The contact owns a 2002 Chevrolet Silverado 1500. The contact stated while driving 5-10 MPH, the airbag warning light illuminated. The independent mechanic was made aware of the failure and referred the contact to the dealer for diagnosis. The dealer and the manufacturer were not notified of the failure. The vehicle was not diagnosed nor repaired. The approximate failure mileage was 143,725.
TL* THE CONTACT OWNS A 2002 CHEVROLET SILVERADO 1500. THE CONTACT STATED THAT ANOTHER VEHICLE IN FRONT WAS MAKING A LEFT, THE DRIVER SLAMMED ON THE BRAKE PEDAL BUT THE LATCH FAILED CAUSING THE PASSENGER TO GO THROUGH THE WINDSHIELD DURING THE IMPACT. THE DRIVER PROCEEDED TO HIT A THIRD VEHICLE THAT WAS AT A STOP SIGN. THE OTHER DRIVER WAS FOUND AT FAULT BY THE POLICE. THE FRONT PASSENGER SEAT BELT LATCH FAILED TO RESTRAIN AND CAUSED THE FRONT PASSENGER INJURIES AT THE FACE, ARM, AND LEFT WRIST. THE FRONT PASSENGER AIR BAG FAILED TO FULLY DEPLOY AND CAUSED THE PASSENGERS HEAD HIT THE WINDSHIELD. THE DRIVER SUFFERED INJURIES TO THE STOMACH. MEDICAL ATTENTION WAS PROVIDED TO BOTH INJURED. THE POLICE DEPARTMENT WAS PRESENT AND A POLICE REPORT WAS FILED. THE MANUFACTURER WAS NOTIFIED OF THE FAILURE. THE CONTACT ALSO WANTED TO ADD THAT TSB SEAT 10-08-50-003B DESCRIBES THE SEAT BELT FAILURE. THE VEHICLE WAS TOWED TO THE DRIVER'S RESIDENCE. THE FAILURE MILEAGE WAS NOT AVAILABLE.
VEHICLE WAS TRAVELING DOWN A STATE HIGHWAY WHEN IT WENT OFF THE ROAD, DOWN A SHORT EMBANKMENT, HIT AT LEAST 2 TREES BEFORE COMING TO REST. THE COLLISION WAS A FRONTAL COLLISION AND THE VEHICLE WAS A TOTAL LOSS. THE AIRBAG, HOWEVER, DID NOT DEPLOY DURING THE ACCIEENT.
ON THURSDAY, AUGUST 6, 2015 AT APPROXIMATELY 1:20 PM NATCHEZ RAILWAY EMPLOYEES [XXX] WERE ATTEMPTING TO START VEHICLE #1828 WHICH WAS BEING STORED ON THE GROUNDS OF THE NATCHEZ DEPOT. VEHICLE #1828 IS A 2002 CHEVROLET 2500 PICKUP TRUCK WITH VIN #[XXX] AND HAD BEEN DRIVEN FROM THE GRENADA RAILWAY APPROXIMATELY THREE WEEKS PRIOR. [XXX] WAS SITTING IN THE DRIVER'S SEAT AND THE BATTERY APPEARED TO BE DEAD. WHEN HE PULLED THE HOOD RELEASE LATCH, THE PASSENGER SIDE AIR BAG ACTIVATED WITH A LARGE BANG AND THE CAB OF THE TRUCK WAS FILLED WITH A WHITE POWDER. [XXX] SUFFERED A SLIGHT RINGING IN THE EARS FROM THE NOISE. A CHECK WITH THE GENERAL MOTORS WEBSITE INDICATES THIS VEHICLE IS NOT SUBJECT TO ANY RECALLS, ESPECIALLY FOR DEFECTIVE AIRBAGS. AT THE TIME OF THE INCIDENT, THE AIR TEMPERATURE WAS APPROXIMATELY 95⁰ F WITH 60% HUMIDITY. THE VEHICLE HAS BEEN PARKED FOR OVER TWO WEEKS WITH THE DOORS AND WINDOWS CLOSED. TL* THE CONTACT OWNED A 2002 CHEVROLET SILVERADO 2500 HD. WHILE DRIVING APPROXIMATELY 75 MPH, A MESSAGE INDICATING THAT THE ENGINE OVERHEATED DISPLAYED AND SMOKE APPEARED UNDER THE HOOD OF THE VEHICLE. THE FLAMES QUICKLY SPREAD TO THE CABIN OF THE VEHICLE. WHILE THE FLAMES WERE INSIDE THE CABIN, THE CONTACT ATTEMPTED TO EXIT THE VEHICLE BUT THE DRIVER SIDE SEAT BELT WOULD NOT IMMEDIATELY UNLOCK. THE SEAT BELT FINALLY UNLOCKED, BUT THE DRIVER SIDE DOOR WOULD NOT UNLOCK. THE DOOR EVENTUALLY UNLOCKED AND THE CONTACT WAS ABLE TO EXIT THE VEHICLE. THE FIRE DEPARTMENT WAS CALLED TO THE SCENE TO EXTINGUISH THE FLAMES. THE VEHICLE WAS DESTROYED. THERE WERE NO INJURIES. THE MANUFACTURER WAS NOT NOTIFIED OF THE FAILURE. THE FAILURE MILEAGE WAS 247,433.
AS I WAS DRIVING AT 75 MPH, THE INDICATOR LIGHT SAID, ENGINE OVERHEATED. I THEN NOTICED WHITE SMOKE COMING FROM THE HOOD. THE WHITE SMOKE TURNED BLACK. I PULLED OVER TO THE SIDE OF THE ROAD AND BEFORE I COULD SHUT THE ENGINE OFF, THE ENGINE SHUT OFF ON ITS OWN. FIRE BEGAN COMING UP WHERE THE WINDSHIELD WIPERS ARE LOCATED AND PROCEEDED THROUGH THE DASHBOARD. I TRIED TO UNLATCH MY SEAT BELT BUT IT WOULD NOT RELEASE. AFTER SEVERAL TRIES IT RELEASED. I ATTEMPTED TO OPEN THE DOOR; HOWEVER, I HAD THE SAME PROBLEM. THE DOOR DID NOT OPEN. AFTER BUMPING THE DOOR FORCIBLY MANY TIMES, IT ALSO CAME OPEN. MIND YOU, ALL THE WHILE THE FLAMES ARE COMING INSIDE THE CAB. I CALLED 911 AND THE COUNTY FIRE DEPARTMENT WAS DISPATCHED, BUT NOT BEFORE IT WAS FULLY ENGULFED IN FLAMES. THE VEHICLE WAS WELL MAINTAINED. I WANT TO KNOW WHAT STEPS TO TAKE TO PREVENT THIS FROM EVER HAPPENING AGAIN?

I COULD FEEL THE ABS ENGAGE BUT NOT STOP HITTING A VEHICLE IN FRONT OF ME. WHEN I EXITED THE VEHICLE I COULD SMELL THE BRAKE FLUID AND OBSERVED FLUID LEAKING FROM MULTIPLE LOCATIONS. AFTER EXITING THE OFF RAMP OF I264 IN PORTSMOUTH, VIRGINIA I SPED UP TO APPROXIMATELY 35 MPH AND APPROACHED THE NEXT STOP LIGHT. UPON APPLYING PRESSURE TO THE BRAKE PEDAL NO RESISTANCE WAS NOTED AND THE SLOWING OF THE VEHICLE WAS FOUND NEGLIGIBLE. I REAR ENDED A VEHICLE IN FRONT OF ME AT APPROXIMATELY 25 MPH. BOTH CARS INVOLVED IN THE ACCIDENT WERE PULLED INTO THE MEDIAN OUT OF TRAFFIC. AS I EXITED THE VEHICLE I NOTICED FLUID DRIPPING FROM THE TRUCK FRAME AT THE FORWARD HINGE OF THE DRIVER SIDE DOOR. THE DRIPPING FLUID WAS BRAKE FLUID. I SUSPECT A RUPTURED BRAKE FLUID LINE OR TUBE HAD RUPTURED WHICH CAUSED THE LOSS OF BRAKING POWER. THERE WAS NO WARNING GIVEN BY THE VEHICLE'S COMPUTER TO WARN OF LOW BRAKE FLUID BECAUSE THE RESERVOIR WAS STILL AT THE NORMAL LEVEL. I BELIEVE CORROSION OF THE BRAKE LINES WAS ACCELERATED DUE TO THE USE OF SALT TREATMENTS ON THE ROADS DURING THE WINTER SNOW STORMS THAT HAVE FALLEN IN THE PAST THREE YEARS. LUCKILY NOBODY WAS SERIOUSLY HURT AND LUCKILY THIS DID NOT HAPPEN WHEN I WAS EXITING THE INTERSTATE AT A MUCH HIGHER RATE OF SPEED. ALSO THE AIR BAG DID NOT DEPLOY UPON IMPACT. THESE ARE TWO SERIOUS SAFETY ISSUES INCURRED IN ONCE INCIDENT TO A TRUCK WHICH HAS LESS THAN 70K MILES AND 12 YEARS OLD. *TR

SEATBELTS DOES NOT CATCH AND WILL NOT RECOIL. STARTED OUT AS LOOSE NOW IT WILL NOT RETRACT. STARTED WHEN I APPLIED THE BRAKES WHEN A DOG RAN IN FRONT OF ME AND SEATBELTS DID NOT CATCH AND MY CHEST HIT THE STEERING WHEEL. THE SEATBELTS DID NOT RETRACT AFTER IT PULL OUT. I WAS TRAVELING 55 MPH WHEN THE DOG CROSSED THE HIGHWAY.

ON SEPTEMBER 27, 2010 WHILE DRIVING A 2002 CHEVROLET SILVERADO 1500 PICKUP MY HUSBAND RAN OFF THE ROAD AND HIS TRUCK CRASHED IN A RAVINE. HIS AIRBAG DID NOT DEPLOY CAUSING HIM A SEVERE BRAIN INJURY KNOWN AS A DAI. HE SPENT 84 DAYS IN THE HOSPITAL AND HAD TO LEARN TO DO EVERYTHING AGAIN. HE MAY NEVER FULLY RECOVER FROM THIS ACCIDENT COMPLETELY. HAD HIS AIRBAG DEPLOYED I BELIEVE HIS INJURY WOULD HAVE NOT BEEN NEARLY AS SERIOUS. HE WAS DRIVING DOWN THE INTERSTATE. THE AIR BAGS FAILED TO DEPLOY AND THE SEAT BELT DID NOT RETRACT. THE DRIVER'S SIDE BUMPER WAS CRUSHED INTO THE FRONT GRILL ALL THE WAY ACROSS TO THE PASSENGER SIDE OF THE VEHICLE. THE CONTACT CALLED THE INSURANCE AGENT, BUT NO POLICE REPORT WAS FILED. HE WAS THE ONLY OCCUPANT IN THE VEHICLE AND THE SEAT BELT WAS WORN PROPERLY AT THE TIME OF THE CRASH. THERE WAS NO MAINTENANCE PERFORMED ON THE AIR BAGS OR SEAT BELT PRIOR TO THE FAILURES. THE CONTACT FILED A COMPLAINT WITH GM CONCERNING HIS AIR BAGS AND SEAT BELT (COMPLAINT NUMBER 71-670143505). GM IS NOT TAKING RESPONSIBILITY FOR THE AIR BAG FAILURE. IN ADDITION, NO ONE CAME OUT TO INSPECT THE VEHICLE. THE FAILURE MILEAGE WAS 61,752.

THE FRONT END OF THE VEHICLE SUSTAINED SEVERE DAMAGE. THE VEHICLE WAS TOWED TO A COLLISION CENTER AND THE VEHICLE WAS REPAIRED. THE CONTACT HAD CONCERNS OF THE SAFETY RISK INVOLVED. THE FAILURE MILEAGE WAS 40,000 AND THE CURRENT MILEAGE WAS 55,000.
