The contact owned a 2011 Toyota Corolla. The contact stated that while he was driving at 40 MPH, another vehicle that was speeding caused a multiple vehicle collision. During the collision, the contact was in the middle lane when the speeding vehicle crashed into a vehicle in the left lane, and that vehicle merged violently into the middle lane, causing the contact to rear-end the vehicle in front due to the driver abruptly stopping. During the attempt to stop, the vehicle failed to decelerate sufficiently. Upon impact, the frontal air bags failed to fully deploy, as they only partially emerged from the housing. The failure experienced was a loss of braking effectiveness at 40 MPH that resulted in a rear-end collision, during which the frontal air bags failed to deploy. The vehicle was towed to a family member's home. The tire pressure warning was the only warning light on at the time of the crash. The driver was injured. Injuries sustained were lower back, right side ribs, and neck pain that required medical attention. The vehicle was deemed totaled by insurance. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was 124,000.
I am submitting a safety concern regarding airbag non-deployment involving my former vehicle, a 2011 Toyota Corolla LE (VIN: [XXX] ). This vehicle was subject to multiple Takata airbag recalls, including a 2018 front airbag recall and a May 2020 Takata non-deployment and seatbelt pretensioner interim recall. Beginning in Spring 2021, the airbag warning light illuminated intermittently. On May 30, 2024, diagnostic trouble codes B1813 and B1818 were recorded, indicating faults involving the driver and passenger airbag squib circuits and potential center airbag sensor assembly involvement. A recall-related repair was completed on June 6, 2024 with installation of a Harness Kit (04009-98112) under extended warranty associated with a class action settlement. Following subsequent service, the airbag warning light was no longer illuminated. After these repairs, the vehicle was later involved in a rear-end collision in which the airbags did not deploy. The vehicle was declared a total loss and is no longer in my possession. Given the vehicle’s documented history of airbag system recalls and prior SRS fault codes, I am submitting this incident for safety documentation and review. p195 /65r/15 91 h. yokohama avid ascend. i bought 4 tires on 04/11/22 at kinderhook toyota my car had 61008 miles on it. on 08/04/25 brought my car to same dealer was told i needed new tires. multipoint inspection said front tires had 3 32nd of tread and back tires had 4 32nd of tread wear left. my car had 80678 miles on it. the tires lasted less than 20000 miles. they had an 85000 mile warranty on them. i can understand tires lasting maybe 60000 miles or so, but only 20000 miles? was told i can contact your office about this. could not find a different page for a non- accident problem. dealer said to contact yokohama customer service and kinderhook toyota would send the tires back.

Part of a customer support repair program through Toyota. It expired, and Toyota Customer Care at 1-888-270-9371 told me to file a complaint at NHTSA.gov. So I am filing a complaint. The paint is peeling on my car rapidly and I have missed the support program deadline because it was not happening in the timeframe they created for this problem. But there is no reason for my pain to be peeling like it is other than manufacture error.

I had an accident because a dog ran away on a road and hit my car in front and I was going at a speed of approximately 20 mph, breaking my front bumper and the airbags did not activate.
While driving on a straight road steering wheel suddenly jerk hard left and right several times for just a few seconds and then went normal. This caused the car to cross lane into oncoming traffic. Fortunately steering went to normal before causing head on collision. I took the car into local Toyota dealer to be checked. They could not duplicate trouble. No warning lamps lit up and no fault codes per dealership technician.
While driving my car seat seat (driver side) starts to over heat. I do not have heated car seats.but within the past few weeks I have noticed that my car seat started to heat up. At first I thought it was because it has been sitting in the sun all day, but as the driving progressed in the fact this is happened three times I feel that there is a flaw In the interior.
Fail to pass Texas vehicle safety & emissions inspection.
Sunroof exploding while driving. Could cause safety issues if inner compartment was opened when the incident occurred. The vehicle has not been inspected yet but I will have a glass company replace the sunroof asap. No symptoms of any fault was present before the incident occurred.

My Transmission On My 2011 Toyota Corolla Has Been Experiencing So Troubles .when being put into gear to drive it acts as if it's in Neutral and put on reverse the same thing . At first it was driving fine and Then a week later when you put it on driver I notice my rpms when getting ramped up but my it was slowly going up to 20 miles pre hour and you can fill it slipping. Now my car is undriveable

Sunroof suddenly exploded while driving 40 MPH on a highway No evidence of projectile was in the car. The glass was volcanoed up. It was around 50 degrees outside. I was immediately confused as to what happened. I was traveling in a merging lane and could have vered into the other lane and hit another car. Glass was Falling down onto me and the passenger seat. No police were called I took the vehicle to a glass repair. Insurance was not involved as my dectiable would not have been meet for the cost of the repairs. No warning or any symptoms the sunroof glass from the car. Itjust suddenly exploded for no reason.

CAR ACCELERATES HARD FROM STATIONARY IDLE. HAPPENING FOR SEVERAL WEEKS. YESTERDAY ACCELERATED FROM IDLE AT A 4 WAY STOP INTERSECTION ALMOST CAUSED AN ACCIDENT. VERY SCARY. NOT COMFORTABLE USING IT UNTIL PROBLEM REMEDIED.

IT WILL ALSO SLOWLY CHANGE WHEN CRUISING AT A CONSTANT SPEED, THE CAR WILL INCREASE SPEED GRADUALLY WITH VERY LIGHT PRESSURE ON IT. 2. OUR CRUISE CONTROL WILL RESET THE SPEED AT VARIOUS TIMES, WE WILL BE GOING 70 MPH ON THE INTERSTATE AND IT WILL GRADUALLY SLOW DOWN WITHOUT US DISABLING IT WITH A BRAKE PRESS OR SWITCHING IT OFF. (MY WIFE DOES NOT USE THE CRUISE CONTROL AND CANNOT VERIFY THIS.) 3. OUR RADIO WILL TURN ON SUDDENLY WITHOUT PRESSING THE POWER BUTTON. 4. OUR RADIO WILL EJECT A CD WITHOUT US HAVING THE RADIO ON. 5. OUR AIR CONDITIONER LIGHT WILL TURN ON AND OFF INTERMITTENTLY WITHOUT US TURNING IT OFF. (I CANNOT VERIFY THIS ALTHOUGH MY WIFE HAS OBSERVED THIS BEHAVIOUR.) 6. CALLED THE DEALERSHIP THAT WE BOUGHT THE CAR FROM (PRIME FORD SACO MAINE) AND THEY TOLD ME THE RECALL WAS TAKEN CARE OF AND WE WERE ALL SET. COME TO FIND OUT, IT WASNT COMPLETED AND MY SON HAS BEEN DRIVING AROUND IN AN UNSAFE VEHICLE.

A POLICE REPORT WAS FILED. THE VEHICLE WAS UNABLE TO BE DRIVEN AND WAS TOWED TO A TOW LOT. THE VEHICLE WAS NOT DIAGNOSED AND WAS DESTROYED. A DEALER WAS NOT CONTACTED. THE CONTACT STATED THAT SHE RECEIVED AN UNKNOWN RECALL NOTICE REGARDING THE AIR BAGS IN 2015. ON OCTOBER 6, 2017, THE VEHICLE WAS SERVICED PER AN UNKNOWN AIR BAG RECALL. WHEN DRIVING ON INTERSTATE ABOVE 55MPH, STEERING SEEMS TO GET DRIFTY. FEELS UNSAFE. VERY SCARY. WE HAVE HAD NEW TIRES INSTALLED AND ALIGNMENT CHECKED. NO SEMIS AROUND US OR HIGH WINDS. TALKED TO TOYOTA DEALERSHIP IN MUSCATINE IA. THEY SAID THERE WERE NO KNOW ISSUES. WE WERE WILLING TO PAY FOR 1 HOUR OF TROUBLE SHOOTING IF THERE WAS A FIX. DEALERSHIP STATES THERE IS NO KNOW PROBLEM. WE HAVE NOTICED PROBLEM FOR MORE THEN 5 MONTHS, IT IS BECOMING MORE PRONOUNCED AND UNSAFE. THIS MY WIFE'S PRIMARY CAR.
TRANSMISSION: MY 2011 TOYOTA MATRIX JERKS AND ACCELERATES SUDDENLY WITH A REVVING NOISE AND MAKING CHOKING SOUNDS LIKE GEARS SHIFTING IN A MANUAL TRANSMISSION CAR. THIS HAPPENS EACH TIME I SLOW DOWN TO MAKE A TURN (LEFT OR RIGHT) OR MERGE INTO THE FREEWAY, WHEN I SLOW DOWN AT A TRAFFIC LIGHT AND TRIES TO TAKE OFF AS THE LIGHT TURNS GREEN, AND WHEN I SLOW DOWN AND TRY CHANGING LANE TO PASS ANOTHER VEHICLE. I HAVE HAD TO PRESS HARD ON THE BREAKS TO AVOID HITTING ANOTHER VEHICLE IN FRONT OF ME. CALLED TOYOTA SOUTH ATLANTA (6865 JONESBORO RD, MORROW, GA 30260 | (770) 961-0225) TO MAKE AN APPOINTMENT ON 7/2 FOR THE AIRBAG RECALL, SAID WILL SEND A MESSAGE TO THE RECALL SPECIALIST, THEY WILL CHECK INVENTORY STATUS, THEN GIVE ME A CALL BACK TO MAKE APPOINTMENTS. NO RETURNED CALL. 7/12 CALLED ONCE MORE AND RECEPTIONIST SAID THE E-MAIL WAS SENT ONCE MORE. "DUE TO THE LARGE AMOUNT OF E-MAILS, RESPONSE MAY BE SLOW", SO I WAITED. 7/23 CALLED ONCE MORE, THE RECALL SPECIALIST WASN'T IN AND WAS TOLD TO CALL BACK THE NEXT DAY. 7/24 CALLED ONE LAST TIME AND ENDED UP TALKING TO THE REGIONAL MANAGER, WHO SAID WOULD FIND OUT THE SITUATION AND CALL ME BACK BUT NEVER DID. I AM STARTING TO THINK THAT TOYOTA SOUTH ATLANTA IS JUST USING THIS METHOD TO AVOID ANY WARRANTY REPAIRS WITHOUT HAVING TO VERBALLY REJECT THE REPAIRS... THE CONTACT RECEIVED THE RECALL NOTIFICATION IN 2017, WHICH INDICATED THAT PARTS WERE UNAVAILABLE. THE DEALER AND MANUFACTURER WERE NOT CONTACTED. THE CONTACT STATED THAT THE VEHICLE WAS REAR ENDED AND THE DRIVER'S SIDE AIR BAGS DID NOT DEPLOY. A POLICE REPORT WAS FILED AND THERE WERE NO INJURIES. THE VEHICLE WAS DESTROYED AND TOWED. THE FAILURE MILEAGE WAS 106,000. PARTS DISTRIBUTION DISCONNECT.
I was notified of the air bag recall a year ago but the car dealerships in my area does not have the parts and I need to use my vehicle daily. So I am not sure what to do from here for that but I believe that the appropriate time to remedy this recall has past already and the fact that we have our children riding with us and we are unaware that their life is in danger or because we can not get it fixed is not acceptable. Then on top of that I am having issues like when my 2011 Corolla is sitting idle it will rev up the engine without touching the gas pedal and does this randomly and without warning and also while I am driving around 55 MPH on the interstate or highway sometimes it will dramatically drop and lose about 10- 15 MPH speed for no reason and this happens randomly as well. And this fluctuation happens during cruise control the most so I no longer even use that function. Also both headlights have moisture in them and are so fogged up now that visibility is almost impossible at night now. Also the weather striping around all my windows have dry rotted and broke off and there is practically nothing left of stripping anymore. Then the center dash above the radio just broke and gave out under my phone holder while driving, it broke into about 15 different pieces. I know that my phone or the holder is even close to heavy enough to cause the dash panel to cave in like that. I called to get a price on the dash and it was like $250+ and who can afford that for a piece of plastic. Lastly my driver and passenger seat will get stuck if you try adjusting them and sometimes it will let you move the seat but to do so you have to hop up and down for several minutes and throughout the adjustment to get the seat to move in any direction.
THE ACCIDENT HAPPENED IN THE HOUSE-ATTACHED GARAGE. I ALREADY ENTERED THE GARAGE, AND THE CAR IS ALMOST STOPPED. THEN I NEED TO HIT THE BRAKE THOROUGHLY TO FULLY STOP THE CAR. I HIT THE BRAKE PEDAL HOWEVER THE CAR ACCELERATED SUDDENLY, JUMPED AHEAD AND CRASH INTO THE HOUSE WALL. EVEN THOUGH EVERYTHING HAPPENED IN A SECOND, I'M SURE THAT IT IT THE LARGER PEDAL(WHICH IS THE BRAKE) I ENGAGED WITH IN THAT SECOND. I HAVE DRIVEN THIS CAR FOR 3 YEARS, AND PARKED IN THIS GARAGE FOR MONTHS. SO I'M PRETTY FAMILIAR WITH THE CAR AND THE PLACE. LUCKILY, THERE IS NO PEOPLE GOT INJURED, AND THE AIR BAG DIDN'T EXPLODE. THE WALL WAS HIT SIGNIFICANTLY, AND THE FRONT OF MY CAR WAS DAMAGED.

I WAS ON A DECLINE ON THE OFF RAMP, AND WHEN I WENT TO STOP FOR A LITE, MY BRAKES WOULD NOT STOP MY CAR. I TRIED TO STOP, AND FELT AS IF THE CAR SPED UP. I HAD COMPLAINED TO THE DEALER AND HAD THEM CHECK MY BREAKS FOUR TIMES BECAUSE OF A STOPPING ISSUE. SHORTLY BEFORE MY CRASH MY BREAKS WOULD SCREECH, AND I THOUGHT IT TO BE DURT ON MY BREAKS, OR BREAK DUST. I HAD TAKEN THE CAR TO A RECOMMENDED BREAK SHOP TO GET MY BREAKS CHECKED OUT. THEY SAID THE BREAK PADS, RODERS, CALIPERS ETC WERE IN GOOD CONDITION WITH ONLY ONE OR TWO ISSUES. I ASKED THEM TO REPLACE THE RODERS, AND BREAK PADS WITH OEM PARTS. THE ISSUE NOW SITS WITH THE BREAKS BEING DEPRESSED AS HARD AS ONE CAN DEPRESS THEM, ONLY TO HAVE THE CAR NOT STOP, AND FEEL LIKE IT WAS SPEEDING UP. AT THE TIME OF THE CRASH, YOU COULD HEAR THE MOTOAR SPEEDING UP EVEN WITH MY FOOT ONLY ON THE BREAK AND CLEARLY THE CAR WOULD NOT STOP.

YOUR WEBSITE STATES THAT IF THE AIRBAG, DEEMED DEFECTIVE, WERE TO DEPLOY, WOULD CAUSE HARM TO THE OCCUPANTS OF THE CAR. DOES THIS MEAN THAT IF THE AIRBAG ON THE DRIVERS SIDE, IS NOT DEFECTIVE, AND THUS, ON IMPACT, WOULD NOT SHOOT DEADLY SCHRAPNEL ONTO THE OCCUPANT????? SOMEHOW, IT SEEMS THAT IF THE PASSENGER SIDE IS DEFECTIVE, WOULD NOT THE DRIVERS SIDE AIRBAG ALSO BE DEFECTIVE?
