THIS IS IN REGARD TO RECALL #16V596 FOR THE REAR SUSPENSION ARM ASSEMBLIES. THE FIX THAT TOYOTA WANTS TO IMPLEMENT INVOLVES ENCASING THE ADJUSTMENT SLEEVES IN EPOXY RESULTING IN THE ARMS NO LONGER BEING ADJUSTABLE. THESE SUSPENSION ARMS ARE ADJUSTABLE SO THAT THE REAR TOE-IN CAN BE ADJUSTED WHEN AN ALIGNMENT IS PERFORMED. THE FIX TOYOTA WANTS TO DO IS LIKE WELDING THE DRIVERS DOOR SHUT BECAUSE THE LATCH BROKE: IT'S NO LONGER FUNCTIONAL.
TL*THE CONTACT OWNS A 2010 TOYOTA RAV-4. THE CONTACT STATED THAT AFTER PURCHASING A VEHICLE THAT CAME EQUIPPED WITH A TOW PACKAGE, HE DECIDED TO HAVE THE TRAILER HITCH ATTACHED AT THE DEALER. PRIOR TO HAVING THE ELECTRICAL HARNESS AND SWITCH MODULE INSTALLED, THE CORPORATE OFFICE ADVISED HIS DEALER NOT TO PERFORM THE WORK DUE TO A KNOWN DEFECT THAT THEY HAVE DISCOVERED. THE DEALER CONTINUED TO DO THE WORK. THE TYPE OF DEFECT HAD NOT BEEN DISCLOSED AND THE CONTACT HAD NOT EXPERIENCED ANY FAILURES. THE CURRENT MILEAGE ON THE VEHICLE WAS 13,000.
ON DECEMBER 24, 2010 MY FATHER EXPERIENCED A THIRD INCIDENT OF UNINTENDED ACCELERATION IN HIS 2010 TOYOTA RAV. DESPITE HAVING ALREADY PERFORMED ALL THE NECESSARY RECALL FIXES, EXTENSIVE TESTING BY 2 TOYOTA DEALERSHIPS AND EVEN HAVING THE ENTIRE GAS PEDAL REPLACED, THE CAR MALFUNCTIONED. THE ENGINE BEGAN TO REV AT A HIGH RATE AS HE SLOWED FOR A 4-WAY STOP. HIS FOOT WAS FIRMLY ON THE BRAKE, NOT THE GAS PEDAL AS TOYOTA LIKES TO CONTEND. THE BRAKE WAS NOT FULLY RESPONSIVE AND HE HAD TO LIFT HIS FOOT FROM THE BRAKE AND DEPRESS IT A SECOND TIME TO BRING THE CAR TO A STOP, THOUGH THE ENGINE CONTINUED TO RACE. HIS FOOT WAS NOT TOUCHING THE ACCELERATOR. HE WAS FORTUNATE TO BE DRIVING AT A SLOW ENOUGH SPEED AND TRAFFIC WAS LIGHT AT THE TIME SO HE WAS ABLE TO AVOID AN ACCIDENT. HE CONTACTED THE DEALERSHIP AND THEY HAVE TESTED THE CAR (AGAIN) AND STILL CAN NOT FIND ANY PROBLEM (AGAIN). NOW THEY SAID THEY NEED TO KEEP THE CAR FOR 4 MORE WEEKS TILL A REP ARRIVES IN THE AREA FOR MORE TESTING. *TR
2010 RAV 4 ACCELERATOR STUCK GOING INTO REVERSE WITH THE RIGHT FOOT SECURELY ON THE BRAKE PEDAL. PLACED CAR IN PARK AND THE CAR HIT RED LINE 7000RPM. LOOKING AT THE ACCELERATOR PEDAL, THERE IS NOTHING TOUCHING THE PEDAL. THE ONLY WAY TO STOP THE ACCELERATION PROBLEM IS TO SHUT ENGINE OFF. STUCK ECM COMPUTER. *TR
TL*THE CONTACT OWNS A 2010 TOYOTA RAV 4. WHILE DRIVING APPROXIMATELY 20 MPH THE GASOLINE TANK STARTED LEAKING AND THE GASOLINE ODOR WAS IN THE VEHICLE. THE VEHICLE WAS TAKEN TO THE DEALER WHERE THE TECHNICIANS STATED THAT THERE WAS A HOLE IN THE FUEL TANK WHICH NEEDED TO BE REPAIRED. THE TECHNICIANS STATED THAT THE VEHICLE HIT DEBRIS WHICH COULD HAVE DAMAGED THE VEHICLE. THE CONTACT STATED THAT THERE WAS NO DEBRIS ON THE ROAD. THE VEHICLE WAS NOT REPAIRED. THE APPROXIMATE FAILURE MILEAGE WAS 27,000.
DATE: 12/19/2010 FROM: [XXX] RE: 2010 TOYOTA RAV 4 LEASE/ AIR BAG SAFETY CONCERN I RECENTLY LEASED A RAV4 FROM YOUR DEALERSHIP. I TRADED A 2007 PRUIS AND HAVE OWNED NUMEROUS TOYOTAS. MY REASON FOR LEASING THE RAV 4 WAS TO MAKE LIFE EASIER AND SAFER FOR MY SPOUSE WHO IS UNDERGOING CANCER TREATMENT. I MADE THIS FACT KNOWN TO MY SALESMAN SONNY ROTHWELL AND BROUGHT HER WITH ME TO TEST HER GETTING IN AND OUT OF THE VEHICLE AND GENERAL COMFORT. THE VEHICLE WAS NOT RUNNING DURING THIS TEST. I DROVE THE VEHICLE APPROXIMATELY 5000 MILES WITHOUT PAYING ATTENTION TO THE DASH WARNING LIGHT REGARDING AIR BAG ACTIVATION. WHEN I FINALLY CHECKED WITH YOUR SERVICE DEPARTMENT, THEY STATED THAT THE AIR BAG LIGHT INDICATES THAT THE AIR BAG IS NOT IN ACTIVE MODE WHEN OFF AND THAT THIS OCCURS WHEN THERE IS NOT ENOUGH WEIGHT IN THE PASSENGER SEAT. I ASKED THE AMOUNT NEEDED TO ACTIVATE THE ON POSITION AND THEY DID NOT KNOW. MY WIFE IS 5 1 AND WEIGHS 86 POUNDS AND I WOULD NEVER HAVE LEASED THIS VEHICLE IF I WOULD HAVE KNOWN SHE WOULD NOT HAVE THE BENEFIT OF AIR BAG SAFETY. AIR BAGS SAVED MY WIFE AND MOTHER IN-LAW SEVERAL YEARS AGO. I CALLED TOYOTA CORPORATE 800 331-4331 AND EXPLAINED MY PROBLEM. THEY STATED THAT IT REQUIRES 90LBS OF WEIGHT TO ACTIVATE AIR BAG AND THAT THIS COULD NOT BE ADJUSTED. REF# 1012030063 I LATER CALLED SERVICE MANAGER DAVID NEWTON AND HE CONFIRMED THE 90 LB WEIGHT REQUIREMENT. HE SUGGESTED I BRING THE CAR IN. PRIOR TO BRINGING THE VEHICLE TO THE DEALER, I TESTED THE FRONT SEAT BY HAVING MY WIFE SIT IN THE SEAT WITH A 16 LB BOWLING BALL ON HER LAP. THE AIR BAG LIGHT WAS STILL SHOWING OFF. I ALSO READ MY OWNERS MANUAL AND FOUND NO INDICATION OF ANY WEIGHT REQUIREMENT FOR AIR BAG ACTIVATION. THERE WERE REFERENCES TO CHILDREN AND LUGGAGE IN PASSENGER SEAT. I BROUGHT THE CAR INTO YOUR DEAL PARTS OF THIS DOCUMENT HAVE BEEN REDACTED TO PROTECT PERSONALLY IDENTIFIABLE INFORMATION PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6). *CC
TOYOTA RAV4 2009. ELECTRONIC SPEED CONTROL MALFUNCTION. UNIT CONTINUES TO BLINK ON AN OFF WITH NO CONTROL OF THE SPEED OF THE VEHICLE. AT THE SAME TIME THE SLIPPERY ROAD SYMBOL SHOWS UP AND FOR NO REASON (ROAD IS PERFECTLY DRY AND NORMAL). INSTRUMENT PANEL SHOWS NO OTHER SIGNS OF ABERRANT BEHAVIOR. *TR
TL*THE CONTACT OWNS A 2010 TOYOTA RAV4. WHILE MOVING THROUGH A PARKING LOT 4 MPH THE VEHICLE SUDDENLY ACCELERATED. THE CONTACT WAS UNABLE TO STOP THE VEHICLE UNTIL IT CRASHED INTO A STONE POOL. THE VEHICLE WAS TOWED FROM THE SCENE TO THE DEALERSHIP. THE VEHICLE SPEED DID NOT INCREASE ENOUGH TO CAUSE THE AIR BAGS TO DEPLOY. THE CONTACT SPOKE WITH THE MANUFACTURER AND WAS TOLD THAT THE VEHICLE WAS NOT INCLUDED IN RECALL CAMPAIGN NUMBER 10V017000, WHICH WAS RELATED TO THE VEHICLE SPEED CONTROL. THE VEHICLE WAS NOT REPAIRED. THE FAILURE AND CURRENT MILEAGES WERE 8000.
TL*THE CONTACT OWNS A 2010 TOYOTA RAV 4. WHILE THE CONTACT WAS DRIVING 5 MPH, THE VEHICLE ACCELERATED OUT OF CONTROL AND CRASHED INTO A WALL OF CONCRETE. THE VEHICLE WAS TOWED TO A LOCAL REPAIR SHOP WHERE THE TECHNICIAN WILL REPAIR THE BODY WORK ONLY. THE CONTACT CALLED THE MANUFACTURER WHO STATED AN INVESTIGATOR WOULD BE SENT TO INSPECT THE VEHICLE. I WAS GOING ABOUT 4 MPH AROUND A GROCERY STORE PARKING LOT AND THE VEHICLE MADE A LOUD ROARING NOISE AND ACCELERATED. I WAS UNABLE TO STOP ACCEPT FOR A CONCRETE POST IN THE PARKING LOT. I HAD NO CONTROL AND NO BRAKES. THIS INCIDENT HAS LEFT ME WITHOUT A VEHICLE FOR USE. THIS IS DETRIMENTAL TO MY DAILY ROUTINE INCLUDING THE CARE OF MY 93 YEAR OLD FATHER WHO HAS DOCTORS APPOINTMENTS AND OTHER NEEDS THAT REQUIRE A VEHICLE. I HAVE ONLY HAD THE CAR FOR 6 MONTHS AND CANNOT AFFORD TO MAKE CAR PAYMENTS OR PAY $40.00 FOR CAR RENTAL. IT WILL BE OVER 3 WEEKS BEFORE THE DEALERSHIP WILL CHECK ELECTRONIC BOX/THE CAR AND WAS TOLD THAT IT WOULD BE AN ADDITIONAL 3 WEEKS BEFORE I WOULD BE CONTACTED. I WILL NOT BE GIVEN A RENTAL UNTIL PROOF IS DETERMINED. *TR

SHE ENGAGED THE BRAKES AND THE VEHICLE CONTINUED TO ACCELERATED AND CRASH INTO A CONCRETE BARRIER. SHE SUSTAINED BRUISES TO HER ARM AND LEG. A POLICE REPORT WAS FILED. THE VEHICLE WAS DAMAGED FROM THE FENDER AT AN ANGLE TO THE DRIVER'S SIDE DOOR. THE VEHICLE WAS REPAIRED. THE APPROXIMATE FAILURE MILEAGE WAS 8,000 AND THE CURRENT MILEAGE WAS 8,110.
2010 TOYOTA RAV 4. CONSUMER STATES PROBLEM WITH SUDDEN ACCELERATION. *TGW THE CONSUMER STATED HE WAS STOPPED AT A RED LIGHT WAITING FOR THE LIGHT TO TURN GREEN. THE CONSUMER STATED HE HAD HIS FOOT ON THE BRAKE, WHEN THE VEHICLE SUDDENLY ACCELERATED. THE CONSUMER STATED HE QUICKLY TURNED THE IGNITION KEY TO THE OFF POSITION AND THE ENGINE STOPPED. WHEN HE TURNED THE ENGINE ON AGAIN, THE ACCELERATION RETURNED. *JB
WHILE DRIVING MY TOYOTA RAV 4 BUILD SEP/2010, COMPLIANCE OCT 2010 AT 20KM PER HOUR, AUTOMATIC, I WAS BEGINNING TO BRAKE GENTLY AND MOVE INTO NEUTRAL WHEN THE ENGINE REV UP AT 8000 RPM. I COMPLETED THE MANEUVER TO PUT THE CAR IN NEUTRAL (WITHIN MILLISECONDS OF THE PROBLEM HAPPENING) AND AFTER WATCHING THE CLOCK SHOWING 8000 RPM FOR 1 SECOND I TURNED THE CAR OFF. I DID NOT SUFFER ANY INJURIES, BUT I HAVE BAD DREAMS ABOUT IT. MY WIFE WAS IN THE CAR AT THE TIME AND SHE ALSO WITNESSES THE EVENT. TOYOTA IS INVESTIGATING... *TR
WHILE DRIVING IN HEAVY TRAFFIC ON CITY SIDE STREET, VEHICLE IN FRONT STOPPED, I ATTEMPTED TO DO THE SAME, BUT UNSUCCESSFULLY. MY VEHICLE WAS A RENTAL AND RENTAL COMPANY REPLACED IT WITH AN IDENTICAL VEHICLE. SUBSEQUENT EXPERIMENTATION WITH SAID REPLACEMENT SHOWED WHAT I BELIEVE IS A WEAKNESS IN THE ELECTRONICALLY CONTROLLED BRAKE SYSTEM. WITH ONE APPLICATION OF THE BRAKE PEDAL A CERTAIN PERCENTAGE OF BRAKE POWER IS AVAILABLE, WITH A CLOSELY FOLLOWED SECOND APPLICATION, THE PERCENTAGE OF AVAILABLE BRAKING POWER IS INCREASED, AFTER A THIRD, EVEN MORE. THE BRAKING POWER AVAILABLE AFTER THE FIRST APPLICATION IS LOW, EVEN WITH FULL PEDAL APPLICATION. THIS CONSTITUTES A HAZARD IN THE EVENT OF A NEED FOR FULL BRAKING POWER. IT IS MOST LIKELY THAT A PERSON ENCOUNTERING A SITUATION AS I FOUND MYSELF IN WOULD DO WHAT I DID, I.E. APPLY MAXIMUM PRESSURE TO THE BRAKE PEDAL ONLY TO FIND THAT BECAUSE OF THE LOW AVAILABLE BRAKING POWER THE VEHICLE, PERSON, OR ANIMAL WOULD NOT ESCAPE DAMAGE OR INJURY.
TL*THE CONTACT OWNS A 2010 TOYOTA RAV4. THE CONTACT STATED THAT HIS PASSENGER WEIGHS 78 POUNDS AND WHENEVER SHE WAS IN THE PASSENGER SEAT THE AIR BAG LIGHT FAILED TO ENGAGE. THE VEHICLE WAS TAKEN TO THE DEALER WHO STATED THAT THE PERSON SITTING IN THE FRONT SEAT SHOULD BE 80 POUNDS OR MORE BEFORE THE AIR BAG LIGHT WOULD ILLUMINATE. THEY ALSO WERE UNABLE TO ADJUST THE SEAT IN ANY MANNER REGARDING THE AIR BAG LIGHT IN ORDER TO ACTIVATE THE ON POSITION. SHORTLY AFTER DEALER MAINTENANCE, THREE LUG BOLTS BROKE OFF WHILE TORQUING THE LUG NUTS TO 80 FOOT POUNDS. THE LUG BOLTS APPEAR TO MADE FROM A LOW GRADE OF STEEL. THE DEALER WAS GOOD ENOUGH TO REPLACE BROKEN BOLTS, BUT REFUSED TO REPLACE ALL THE LUG BOLTS. I MAINTAIN THAT ALL MY LUG BOLTS COULD BE DAMAGED TO THE POINT OF NEAR FAILURE AND THAT TOYOTA SHOULD BE USING A HIGHER GRADE STEEL IN THESE BOLTS. *TR
MY 2010 TOYOTA RAV4 WAS STOPPED AT A TRAFFIC LIGHT, WITH MY FOOT ON THE BRAKES. THE CAR SLOWLY BEGAN ACCELERATING DESPITE NORMAL PRESSURE BEING APPLIED TO THE BRAKES, AND THEN SUDDENLY THE ENGINE BEGAN REVVING LOUDLY AND THE CAR BEGAN TO ACCELERATE MORE QUICKLY. THE CAR ACCELERATED TO PERHAPS A FEW MILES AN HOUR INTO THE INTERSECTION BEFORE I WAS ABLE TO STOP IT BY APPLYING HEAVY PRESSURE ON THE BRAKES AND ALSO APPLYING THE PARKING BRAKE. AFTER I WAS ABLE TO STOP THE CAR, THE ENGINE CONTINUED TO REV INCREASINGLY LOUDLY FOR SEVERAL MORE SECONDS (WITH OBVIOUS POWER THAT I COULD EASILY FEEL TRYING TO OVERCOME THE BRAKES I WAS APPLYING) UNTIL I WAS ABLE TO SHUT THE ENGINE OFF WITH THE IGNITION SWITCH. THE INCIDENT DID NOT RESULT IN AN ACCIDENT. BEING IN A BUSY AND DANGEROUS INTERSECTION, I DECIDED TO RESTART THE CAR AND DRIVE IT WITH EXTREME CAUTION TO A SAFE AREA TO PARK. WHEN RESTARTED, THE CAR OPERATED NORMALLY AND PRESENTED NO FURTHER PROBLEMS. THE CAR'S AIR CONDITIONING AND SATELLITE RADIO WERE OPERATING AT THE TIME OF THE INCIDENT. WEATHER CONDITIONS WERE CLEAR AND SUNNY, AND THE ROADS WERE DRY. THE CAR DOES HAVE A RUBBER ALL-WEATHER FLOOR MAT ON THE DRIVER'S SIDE, WHICH WAS SECURED BY HOOKS INSTALLED BY THE TOYOTA DEALERSHIP WHERE I PURCHASED THE CAR. THE EDGE OF THE FLOOR MAT IS SEVERAL INCHES AWAY FROM THE ACCELERATOR PEDAL WHEN IT IS FULLY DEPRESSED, AND THE MAT ITSELF WILL NOT PHYSICALLY MOVE OR SLIDE EVEN WHEN PURPOSEFULLY TRYING TO MOVE IT WITH FORCE. I'M ABSOLUTELY CERTAIN THAT IT DID NOT MOVE OR OTHERWISE INTERFERE WITH THE ACCELERATOR PEDAL. THIS IS THE FIRST AND ONLY TIME THAT I HAVE EXPERIENCED THIS PROBLEM. AFTER THE INCIDENT, I DROVE THE CAR WITH EXTREME CARE TO MY TOYOTA DEALERSHIP (AND EXPERIENCED NO FURTHER PROBLEMS), WHERE IT IS NOW BEING HELD FOR INSPECTION. THE CAR'S MILEAGE AT THE TIME OF THE INCIDENT WAS APPROXIMATELY 6,200 MILES. IT WAS ALSO SERVICED BY TOYOTA SEVERAL MONTHS AGO IN RESPONSE TO THE RECALL RELATING TO UNINTENDED ACCELERATION. *TR
MY ADULT DAUGHTER WAS SLOWLY BACKING OUT OF OUR DRIVEWAY IN OUR 2010 TOYOTA RAV4. THE CAR SUDDENLY ACCELERATED. THE CAR WENT OUT OF CONTROL AND THE FRONT SIDE OF THE CAR HIT SOME ROCKS CAUSING THE BUMPER TO BREAK. *TR
VEHICLE WAS AT THE STOP SIGN FOR THE HIGH SPEED TRAIN. THE VEHICLE WAS REARENDED BY A TRUCK. BRAKES WERE APPLIED, THE BRAKES FAILED AND THE VEHICLE CONTINUED TO GO TOWARDS THE RAILROAD CROSSING WITH APPROACH OF THE HIGH SPEED TRAIN. THE DRIVER IN DESPERATION MADE A U-TURN STILL TRYING TO STOP THE VEHICLE. IN DESPERATION THE GEAR WAS SHIFTED TO PARK AND THE KEY REMOVED, THE VEHICLE CONTINUED TO ROLL JUMPED THE CURB AND FORTUNATELY RAN INTO TREE WHICH STOPPED THE VEHICLE. THE VEHICLE BODY DAMAGE IS BEING REPAIRED. *TR
MY VEHICLE IS A 2010 TOYOTA RAV4-V6 WITH AUTOMATIC TRANSMISSION. WHILE COASTING OR MAINTAIN A LOW SPEED OF APPROXIMATELY 5MPH, I PRESS THE ACCELERATOR AND THE VEHICLE HESITATES ABOUT 3-5 SECONDS. DURING THE HESITATION, THE ENGINE REVS HIGH AND THE CAR DOES NOT ACCELERATE. AT THE END OF THE HESITATION, THE TRANSMISSION GOES INTO GEAR WITH A THUD AND A JERK TO THE CAR. I WAS INVOLVED IN THREE NEAR-MISS ACCIDENTS WHILE TRYING TO MERGE INTO TRAFFIC SINCE THE VEHICLE WOULD NOT MOVE FOR THE 3-5 SECONDS OF HESITATION. I SEARCHED THE PROBLEM ON THE INTERNET AND FOUND SIMILAR PROBLEMS WITH OTHER RAV4 OWNERS. *TR
I BOUGHT THE 2010 TOYOTA RAV4 THIS JULY. THE CAR WAS FINE UNTIL TODAY, SEPTEMBER 19TH, 2010. TODAY AT 11:45 AM, I WAS BRAKING AS I APPROACHED THE RED LIGHT GOING AT 25MPH. WHEN I PRESSED THE BRAKE THE CAR CONTINUED TO ACCELERATE. I PRESSED THE BRAKE EVEN HARDER, THE CAR JUMPED THE LIGHT, AND EVEN THOUGH THE CAR DID SLOWED, I HAD TO SLAM IN ORDER TO COME TO A COMPLETE HALT. I THEN PULLED OVER AND RESTARTED THE ENGINE. ALTHOUGH THERE WAS NO ACCIDENT I WAS VERY SCARED. AT 4:30 PM THE SAME DAY, I JUST EXITED THE HIGHWAY AND WAS COMING TO THE RED LIGHT. I WAS BRAKING AT 25MPH, YET THE CAR CONTINUED TO ACCELERATE. I HAD TO SLAM THE BRAKES EXTREMELY HARD AND ALTHOUGH IT DID COME TO A COMPLETE STOP, I HEARD THE ENGINE REVVING. AGAIN I WAS VERY SCARED. NO ACCIDENTS OCCURRED. *TR
AFTER 148,603 MILES ON MY 2002 RAV4, THE TRANSMISSION BEGAN SHIFTING BACK & FORTH BETWEEN 1ST & 2ND GEAR WITH VIOLENT SHAKING. SHIFTING FROM DRIVE TO REVERSE ALSO CAUSED VIOLENT/LOUD CLUNKING. A SCANNER WAS USED TO RETRIEVE TROUBLE CODES WHICH YIELDED A P0755 CODE (SHIFT SOLENOID MALFUNCTION). UPON RESEARCH ON VARIOUS TOYOTA RAV4 FORUMS I FOUND MANY OTHERS WITH SAME SYMPTOMS. *TR
MY NEW 2010 TOYOTA RAV4 MAKES SQUEAKING NOISE FROM THE REAR WHEEL WHILE I APPLY BRAKE AFTER A COLD START. THE NOISE GOES AWAY AFTER APPLYING THE BRAKES COUPLE OF TIMES. *TR

ON SEPTEMBER 1 I WAS DRIVING IN A CONGESTED AREA APPROACHING AN INTERSECTION WITH STOPPED TRAFFIC. AS I WAS APPLYING MY BRAKES I SUDDENLY FELT MY 2010 TOYOTA RAV4 LURCH FORWARD - AT FIRST I THOUGHT I HAD BEEN HIT FROM BEHIND, BUT A QUICK GLANCE IN MY REARVIEW MIRROR INDICATED THE CAR BEHIND ME WAS STILL AT A SAFE DISTANCE. I QUICKLY TURNED OFF MY RADIO TO HEAR MY ENGINE ROARING AS I WAS CONTINUING TO APPLY THE BRAKES. I THEN PUT THE RAV4 IN NEUTRAL WHILE THE ENGINE CONTINUED TO ROAR. ONCE IN NEUTRAL, THE RAV4 CAME TO A STOP. I THEN DEPRESSED THE ACCELERATOR SEVERAL TIMES AND THE ENGINE SETTLED DOWN. AS I APPROACHED THE INTERSECTION I WAS TRAVELING AT A SLOW RATE OF SPEED. WHEN I APPLIED THE BRAKES THE RAV 4 WAS SLOWLY COMING TO A STOP UNTIL I FELT THE CAR LURCH FORWARD. ONCE I BEGAN APPLYING THE BRAKES I DID NOT TAKE MY FOOT OFF THE BRAKE PEDAL, NOR DID I APPLY ANY PRESSURE TO THE ACCELERATOR. I WAS DRIVING MY 2007 TOYOTA RAV4 UP A MOUNTAIN DIRT ROAD IN WORLDS END STATE PARK IN SULLIVAN COUNTY PA. I WAS TRAVELING APPROX 15 MPH. I HIT THE BRAKE TO SLOW DOWN WHEN THE THROTTLE SUDDENLY WENT TO FULL ACCELERATION. I PRESSED HARD ON THE BRAKE, PUT THE TRANSMISSION INTO NEUTRAL, STOPPED THE VEHICLE AND SHUT OFF THE ENGINE. I PRESSED THE GAS PEDAL A FEW TIMES AND RESTARTED THE ENGINE. EVERYTHING SEEMED FINE. THE SAME THING OCCURRED ON THE WAY DOWN THE MOUNTAIN. THIS TIME I WAS IN LOW GEAR AND THE BRAKE WOULD NOT HOLD THE VEHICLE. IT TOOK ME A FEW SECONDS TO SHIFT INTO NEUTRAL AND SHUT OFF THE ENGINE. THE ROAD TOOK A SHARP TURN TO THE RIGHT, WHICH IS WHY I HIT THE BRAKE TO SLOW DOWN, AND I JUST MANAGED TO STOP SHORT OF GOING OFF THE ROAD INTO THE TREES. I HAD NO FURTHER PROBLEMS ON MY TRIP HOME. AT APPROXIMATELY 5:00 PM ON JULY 19TH, 2010, OUR TOYOTA RAV 4 (MODEL YEAR 2010 MILEAGE 6450) EXPERIENCED AN INCIDENT OF UNINTENDED ACCELERATION. DRIVING HOME WITH 4 MEMBERS OF THE FAMILY, WE APPROACHED AN INTERSECTION AND THE DRIVER BEGAN TO APPLY THE BRAKE. THE ROADWAY WAS CROWDED, SO OUR SPEED WAS FORTUNATELY MINIMAL (APPROXIMATELY 25 MPH). WHILE APPLYING THE BRAKE, THE ENGINE SUDDENLY BEGAN TO REV AT A HIGH RATE, THE DRIVER SAID THE CAR WAS NOT RESPONDING WELL TO THE BRAKE AND WE MAY NOT STOP IN TIME AS WE APPROACHED THE INTERSECTION. INSTINCTIVELY, I LOOKED AT THE DRIVER'S FOOT TO SEE IF THERE WAS SOME PROBLEM WITH HIM HITTING THE GAS PEDAL, BUT COULD CLEARLY SEE BOTH FEET AND ONLY THE BRAKE BEING APPLIED. THE DRIVER PUMPED THE BRAKE, LIFTING HIS FOOT FROM THE BRAKE AND REAPPLYING; WE JOLTED FORWARD HE LIFTED HIS FOOT FROM THE BRAKE OF THE REVVING CAR AND THEN REAPPLIED THE BRAKE. THE ENGINE SUDDENLY SLOWED AND RETURNED TO A NORMAL IDLE AND WE WERE ABLE TO STOP WITHOUT HITTING ANYONE. WE DID NOT FILE A POLICE REPORT, BUT CONTACTED THE DEALER THE FOLLOWING MORNING AND WERE INSTRUCTED TO BRING THE CAR IN FOR INSPECTION THE FOLLOWING DAY. NOT SURPRISINGLY, AS THEY APPEAR INCAPABLE OF DETECTING THE PROBLEM WITH THEIR DIAGNOSTICS, NO PROBLEM WAS FOUND AND WE WERE ASKED TO RECLAIM THE CAR A FEW DAYS LATER. WE TOLD THE DEALERSHIP THAT WE CONSIDER THIS LEASED CAR TO BE UNSAFE AND DO NOT WANT ANYTHING TO DO WITH THE CAR AND WISHED TO RETURN IT AS IT IS CLEARLY DEFECTIVE. 4 MEMBERS OF MY FAMILY WITNESSED THE EVENT AND NO ONE IS GETTING IN THE CAR AGAIN. THE TOYOTA DEALERSHIP'S CUSTOMER SERVICE REPRESENTATIVE SAID THEY ARE ONLY DEALER, THEY ARE NOT THE MANUFACTURERS AND WE HAD TO ADDRESS ANY CONCERNS REGARDING A RETURN TO THE MANUFACTURER, IN THEIR OPINION IT CHECKED OUT FINE. THE CAR HAD ALREADY BEEN SERVICED BY THE DEALER SEVERAL MONTHS PRIOR, TO ADDRESS THE RAV 4 RECALL FOR THE UNINTENDED ACCELERATION PROBLEM. CLEARLY IN OUR CASE THEIR FIX DID NOT FIX THE PROBLEM. *TR

FORTUNATELY, SINCE MY HUSBAND WAS ABOUT TO BRAKE, WE WERE ABLE TO NARROWLY AVOID A COLLISION WITH THE VEHICLE AHEAD OF US. WE CONTACTED OUR LOCAL DEALERSHIP TO INSPECT THE NEW TOYOTA. THEY COULDN'T IDENTIFY THE CAUSE OF THE INCIDENT, BUT THEY DID INDICATE THAT IT MAY HAVE BEEN A FLOOR MAT PROBLEM. WE EXPLAINED THAT THE ACCELERATOR DID NOT SEEM STUCK AND THAT WE WERE TRAVELING AT AN EXTREMELY SLOW RATE OF SPEED AND THEN UNINTENTIONALLY ACCELERATED. WE CONTINUED TO BE CONCERNED AND SO WE CONTACTED TOYOTA. THEY EXPLAINED THAT THE DEVICE INSTALLED PURSUANT TO THE SAFETY RECALL WAS INTENDED TO ASSIST THE ACCELERATOR IN RETURNING TO AN IDLE POSITION. HOWEVER, AS WE EXPLAINED, THE ACCELERATOR WAS IN AN IDLE POSITION WHEN THIS INCIDENT OCCURRED. WE CONTINUE TO BE SCARED TO DEATH TO DRIVE OUR NEW TOYOTA RAV4. AFTER HAVING HAD TWO EVENTS OF SUDDEN ACCELERATION IN AN 2007 TOYOTA RAV4, THE DEALER, METRO TOYOTA, TALKED US UNTO TRADING IN THE 2007, FOR A BRAND NEW 2010 RAV4, AFTER ASSURING US THAT THERE WERE NO SIMILAR COMPLAINTS WITH THE 2010 MODEL. WE DROVE THIS CAR FOR ABOUT 230 MILES, WHEN ON JUNE 20 ,2010, A MILE AFTER EXITING THE FREEWAY, WITH MY WIFE IN THE PASSENGER SEAT, I WAS COMING UP TO A RED LIGHT,GOING DOWN A SLIGHT SLOPE,ON THE ROAD,WHEN THE CAR ENGINE SURGED. I PUT THE GEAR IN NEUTRAL, DROVE THE CAR PARALLER AND NEAR SOMEBODY'E DRIVEWAY, PUT THE CAR IN PARK, AND WAS PREPARING TO GO KNOCK ON THE DOOR OF THE HOMEOWNER TO ASK HIM/HER TO COME AND WITNESS THE,RPMS, WHEN THE CAR SURGING STOPPED. I CAREFULLY DROVE THE CAR HOME. I DID NOT CALL THE DEALER THAT EVENING. THE NEXT DAY, I WAS GOING TO A NEARBY GROCERY, PREPARING TO GET INTO A LEFT TURNING LANE, APPROACHING A RED LIGHT, AGAIN GOING DOWN A SLIGHT INCLINE ON THE ROAD, MY FOOT ON THE BRAKE, WHEN I FELT THE ENGINE REV UP STRONGLY. I PRESSED HARD ON THE BRAKE, PUT THE GEAR IN NEUTRAL, LIFTED UP THE HAND BRAKE, AND A GOT OUT OF THE CAR FOR A WHILE HOPING TO SPOT A POLICE CAR OR HAVE SOMEONE ELSE COME AND WITNESS THIS TERRIBLE EVENT. SEEING NO ONE WILLING TO STOP, I GOT BACK IN THE CAR, AGAIN PRESSED MY FOOT ON THE BRAKE, BUT THE RPMS REMAINED ABOVE 5000. AFTER WAITING ONE MORE MINUTE, THE SPEED OF THE ENGINE CAME DOWN TO NORMAL, I PICKED UP THE REQUIRED ITEMS AT THE GROCERY STORE, AND RETURNED TO MY HOME NEARBY. I CALLED THE DEALER, THEY SENT A MAN WHO TOOK THE CAR TO SERVICE. THE CAR WAS THERE FOR OVER THREE WEEKS. THEIR ANSWER WAS THAT THE CAR CHECKED OK, THEY ATTEMPTED NO REPAIR. EACH TIME I WAS ABLE TO DETERMINE THAT THE CAUSE OF THE UNINTENDED ACCELERATION WAS THAT MY FOOT WAS OVERLAPPING THE BRAKE PEDAL AND WAS ALSO DEPRESSING THE ACCELERATION PEDAL AT THE SAME TIME. THE ACCELERATION WAS OVERRIDING THE BRAKING UNTIL I RELEASED PRESSURE ON THE BRAKE PEDAL. THIS IS A VERY SERIOUS SAFETY ISSUE! LAST WEEK I ALMOST KILLED A MOTHER AND HER YOUNG CHILD DURING A PARKING SITUATION. I WAS TRAVELING AT ABOUT 5MPH WHEN I INTENDED TO COME TO A SLOW STOP OUTSIDE A NOSE IN PARKING SPOT. THE YOUNG MOTHER WAS TIEING THE SHOESTRINGS OF HER YOUNGSTER NEAR THE OUTSIDE MIDDLE OF THE PARKING SPOT I INTENDED TO USE. MY CAR WAS TRAVELING DIRECTLY TOWARD HER WHEN I SLOWLY DEPRESSED THE BRAKE PEDAL. THE CAR STARTED TO ACCELERATE! I IMMEDIATELY RECOGNIZED THE PROBLEM AND STOPPED THE CAR BY SHIFTING MY RIGHT FOOT TO THE LEFT WHICH ALLOWED ME TO APPLY PRESSURE TO THE BRAKE PEDAL WITHOUT SIMULTANEOUSLY DEPRESSING THE ACCELERATOR PEDAL! WE STOPPED ABOUT 8 FEET FROM THE MOTHER AND CHILD. HAD I NOT EXPERIENCED THIS PROBLEM SEVERAL TIMES BEFORE AND THEREFORE HAD KNOWN HOW TO CORRECT IT, MY RESPONSE WOULD HAVE BEEN TO DEPRESS THE BRAKE PEDAL EVEN MORE. I COULD EASILY HAVE DRIVEN OVER THE MOTHER AND HER CHILD! I AM AWARE THAT TOYOTA HAS A RECALL ON MANY OF ITS PRODUCTS BECAUSE OF UNINTENTIONAL ACCELERATIONS; BUT I AM UNAWARE OF ANY INVESTIGATIONS THE FOCUS ON THE CAUSE OF MY NEAR-FATAL INCIDENT. I AM CERTAIN THAT SOME OF THE ACCIDENTS REPORTED ON THE RAV 4, V6 ARE ATTRIBUTABLE TO THE LACK OF ADEQUATE SEPARATION BETWEEN THE BRAKE AND ACCELERATOR PEDALS! I HAVE A TOYOTA 2010 RAV4, BUT PRESUMED ALL NEW TOYOTA VEHICLES, ENCOURAGE RECIRCULATING TOXIC GASES IN A CLOSED VEHICLE. TOYOTA'S AUTOMATIC CLIMATE CONTROL SYSTEM AUTOMATICALLY PUTS THE AIR ON RECIRCULATE EVERY TIME THE CAR IS STARTED. REGARDLESS OF INSIDE / OUTDOOR TEMP; REGARDLESS OF HUMID OR DRY; REGARDLESS OF EVERYTHING THE AIR IS ALWAYS ON RECIRCULATE WHENEVER THE CAR IS STARTED. TOYOTA CLAIMS THAT THE CAR AUTOMATICALLY SENSES AND ADJUSTS ACCORDING TO CONDITIONS - BUT THIS IN NOT TRUE! THE CAR ALWAYS STARTS AND STAYS ON RECIRCULATE. KNOWING THE HAZARDS AND RECOMMENDATIONS FOR SAFETY LEVELS OF THE TOXIC GASES THAT ACCUMULATE IN A CLOSED CAR (SUCH AS, HTTP://WWW.SNOPES.COM/MEDICAL/TOXINS/BENZENE.ASP), TOYOTA'S CURRENT AIR SYSTEM IS PROVING HAZARDOUS TO EVERYONE'S HEALTH! THE VEHICLE CONTINUED TO ACCELERATE WHILE THE CONTACT WAS APPLYING EXTREME FORCE TO THE BRAKES. HE THEN SHIFTED THE VEHICLE INTO NEUTRAL AND THEN INTO PARK. THE VEHICLE SLOWLY BEGAN TO DECREASE IN SPEED. THE DEALER ADVISED BRINGING THE VEHICLE IN FOR DIAGNOSTIC TESTING AND FURTHER INSPECTION. THE FAILURE AND CURRENT MILEAGES WERE 17,000.
