Toyota Motor Engineering & Manufacturing (Toyota) notified the agency on May 13, 2015 that they are recalling certain model year 2004 Sequoia vehicles manufactured August 18, 2003, to August 19, 2004, and 2003-2004 Tundra vehicles manufactured March 18, 2003, to August 25, 2004. On June 16, 2015, Toyota expanded the recall to include an additional 1,270,468 vehicles, including model year 2002-2003 and 2005-2007 Sequoia, 2005-2006 Tundra, 2003-2007 Corolla, 2003-2007 Corolla Matrix, 2003-2007 Pontiac Vibe and 2002-2007 Lexus SC vehicles. This is to address a safety defect in the passenger side frontal air bag inflator which may produce excessive internal pressure causing the inflator to rupture upon deployment of the air bag. This recall addresses both the passenger side frontal air bags that were originally installed in the vehicles, as well as replacement air bags that may have been installed as replacement service parts. A replacement air bag may have been installed, as one example, if a vehicle had been in a crash necessitating the replacement of the passenger side frontal air bag.
Remedy Status
2003-2004 Corolla and Corolla Matrix vehicles that are recalled under both 15V-043 and 15V-285 are under a Do Not Drive advisory. Owners of these vehicles are advised not to drive their vehicles until the recall remedy has been completed. Toyota will notify their owners. Pontiac Vibe owners will be notified by GM. Dealers will replace the front passenger air bag. The recall began on July 13, 2015. Toyota owners may contact Toyota customer service at 1-800-331-4331. Pontiac owners may call 1-800-762-2737. NOTE: This recall supersedes recall 14V-312 in its entirety.
Toyota Motor Engineering & Manufacturing (Toyota) notified the agency on May 13, 2015, that they are recalling 2005-2007 Lexus SC, Toyota Corolla, Corolla Matrix, Sequoia, and Pontiac Vibe, and 2005-2006 Toyota Tundra vehicles that were originally sold, or ever registered, in Florida, along the Gulf Coast, Puerto Rico, Hawaii, the U.S. Virgin Islands, Guam, Saipan and American Samoa. On June 16, 2015 Toyota expanded the recall to cover an additional 769,055 vehicles, including certain model year 2002-2004 Lexus SC, Toyota Sequoia, 2003-2004 Toyota Corolla Matrix, Toyota Corolla, Pontiac Vibe, and Toyota Tundra vehicles, as well the vehicles covered by the May 13, 2015 notification in the portions of Georgia, Alabama, Louisiana, Mississippi and Texas that were not previously included. The inflators in the front passenger air bags are susceptible to rupture in the event of a crash necessitating the deployment of the front passenger air bag.
Remedy Status
2003-2004 Corolla and Corolla Matrix vehicles that are recalled under both 15V-043 and 15V-286 are under a Do Not Drive advisory. TOYOTA IS RECALLING CERTAIN MODEL YEAR 2005-2008 COROLLA AND COROLLA MATRIX VEHICLES. THE ENGINE CONTROL MODULE (ECM) FOR THE SUBJECT MODELS EQUIPPED WITH THE 1ZZ-FE ENGINE AND TWO-WHEEL DRIVE MAY HAVE BEEN IMPROPERLY MANUFACTURED. THERE IS A POSSIBILITY THAT A CRACK MAY DEVELOP AT CERTAIN SOLDER POINTS OR ON VARISTORS ON THE CIRCUIT BOARD.
Remedy Status
DEALERS WILL INSPECT THE PRODUCTION NUMBER OF THE ECM AND REPLACE THE ECM IF NECESSARY. THIS SERVICE WILL BE PERFORMED FREE OF CHARGE. TOYOTA WILL MAIL AN INTERIM OWNER NOTIFICATION THE MIDDLE OF SEPTEMBER 2010 TO ADVISE OWNERS OF THIS DEFECT AND RECALL. THIS INTERIM NOTIFICATION WILL INFORM OWNERS THAT THEY WILL RECEIVE A FUTURE NOTIFICATION WHEN PARTS BECOME AVAILABLE TO HAVE THE FREE REMEDY PERFORMED ON THEIR VEHICLE. Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2010-2016 4Runner, 2003-2006 Tundra, 2003-2013 Corolla, 2009-2010 Corolla Matrix, 2004-2005 RAV4, 2002-2007 Sequoia, 2011-2013 Sienna, 2008-2012 Scion xB, 2008-2009 Lexus IS-F, 2007-2012 Yaris and Lexus ES350, 2010-2017 Lexus GX460, 2002-2010 Lexus SC430, 2006-2012 Lexus IS250 and IS350 and 2010-2015 Lexus IS250C and IS350C vehicles. These vehicles had their driver or passenger frontal air bag inflators previously replaced under a prior recall using inflators of the same design. The inflators may explode due to propellant degradation occurring after long-term exposure to high absolute humidity, temperature and temperature cycling.
Remedy Status
Toyota will notify owners. Depending on the vehicle model, dealers will replace the front driver inflator, front passenger airbag inflator, the airbag assembly, or the front passenger airbag sub-assembly and inflator. The recall began November 20, 2019. Owners may contact Toyota customer service at 1-888-270-9371 or Lexus customer service at 1-800-255-3987. Toyota's numbers for this recall are J0A, J0B, and J0C. Lexus' numbers for this recall are JLI, JLJ, and JLK.

The air bag may not unfold as designed during inflation under high temperature conditions, possibly resulting in the air bag not inflating properly.

The contact owns a 2006 Toyota Corolla. The contact's son was driving at 55 MPH, another vehicle ran a stop sign and pulled into his path, resulting in a frontal collision with the side of the other vehicle. The contact’s vehicle spun around and came to rest on the shoulder of the road. Despite the high-speed frontal impact, the air bags failed to deploy. A police report was filed, and the vehicle was towed to an auto body shop; it has not been repaired. The contact's son sustained a bruised knee, neck and back pain, and a headache, for which he was treated with Tylenol. The contact referenced NHTSA ID Number: 19V741000(AIR BAGS), recall repair was done a year before. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was 265,000.
Brought vehicle in three times, airbag was nothing at time, now the airbag lights all the time
Unlatching & False Latching back seat located in the middle of the seat. Incident was discovered after recent purchased from my little sister who also just got the vehicle as well and decided to sell it to me without being aware of the defective unlatching and false Latching buckle.
The contact owns a 2006 Toyota Corolla. The contact stated that while driving at an undisclosed speed, the air bag warning light illuminated. The dealer was made aware of the failure; and the contact was informed that there were no open recalls associated with the VIN. The vehicle was not diagnosed or repaired. The contact referenced an unknown recall with a similar failure; however, the VIN was not included in the recall. The manufacturer was not made aware of the failure. The failure mileage was approximately 145,000.
On my way to work when the I hit the brakes... brakes locked up causing me to be unable to come to a complete stop.. Driver side airbag deployed but the passenger side airbag caused the dash to explode causing a fire in the dash. With the dash exploding and the passenger side airbag not deploying correctly it cause the passenger to hit windshield! Unknown Unknown Just a check engine light for knock sensor.
The contact owns a 2006 Toyota Corolla Matrix. The contact stated while driving 45 MPH, the check engine warning light and the battery warning light illuminated, and the vehicle lost motive power. The contact was able to restart the vehicle and pull over on the side of the road safely where the vehicle shut off again and failed to restart. The vehicle was towed to an independent mechanic who stated that the vehicle had experienced a similar failure as in NHTSA Campaign Number: 10V384000 (Engine and Engine Cooling) and advised to take the vehicle to the dealer. The dealer was not contacted. The vehicle was not repaired. The manufacturer was not contacted. The failure mileage was 200,025.
Inflator Rupture - front passenger inflator. A vehicle collision at Alma School and 8th Avenue where one vehicle turned left in front of the oncoming involved 2006 Toyota Corolla resulting in the Corolla striking the right side of the other vehicle. During the collision the airbags were commanded to deploy and the passenger inflator had a significant rupture/failure which sent shrapnel out of the dash, striking the passenger. Our unit has investigated two confirmed Takata inflator fatalities and after a brief visual inspection it was confirmed the passenger inflator is in several pieces. The appearance of the inflator is consistent with prior observed passenger side Takata inflators. The car has been preserved in our parking lot and the passenger side airbag, with related tears and shrapnel holes, was obtained and preserved as well. The crash was filmed on intersection CCTV and the inflator can be seen expelling fragments outwards. A Dropbox file has been created to exchange photos and video upon request. Thank you. I am a Detective with the Mesa Police Department Vehicular Crimes Unit and was made aware of NHTSA's interest in identifying inflator related cases.
Air bag light has been on for awhile and now the horn is not working. When the horn stopped working I took it in for repair again. Toyota repair department says it's a failure of the spiral cable. I found there was a recall to replace the spiral cable in Toyota Corolla Matrix vehicles but 2009 - 2010 year models, not my year of 2006. The documentation I found was very clear in stating it was going to do a recall not because of a manufacturing defect but because of a design issue. I think my vehicle should have been included in the recall because it probably has the same or similar design.
On March 17, 2023 at approximately 4:12 PM on Hwy 288 northbound just south of Southmore Boulevard I was struck from behind by a Blue Jetta. Skidding into me I think he was still going about 30 miles per hour if not a little more based on the violence of the crash. My vehicle was pushed forward into the car ahead of me damaging the front of my vehicle as well. Despite the impact, my air bags did NOT deploy. I had taken the vehicle in to the Toyota shop here in Humble, TX and had the "final fix" done on the airbags. I have been physically injured as a result of this automobile accident. I have filed with my Progressive Claim Adjuster and they are talking about totaling the vehicle. Policy Number 46753320. My concern is, despite the vehicle air bags being replaced, they did not deploy. The force of the impact should have been sufficient to activate them. It could of course be a defective switch given her age, but I'd rather let science do the talking.
Acccelerator gives no response when pressed without warning Unknown
THE SEATBELT IN THE REAR PASSENGER DOESN'T FUNCTION PROPERLY, IT EITHER GETS STUCK IN TRYING TO FASTEN IT OR ONCE IT'S ON IT DOESN'T FLUCTUATE AND GETS STUCK IN A TIGHT POSITION WHERE IT HAS TO BE RELEASED IN ORDER NOT TO OVERLY CONSTRAINT THE PASSENGER. THE KIDS ARE THE ONES SITTING IN THIS SEAT AND USING SEAT BELT SO IT'S CONCERNING FOR THEM NOT TO BE ABLE TO ABIDE BY STATE LAWS OF WEARING SEAT BELTS AND KEEPING THEM SAFE BECAUSE THEY WEREN'T ABLE TO RETRACT THE SEAT BELT.
TL* THE CONTACT'S GIRLFRIEND OWNS A 2006 TOYOTA MATRIX. THE CONTACT STATED THAT WHILE HIS GIRLFRIEND WAS DRIVING 25 MPH, HER VEHICLE WAS HIT BY ANOTHER MOTORIST AT A LOW RATE OF SPEED. DESPITE MINIMAL IMPACT, THE PASSENGER AIR BAG DEPLOYED WITHOUT AN OCCUPANT IN THE SEAT. DUE TO THE SHOCK OF THE AIR BAG DEPLOYMENT, THE CONTACT'S GIRLFRIEND JERKED THE WHEEL AND HER BACK PASSENGER TIRE HAD HIT A CURB. A POLICE REPORT WAS FILED AND THE CONTACT'S GIRLFRIEND WAS ABLE TO DRIVE THE VEHICLE HOME. THE NEXT DAY, HIS GIRLFRIEND WAS TREATED AT A HOSPITAL FOR INJURIES TO HER NECK AND RIGHT ARM. THE CONTACT THEN CALLED THE MANUFACTURER WHO INFORMED HIM THAT THERE WAS A RECALL ON THE VEHICLE FOR NHTSA CAMPAIGN NUMBER: 19V627000(AIR BAGS). AN APPOINTMENT WAS MADE AT POHANKA TOYOTA OF SALISBURY(2010 N SALISBURY BLVD, SALISBURY, MD 21801) WHERE AN INSPECTOR WAS SENT TO THE DEALER TO INSPECT THE AIR BAG. AFTER THE INSPECTION, THE CONTACT WAS INFORMED TO PICK UP THE VEHICLE AND WAS ALSO INFORMED THAT NO REPAIRS WOULD BE MADE TO THE VEHICLE UNTIL THE INVESTIGATION WAS SETTLED. THE VEHICLE HAD YET TO BE REPAIRED. THE FAILURE MILEAGE WAS 282,699.
I JUST BOUGHT A 2006 MATRIX (TOYOTA). IT ONLY HAS 88,000 MILES ON IT (OCTOBER, 2020). THE CAR IS IN GREAT CONDITION AND SEEMS TO HAVE BEEN WELL MAINTAINED. THE CHECK ENGINE LIGHT COMES ON AND OFF. I'VE TAKEN IT TO 2 SEPARATE QUALIFIED MECHANICS. THEY CAN'T FIND ANYTHING WRONG. WHEN THE LIGHT IS OFF THE CAR RUNS PERFECT. WHILE I WAS DRIVING TO WORK THE GAS PEDAL WAS ALL THE WAY TO THE FLOOR AND THE CAR WOULD NOT ACCELERATE AT ALL ON A STRAIGHT AWAY FLAT ROAD. I WAS DRIVING 30MPH LEAVING TOWN. THE SPEED LIMIT INCREASED TO 40MPH BEFORE FURTHER INCREASING TO 65MPH HIGHWAY. IT'S A STRAIGHT FLAT ROAD. I COULD NOT GET THE CAR OVER 55MPH. WHEN I GOT TO A SMALL HILL THE CAR FINALLY MADE IT TO THE TOP 45 MPH WITH THE GAS PEDAL ALL THE WAY DEPRESSED. I WAS ONLY ABLE TO GET THE CAR UP TO HIGHWAY SPEED AS I WAS COMING DOWN THE OTHER SIDE OF THE HILL WITH THE GAS PEDAL STILL DEPRESSED COMPLETELY TO THE FLOOR. I USED A HALF OF A TANK OF GAS TO DRIVE 15 MILES. I AM WRITING THIS AS I AM CONCERNED. WHEN I LOOKED ONLINE I FOUND THE PROBLEM I AM DESCRIBING WAS A COMMON PROBLEM AMONGST OTHER MATRIX OWNERS. I COULD ALSO SEE SEVERAL OTHER COMPLAINTS WHERE THE MATRIX ACCELERATED AND WAS UNABLE TO BE STOPPED. WHY HASN'T THE 2006 BEEN RECALLED? WHAT IS THE LIKELIHOOD OF THE MATRIX ACCELERATING OUT OF CONTROL ON ME AFTER THE PEDAL IS FLOORED WITH NO ACCELERATION? THE RISK SEEMS PRETTY HIGH CONSIDERING THE AMOUNT OF COMPLAINTS ONLINE. I AM A MOTHER OF 2. ARE MY KIDS SAFE IN THIS CAR? ARE OTHER DRIVERS ON THE ROAD SAFE FROM THIS CAR?
TL* THE CONTACT OWNS A 2006 TOYOTA COROLLA. THE CONTACT STATED THAT WHILE PARKING THE VEHICLE AT AN ANGLE, WITHOUT THE PARKING BRAKE SET, AFTER EXITING THE VEHICLE TO CHECK HIS MAILBOX, THE VEHICLE ROLLED BACKWARDS. THE CONTACT STATED HE REPLACED THE BUSHING ON HIS OWN. THE VEHICLE WAS NOT DIAGNOSED. A DEALER WAS NOT CONTACTED. THE MANUFACTURER HAD NOT BEEN INFORMED OF FAILURE. THE FAILURE MILEAGE WAS APPROXIMATELY 239,450.
We had recall work done for Airbag, sensor light came on after repairs. One dealer quoted $150 to look at the sensor another wanted $450+. The light was not on before recall work was done. It is clear that whatever they did for the recall caused this to happen, don't care if it's not under warranty the dealer caused the issue electrical whatever and should fix it. Obviously this is a known issue with the 2006 Toyota and should also be part of the recall.
Dealer repaired airbag recall, now SRS light is on and won’t turn off. Dealer now wants hundreds of dollars to check and or fix possible issue told that it may be something else and more work needed. This issue should be part of recall repairs. This car only has 63,200 miles on it well maintained should not have these issues.
I HAVE AN AFTERMARKET WEBASTO HOLLANDIA 700 SUNROOF THAT GOT RECALLED FOR "IN THE EVENT OF A SEPERATION, THE FALLING GLASS COULD CREATE A ROAD HAZARD OR STRIKE A TRAILING VEHICLE AND CAUSE A CRASH." THE GLASS FOR THE MOONROOF FELL APART FROM THE VEHICLE AND IS GONE UNFORTUNATELY. I WAS TRYING TO BUY THE GLASS FROM TOYOTA WITH THE HELP FROM MY AUTO BODY SHOP "KURODA AUTO BODY INC." WHEN THEY WERE BUYING THE OEM GLASS FORM TOYOTA IN CALIFORNIA. THUS THEY CANT FIX THE CAR BECAUSE THEY CANT GET THE GLASS AND OEM DOESNT FIT EXCEPT WEBASTO. IT SAYS THAT WEBASTO WILL BE PROVIDING THE RECALLS BUT THE ONLY PROBLEM IS THAT I LIVE IN HAWAII AND THERE ARE NO WEBASTO DEALERS IN HAWAII, CLOSEST IS IN CALIFORNIA WITH LIKE 8 LOCATIONS, THATS THE CLOSEST FOR US, PAY FOR LIVING IN PARADISE I SUPPOSE. IM OKAY WITH THE PARTS BEING SHIPPED TO ME OR TO MY AUTOBODY SHOP THAT CAN DO IT AND OF COURSE WEBASTO PAYING FOR THE LABOR FEES FOR THE RECALL. IM OPEN TO PAYING FOR SHIPPING COSTS IF THEY AGREE BECAUSE I DONT KNOW IF A RECALL PAYS FOR SHIPPING. I NEED A NEW GLASS, THATS THE ONLY THING MISSING, MECHANISM WORKS AND MOVES BUT ITS CORRODED AND I WOULDNT MIND FOR A WHOLE NEW SET IF POSSIBLE. IN THE PICTURES IT IS BETWEEN THE VIN NUMBER OF THE SUNROOF MODEL UNDER THE RECALL. I THINK IT WAS FIX BEFORE FROM ONE OF THE PICTURES SAYING IT WAS CLAIMED ALREADY FROM MY TOYOTA VIN NUMBER, BUT APPARENTLY IF IT WAS FIX THIS WOULDNT HAVE HAPPENED AGAIN SO ANOTHER RECALL FIX I THINK THIS MAY BE.
I BOUGHT THE CAR NEW IT HAD ALWAYS BEEN DEALER MAINTAINED THE CAR WAS IN EXCELLENT CONDITION THE CAR HAD BEEN SERVICED TWO MONTHS EARLIER A TIRE ROTATION AND OIL CHANGE THE CAR HAS 110K MILES DRIVING THE CAR ON A PAVED ROAD AT 35MPH THE TIE ROD SNAPPED RESULTING IN ACCIDENT THE GARAGE SAID IT LOOKED LIKE THE TIRE LUG NUTS WERE PUT ON TOO TIGHT MY CAR HAS BEEN SITTING AT THE DEALERSHIP FROM WHERE I BOUGHT THE CAR I THOUGHT THEY SHOULD FIX IT BECAUSE I WAS TOLD IT WAS THERE FAULT THE CAR HAS BEEN SITTING THERE FOR A YEAR THE DEALERSHIP WON'T TALK TO ME IS THIS SOMETHING THAT SHOULD HAPPEN TO A WELL MAINTAINED AND WELL CARED FOR CAR A TOYOTA REP LOOKED AT THE CAR AND WOULD TAKE NO RESPONSIBILITY PLEASE TELL ME IF THIS IS NORMAL TO HAPPEN THANK YOU [XXX] INFORMATION Redacted PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6). *CC
DRIVER WAS MAKING LEFT HAND TURN ON CITY STREET WHEN VEHICLE SUDDENLY ACCELERATED AND STEERED TO THE RIGHT CAUSING THE VEHICLE TO CRASH INTO A BLOCK WALL. DRIVER DOES NOT KNOW WHY VEHICLE SUDDENLY ACCELERATED, THERE WAS NO EXTRA INPUT ON THE ACCELERATOR.
I purchased a Toyota three years ago, immediately I complained about the brakes scrubbing and squeaky sounds, the dealer denied anything was wrong with the brakes. After paying the last payment the other day, now the brake pedal is stiff as a board and takes alot of pressure to stop, which is a safety hazzard to all. Also there have been no warning lights come on notification of any problems. I've also noticed Electrical problems like the dash lights going off and on the clock works when it takes a notion.
