I RECEIVED NOTIFICATION OF NHTSA CAMPAIGN NUMBERS: 16V096000 (SEAT BELTS) AND 15V577000 (VISIBILITY/WIPER), BUT HAVE BEEN UNABLE TO OBTAIN THE PART FROM THE DEALER FOR AN UNREASONABLE AMOUNT OF TIME. THE MANUFACTURER WAS UNABLE TO PROVIDE A SPECIFIC DATE WHEN THE PARTS WOULD BE AVAILABLE. I HAVE NOT EXPERIENCED A FAILURE.

I WAS DRIVING IN HEAVY TRAFFIC AT APPX: 35-40MPH. ON US INTERSTATE #66. THE WEATHER WAS CLEAR, AND THE ROAD WAS RELATIVELY LEVEL. AS THE OTHER CARS SLOWED, I TOOK MY FOOT OFF THE ACCELERATOR BUT THE CAR SURGED FORWARD. UPDATED 01/11/17*LJ I --LITERALLY--HAD TO STAND ON THE BRAKE AS THE ENGINE SURGED. I MANAGED TO PUSH THE SHIFTER INTO PARK. THE ENGINE CONTINUED TO RACE BUT, WHEN I PUMPED THE ACCELERATOR SEVERAL TIMES, THE ENGINE SPEED DROPPED BACK TO IDLE. I DID NOT HIT ANY OTHER VEHICLE OR OBJECT. I HAVE HAD TWO (2) SIMILAR INCIDENTS IN THE LAST 5-6 MONTHS-- BUT THIS IS THE FIRST TIME THAT I REPORTED IT--TO BOTH NHTSA AND TOYOTA. I TOOK IT TO THE TOYOTA DEALERSHIP, EXPECTING A QUICK FIX BUT INSTEAD, THE DEALERSHIP PERSONNEL BECAME DEFENSIVE, CLAIMED THAT THE WELL KNOWN HISTORY OF TOYOTA ACCELERATION PROBLEMS WAS A FRAUD. SUDDEN ONSET INTERMITTENT HARD SHAKING VIBRATION AND METAL-LIKE LOUD RATTLING COMING FROM DRIVER BACK WHEEL/AXEL REGION BEGINS AT 35-40MPH (WHEN DRIVING STRAIGHT ON RESIDENTIAL ROADS, ESPECIALLY WHEN ROAD IS ROUGH). FEELS LIKE SOMETHING UNDER THAT CORNER OF THE CAR IS GOING TO CRACK APART/CAVE IN. IS THIS SAME REAR SUSPENSION ISSUE THAT THE 2006-2011 RAV4 WAS RECALLED FOR?
TL* THE CONTACT OWNS A 2012 TOYOTA RAV4. WHILE ATTEMPTING TO ENGAGE THE BRAKES, THE VEHICLE ACCELERATED WITHOUT WARNING. THE VEHICLE WAS TAKEN TO THE DEALER, BUT THE TECHNICIAN WAS UNABLE TO DIAGNOSE THE CAUSE OF THE FAILURE. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE. THE VEHICLE WAS NOT REPAIRED. THE FAILURE MILEAGE WAS 55,000. THE CONSUMER WROTE IN ON BEHALF OF HER MOTHER. THE CONSUMERS MOTHER EXPERIENCED SUDDEN ACCELERATION ON THE DIFFERENT OCCASIONS. EACH TIME, WHEN HER MOTHER APPLIED THE BRAKE, THE VEHICLE SHUDDERED AND THEN ACCELERATED ON ITS OWN AND ON EACH OCCURRENCES, HER MOTHER WAS ABLE TO TURN THE IGNITION OFF TO STOP THE VEHICLE. I WAS NOTIFIED AT THE END OF APRIL THAT THERE WAS A RECALL REGARDING THE SAFETY OF THE BACKSEAT SEATBELTS POSSIBLY SEPARATING IN THE EVENT OF A FRONTAL CRASH. I WAS PROVIDED WITH A COURTESY VEHICLE AND HAVE BEEN DRIVING IT FOR OVER 2 MONTHS WHILE STILL PAYING FOR MY VEHICLE THAT SITS IN A DEALERSHIP LOT AWAITING A FIX. I CONTINUE TO BE TOLD THAT THERE IS NOT YET A FIX FOR THIS RECALL. I FIND IT UNACCEPTABLE TO NOT HAVE IDENTIFIED A FIX OR MADE PARTS AVAILABLE TO RECTIFY THIS MATTER. AT THIS TIME, I WOULD LIKE AN ALTERNATIVE TO THIS MATTER.

IT HAD NOT BEEN DETERMINED WHETHER THE VEHICLE WAS DESTROYED OR SALVAGEABLE. A POLICE REPORT WAS FILED. THE MANUFACTURER WAS NOT NOTIFIED. THERE WAS ONE INJURY THAT REQUIRED MEDICAL ATTENTION. THE DRIVER WAS SEVEN MONTHS PREGNANT AND REQUIRED X-RAYS DUE TO THE RISK TO THE INFANT. THE FAILURE MILEAGE WAS APPROXIMATELY 59,000.

THIS RECALL AFFECTS THE REAR PASSENGER SAFETY BELTS IN THE VEHICLE. THERE IS A POTENTIAL THE SEAT BELTS MAY BECOME CUT OR SEPARATED. I HAVE A FAMILY TO DRIVE AROUND SO I ASKED THAT TOYOTA PROVIDE A LOANER VEHICLE UNTIL THE ISSUE COULD BE RESOLVED. I CONTACTED BOTH TOYOTA CUSTOMER SERVICE AND MY LOCAL TOYOTA DEALERSHIP, BOTH DENIED MY REQUEST FOR A SAFE VEHICLE TO DRIVE TO PROTECT MY FAMILY. I ALSO HAVE AN INFANT AND MY INFANT CAN ONLY RIDE IN THE BACK OF THE CAR. THE RAV 4 IS A FOUR PASSENGER VEHICLE AND WE WERE ADVISED BY A CAR SEAT SPECIALIST NOT TO INSTALL THE CAR SEAT IN THE MIDDLE. WE WOULD LIKE A LOANER VEHICLE AS SOON AS POSSIBLE AND WE MAY SEEK LEGAL REPRESENTATION TO TAKE ACTION AGAINST TOYOTA IN ORDER TO PROTECT MY FAMILY FROM THIS UNSAFE VEHICLE. NHTSA RECALL 16V-096

ACCORDING TO NHTSA CAMPAINGN NUMBER, MY 2012 RAV4 SHOULD HAVE BEEN RECALLED IN 2013 . I WAS NOT NOTIFIED THEN OR NEVER. I AM AFRAID TO DRIVE MY VEHICLE AND CAN'T SEEM TO GET ANY ANSWERS ABOUT MY AIRBAG. ALL I WANT TO KNOW IS WHAT KIND OF AIRBAG I HAVE AND IF IT'S MADE BY TAKADA , I WANT IT CHANGED.

THE VEHICLE WAS DRIVEN TO THE DEALER AND THE CONTACT WAS REFERRED TO THE MANUFACTURER FOR DIAGNOSTIC TESTING. IT WAS DIAGNOSED THAT THE WINDSHIELD WIPER MOTOR NEEDED REPLACEMENT. THE CONTACT RECEIVED A RECALL NOTIFICATION FOR NHTSA CAMPAIGN NUMBER: 15V577000 (VISIBILITY/WIPER). THE REMEDY WAS NOT YET AVAILABLE. THE CONTACT ALSO STATED THAT THE FAILURE OCCURRED EVERY TIME IT RAINED AT NIGHT. THE MANUFACTURER WAS NOTIFIED OF THE FAILURE. THE APPROXIMATE FAILURE MILEAGE WAS 25,000.
GOT INTO A SEVERE CAR ACCIDENT. THE VEHICLE THAT HIT US WAS GOING APPROX 50MPH HEAD ON. OUR AIR BAGS DID NOT DEPLOY...UPDATED 03/15/16 *BF *TR
MY 2012 RAV4 EV HAS AN UNSAFE CONDITION WHERE THE ELECTRONIC TRANSMISSION DOES NOT ACTUALLY ENGAGE FROM R TO D, OR D TO R WHEN STATIONARY, EVEN THOUGH THE VEHICLE INSTRUMENTATION INDICATES SO. THE VEHICLE INSTEAD MOVES INADVERTENTLY IN THE WRONG DIRECTION. IT HAS BEEN A CHRONIC ISSUE WITH THE VEHICLE, AND I HAVE REPORTED IT TO THE TOYOTA DEALER ON SEVERAL OCCASIONS. EACH TIME I WAS TOLD THAT THE TECHNICIAN COULD NOT DUPLICATE THE CONDITION, BUT THAT A SOFTWARE UPDATE WOULD BE INSTALLED AND MIGHT CORRECT THE MATTER. IT HAS NOT, AND MY WIFE AND I CONTINUE TO EXPERIENCE THE PROBLEM RANDOMLY. ON MARCH 5, 2016 HOWEVER, THE CONDITION CAUSED AN ACCIDENT AS I WAS LEAVING A PARKING SPOT ON THE STREET. I INITIALLY HAD TROUBLE GETTING THE CAR TO ENGAGE IN REVERSE OR DRIVE. I TURNED THE CAR OFF AND RESTARTED IT (REBOOTING THE SOFTWARE). THIS TIME REVERSE ENGAGED AND I MOVED THE CAR BACKWARD TO MAKE ROOM TO EXIT THE PARKING SPACE AND CAME TO A COMPLETE STOP. I THEN PUT THE CAR IN DRIVE, HEARD THE REVERSE 'BEEP' STOP, VISUALLY CHECKED THE INSTRUMENTATION CLUSTER TO VERIFY THE INDICATOR WAS ON 'D', APPLIED THE HILL-ASSIST FUNCTION, AND PUSHED THE ACCELERATOR TO DRIVE FORWARD. THE CAR INSTEAD LUNGED BACKWARD AND SLAMMED INTO THE CAR BEHIND ME. THIS IS A SERIOUS AND DANGEROUS CONDITION. IT COULD HAVE EASILY BEEN A PEDESTRIAN BEHIND ME INSTEAD OF A CAR. I CAN NO LONGER DRIVE THIS CAR AND FEEL SAFE WITH MY FAMILY, OR FOR THE PEOPLE AROUND THE VEHICLE. I GOT OUT OF MY CAR AND FORGOT TO PUT IT IN PARK. BECAUSE THE CAR HAS KEYLESS START, I WAS ABLE TO LEAVE THE CAR WITHOUT TAKING A KEY OUT. THE CAR STARTED TO ROLL BACK AND BECAUSE I LIVE AT THE TOP OF A HILL, IT BEGAN TO PICK UP SPEED. MY NEIGHBOR LIVES AT THE BOTTOM OF MY DRIVEWAY AND THE CAR WAS HEADING STRAIGHT TO THEIR HOUSE. LUKILY, THE CAR HIT A TREE WHICH STOPPED IT FROM HITTING THEIR HOUSE AND POTENTIALLY KILLING SOMEONE. THE ADJUSTER JUST TOLD ME MY CAR IS TOTALED DUE TO THE SPEED AT WHICH IT HIT THE TREE. HOWEVER, HE ALSO INDICATED THAT THE SPEED AT WHICH THE CAR WAS TRAVELLING WHEN IT HIT THE TREE, IT MAY HAVE HIT THE NEIGHBOR'S HOUSE AND CONTINUED THROUGH THE OTHER SIDE. THIS IS A SIGNIFICANT SAFETY FLAW OF WHICH NO ONE I SPEAK TO ABOUT IT IS AWARE. THEY ACTUALLY TEST IT WHICH THEIR OWN CAR AND NOTICE THE SAME ISSUE. IN MY OPINION, THERE SHOULD BE A MODIFICATION TO THIS FEATURE TO PREVENT THIS ISSUE. ESPECIALLY IS THERE ARE KIDS NEAR THE CAR.

WIRING HARNESS THAT CONTROLS NORMAL 2WD/4WD HAS BEEN CHEWED BY RODENTS LEAVING VEHICLE STUCK IN 4WD. VEHICLE WAS PARKED ON GARAGE APRON WHEN WIRES WERE CHEWED.
PURCHASE NEW OFF LOT. VEHICLE BARELY USED. 2012 ONLY HAS SLIGHTLY OVER 26,000 ORIGINAL MILES. KEPT COVERED WHEN NOT IN USE. PAINT IS JUST POPPING ON IN SEVERAL LOCATIONS RIGHT DOWN TO THE PRIMER LAYER. STARTED NEAR BACK WINDOW, THEN WHEEL WELL AREA, ROOF SPOTS. WE HAVE PUT TOUCH UP PAINT ON THE BACK WINDOW THINKING WE COULD SLOW THE PROCESS BUT IT KEEPS GETTING BIGGER. NO ONE SEEMS TO HAVE AN ANSWER OTHER THAN THERE ARE A LOT OF COMPLAINTS FOR THE SAME ISSUE. ALL WILLING TO GIVE ESTIMATES FOR ME TO PAY FOR IT. OBVIOUSLY IT IS A PROBLEM THAT TOYOTA SHOULD BE PAYING FOR!!!!! THIS VEHICLE WAS TO BE MY LAST ONE AT MY AGE. I NEED ANSWERS
12/21/15 - MY VEHICLE WAS STATIONARY. I JUST FINISHED REFUELING AND STARTED THE CAR. I HAD MY RIGHT FOOT ON THE BRAKE, AND SWITCHED FROM PARK TO DRIVE. MY LEFT FOOT STAYED ON THE FLOOR, TO THE LEFT OF THE BRAKE PEDAL. WITHOUT REMOVING MY RIGHT FOOT FROM THE BRAKE, THE ENGINE REVVED AND THE CAR LURCHED FORWARD. I PRESSED EXTREMELY HARD ON THE BRAKE TO KEEP THE CAR FROM MOVING FORWARD, AND COULD HEAR AND FEEL THE ENGINE AND BRAKE FIGHTING EACH OTHER. I WAS TRAVELLING NORTHBOUND ON I-85 IN THE CITY OF ATLANTA AT AROUND 55 MILES PER HOUR. A CAR FROM THE HOV LANE ABRUPTLY CHANGED LANES AT HIGH SPEED AND HIT ME ON THE DRIVERS SIDE. THIS WAS A BIG IMPACT AND DUE TO THIS MY CAR SWERVED TO THE RIGHT LANE AND HIT ANOTHER MOVING TRUCK ON ITS DRIVERS SIDE. THE CAR HAS BEEN TOTALED BY MY INSURANCE COMPANY. MY CONCERN IS THAT, DUE TO THE NATURE OF THE ACCIDENT AND THE IMPACT THE AIR BAGS OF THE CAR SHOULD HAVE DEPLOYED AND THEY DIDN'T. THIS I THINK IS A LAPSE IN THE SAFETY OF THE CAR AND IS A MATTER OF LIFE AND DEATH. THE ACCIDENT TOOK PLACE ON 10/17/2015 AROUND 7:30 AM ON I-85N IN THE CITY OF ATLANTA. I HAVE ATTACHED THE POLICE REPORT FOR REFERENCE. PLEASE INVESTIGATE THE SAME.
ON OCTOBER 17 SHORTLY BEFORE 2 PM PST IN CLEAR WEATHER, I WAS PULLING INTO A PARKING SPOT ON PRIVATE PROPERTY AT ABOUT 2 TO 3 MPH. I TURNED THE VEHICLE INTO THE SPOT AND APPLIED THE BRAKES BUT THE CAR SURGED OVER THE CURB, HIT A TREE, WENT OVER AN EMBANKMENT AND CRASHED ONTO A PARKED CAR. (THE AIRBAG DID NOT DEPLOY, BUT THERE WERE NO INJURIES.) THE IGNITION WOULD NOT TURN OFF AFTER REPEATED ATTEMPTS. FINALLY I DEPRESSED THE IGNITION BUTTON FOR ABOUT 10 SECONDS AND IT TURNED OFF. JUST 5 DAYS EARLIER, THE CAR HAD UNDERGONE A SAFETY RECALL REPAIR TO THE ELECTRONIC CIRCUITRY. TOYOTA HAD ISSUED A SAFETY RECALL DUE TO A SOFTWARE DEFECT WHICH AFFECTED "COMPONENTS IN THE ELECTRIC VEHICLE TRACTION MOTOR ASSEMBLY." WE INFORMED TOYOTA THIS APPEARED TO BE AN UNINTENDED ACCELERATION INCIDENT. THEY PROMISED TO ASSIGN AN ENGINEER TO THOROUGHLY INSPECT THE VEHICLE AND DOWNLOAD INFORMATION FROM THE EVENT DATA RECORDER (EDR). THE ENGINEER INSPECTED THE CAR AND PREPARED A 22 PAGE REPORT. HOWEVER HE WAS UNABLE TO DOWNLOAD THE EDR INFORMATION DUE TO A DATA COMMUNICATION ERROR. TOYOTA CLAIMED IT WAS DUE TO A LOW BATTERY. THE ENGINEER CONFIRMED TO ME THAT IT WAS A DATA COMMUNICATION ERROR AND THAT THE VEHICLE WAS HALF CHARGED WHEN HE DROVE IT. TOYOTA REFUSED TO SHARE THE ENGINEER'S REPORT. THEY STATED IN SIX SHORT SENTENCES THAT THE ACCELERATION AND BRAKE PEDALS BOTH WORKED. THEY DID NOT ADDRESS THE RECALL, THE INCIDENT WITH THE IGNITION SWITCH, OR THE DATA COMMUNICATION ERROR, ALL OF WHICH SIGNAL POSSIBLE PROBLEMS WITH THE ELECTRIC CIRCUITRY/SOFTWARE. WHEN WE WROTE THEM A DETAILED LETTER WITH ALL THE ATTACHMENTS TO DOCUMENT THE RECALL, THEY SIMPLY RESPONDED THAT THE ACCELERATION PEDAL AND BRAKE PEDAL WORKED.

RECEIVED NHTSA RECALL FROM TOYOTA IN SEPT. 2015. RECALL INVOLVES WINDSHIELD WIPERS WHICH MAY BECOME INOPERATIVE. HAVE CALLED DEALER NUMEROUS TIMES UP TO THIS DATE. I AM TOLD THEY HAVE NOT RECEIVED REPLACEMENT PARTS TO COMPLETE REPAIR. I FEEL THEY HAVE BEEN GIVEN PLENTY OF TIME TO REMEDY A SITUATION WHICH COULD ENDANGER DRIVING SAFETY. *CN
I RECEIVED A RECALL NOTICE REGARDING WATER DRIPPING ONTO THE WINDSHIELD WIPER MOTOR LINK. OVER TIME THIS PROBLEM CAN CAUSE CORROSION & WEAR @ THE WIPER MOTOR LINK JOINT RESULTING IN SEPARATION OF THE WIPER LINK FROM THE WIPER MOTOR CRANK ARM. RECALL DATE WAS 9-17-15 (OVER A YEAR). I WOULD LIKE TO HAVE THIS REMEDIED IMMEDIATELY TO AVOID LOSS OF MY WIPER AND A CORROSION PROBLEM.
