IT FROM SIDE TO SIDE TRYING TO FIND THE CENTER. I CAN CATEGORIZE THIS ISSUE AS VERY DISTRACTING AS TOO MUCH ATTENTION IS BEING DIVERTED ONLY TO KEEP THIS CAR INSIDE SAFE DISTANCES FROM OTHER CARS. *TR

I STOPPED AT A RED LIGHT. SEVERAL ENGINE LIGHTS STARTED BLINKING. WHEN THE LIGHT TURNED GREEN, MY CAR WOULD ONLY MOVE VERY SLOWLY. I WAS LUCKY ENOUGH TO GET THROUGH THE LIGHT WITH ONCOMING TRAFFIC AND THEN COAST DOWNHILL TO A RESIDENTIAL AREA. I HAD JUST GOTTEN OFF OF A 4-LANE DIVIDED HIGHWAY. I HAD TO HAVE THE CAR TOWED TO MY TOYOTA DEALERSHIP. THEY TOLD ME THAT MY ENGINE CONTROL MODULE HAD SHORT-CIRCUITED AND REPLACED IT IN THE NEXT TWO DAYS, THE PART # BEING 8966102F54. THE CAR IS RUNNING FINE NOW. I'M SCARED THOUGH, THAT THIS MIGHT HAPPEN AGAIN, OR THAT IT COULD HAVE HAPPENED AT HIGH SPEED, OR THAT MY TEENAGE DAUGHTERS WOULD HAVE PANICKED IN THE SITUATION AND MAY HAVE BEEN HIT WHILE GOING THROUGH THE GREEN LIGHT. I WAS DRIVING IN THE MORNING ON A VERY COLD DAY AND WAS DRIVING AROUND 30MPH. I APPLIED THE BRAKES AS I COULD SEE THE STOP LIGHT ABOUT 200 FEET JUST CAME ON. WHEN I APPLIED THE BRAKES, THE ABS BRAKES LOCKED UP AND IT TOOK THE ENTIRE 200 FEET TO FINALLY GET THE CAR TO STOP. *TR

AT SPEEDS OVER 45 THE VEHICLE WANDERS IN LANES AND GIVES A "OUT OF CONTROL" FEELING ON THE FREEWAY. MORE NOTICEABLE ON CONCRETE ROADWAYS. THIS HAS BEEN AN EVER INCREASING PROBLEM SINCE VEHICLE WAS PURCHASED USED FROM DEALER RIVER VIEW AUTO SALES IN LAKE HAVASU ARIZONA ON 09-19-2013 WITH AN ESTIMATED 75000 MILES (ORIGINAL ODO READING NOT DISCLOSED ON DEALER DOCUMENTS). *TR
AFTER DEALER DID SMARTSTOP TECHNOLOGY UP DATE, OVER A PERIOD OF A FEW MONTHS THE ENGINE PERFORMANCE HAS DROPPED OFF, AS HAS THE GAS MILEAGE BY ABOUT 5MPG WITH NO CHANGE IN DRIVING HABITS. TALKED TO DEALER ON 12/11/2014 AND WAS INFORMED THE CAR HAS TO RELEARN THESE FUNCTIONS. ASKED HOW LONG THIS WOULD TAKE AND WAS TOLD A FEW MONTHS WHICH I FIND UNACCEPTABLE. MILEAGE ON CAR NOW IS ABOUT 28000, TO ME THAT IS MORE THAN ENOUGH TIME TO RELEARN ALL FUNCTIONS. *TR
I WAS DRIVING MY 2010 TOYOTA COROLLA UPHILL AND IT FELT AS IF I SLID/SLIPPED A LITTLE ON THE ROAD, WHICH WAS NOT SLIPPERY. THE VSC OFF LIGHT AND CHECK ENGINE LIGHT BOTH STARTED FLASHING ON AND OFF...AND IT SEEMED AS IF I LOST POWER. FINALLY REGAINED POWER TO A NORMAL SPEED BUT LIGHTS STILL FLASHING AND POWER LOST AGAIN. THIS HAPPENED FOR THE FIRST TIME ON SUNDAY NIGHT. THEN DIDNT OCCUR AGAIN UNTIL WEDNESDAY AFTERNOON. SAME THING DRIVING ALONG NORMALLY AND IT FELT AS IF MY VEHICLE WAS GOING TO SHUTDOWN PLEASE HELP. *TR

THE MANUFACTURER WAS NOTIFIED OF THE FAILURE AND STATED THAT THE VEHICLE WAS NOT INCLUDED IN NHTSA CAMPAIGN NUMBER: 09V338000 (SERVICE BRAKES). THE APPROXIMATE FAILURE MILEAGE WAS 50,000.

I [XXX] THE OWNER OF TOYOTA COROLLA 2010 (LICENSE PLATE: [XXX], LICENSED WITH THE STATE OF MARYLAND VIN NO: [XXX]), RESIDING AT [XXX], WHILE TRYING TO PARK IN MY RESIDENTIAL APARTMENTS AT THE ABOVE MENTIONED ADDRESS (AT A SPEED OF 3-5 MPH), APPLIED THE BRAKES AND ALL OF A SUDDEN IT CAUSED AN UNINTENDED ACCELERATION AND I HIT THE HUGE METALLIC TRASH BINS IN FRONT OF ME AT A VERY HIGH SPEED ON OCTOBER 28TH, 2013. THE AIRBAGS DID NOT DEPLOY, BUT THE FRONT OF MY CAR GOT DAMAGED HEAVILY. IT HAS BEEN TOWED AWAY BY MY INSURANCE COMPANY GEICO TO ONE OF THEIR BODY SHOPS. THE INITIAL ESTIMATES AS PER MY INSURANCE AGENT ARE SEVERAL GRAND (DOLLARS) AND THE DAMAGE TO THE CAR IS EXTENSIVE. THE INSURANCE COMPANY'S BODY SHOP IS WORKING ON IT. TWO MONTHS AGO IN WET COLD HIGH ALTITUDE THERE WAS EXTREME SCRUBBING OF FRONT WHEEL BRAKES, AGAIN WAS INSPECTED AND TOLD THAT BRAKES WERE GOOD AND WOULD PROBABLY NEED NEW PADS AT NEXT INSPECTION. MY TOYOTA COROLLA 2010 CAR HAS MANUAL TRANSMISSION. WHEN I DROVE AT ABOUT 35MPH ON CITY STREET AND HIT BRAKE PEDAL, ENGINE SUDDENLY ROARED TO 8000RPM. I DEPRESSED THE CLUTCH FULLY, TURNED OFF ENGINE, HIT HARD THE BRAKE PEDAL,TURNED LEFT TO MEDIAN; THE CAR HIT HARD THE MEDIAN CURB AND STOPPED PARTLY IN LEFT LANE. AFTER SEVERAL ATTEMPTS ON PEDALS AND TURNED ON ENGINE, IT STILL ROARED IN THE FIRST SEVERAL TRIES. THE CAR EVENTUALLY WORKED NORMALLY AFTER 10 MINUTES AND NO INJURY HAPPENED. I NEVER LET THE ENGINE RUN OVER 5000RPM EVEN WHEN I TESTED THE CAR AND ENGINE; MY MAXIMAL RUNNING RPM IS ABOUT 3000. THIS 8000RPM SHOULD NOT BE FROM MY OPERATION. I WAS NOT CONVINCED ON 'STICK PEDAL" OR "FLOOR MAT" CAUSE FOR THIS CASE. THE SEQUENCE "TURNED OFF ENGINE, BRAKED" MIGHT BE GOOD TO "BRAKED TO STOP AND TURNED OFF ENGINE" BECAUSE IT IS TOO HARD TO BRAKE WITH ENGINE OFF. ON MONDAY JULY 29,2013 WHILE I WAS SET ON 70 MPH CRUISE CONTROL, MY TOYOTA COROLLA SUDDENLY SURGED FORWARD. I HAVE NO IDEA HOW FAST IT MADE THE CAR GO ,BUT I HAVE NEVER BEEN IN ANYTHING GOING THAT FAST. I HAD TO TAKE THE CAR INTO THE GRASS MEDIAN TO AVOID HITTING ANOTHER CAR WHEN THE BRAKES WOULD NOT STOP MY CAR. WE STILL FLEW DOWN THE GRASS MEDIAN LIKE A SPEED BOAT HITTING WAVES, WE WERE BOUNCING, THE CAR WAS BOUNCING, I HAD SEVERE CRAMPS AND STABBING ACROSS MY LOWER KIDNEY AREA WHEN WE FINALLY CAME TO A STOP. I REPORTED THE INCIDENT IN GREAT DETAIL TO TOYOTA CORPORATE AND MY CASE WAS #1307291802.(THIS CASE # IS NO LONGER VALID) I WAS TOLD AT THAT TIME IF I FELT SAFE, I COULD TAKE IT TO MY DEALER IN D.M.,IOWA WHERE I LEASED THE CAR IN 2010, OR IF I FELT UNSAFE, I COULD TAKE IT TO ADAMS TOYOTA IN LEE'S SUMMIT, MISSOURI. THEN, MY DAUGHTER-IN-LAW CALLED ADAMS ON MY BEHALF AND THEY GAVE HER AN ENTIRELY NEW CASE NUMBER BECAUSE THE FIRST REP WHO TOOK THE REPORT DID NOT UNDERSTAND THERE WAS DAMAGE .MY NEW CASE # IS 1308011967. I FELT I NEVER WANT TO BE IN THAT COROLLA AGAIN, SO I DID TAKE IT TO ADAMS AND THEY PROVIDED ME WITH A SCION RENTAL TO GET BACK TO HOME AND WORK IN IOWA. TOYOTA HAS INSPECTED THE CAR AND HAS UP TO 30 DAYS TO GIVE ME THEIR REPORT. I AM TO RECEIVE A COPY OF THE DATA IMAGING EXTRACTED FROM THE BLACK BOX ALSO.
