Several weeks ago while driving at night the headlights suddenly went out creating a very dangerous situation. When I tried to turn the lights back on they blinked a couple of times and then remained off. Over the next several days the lights would come on sometimes and other times they would not. I changed the light switch but that did not help. I sent the TIPM in to a repair center but they have been unable to identify a problem with it.09
The headlights will go off and stay off unexpectedly while driving at night in the dark. Lights will not go back on for days. Then they will work for a random amount of time, then stop working again while driving in the dark again. You may examine this vehicle if you would like. I think that driving in the dark and the lights unexpectedly going off definitely puts my life and the lives of my family and other drivers at great risk. This problem has been confirmed at my local Firestone auto repair center. Neither the police or the manufacturer have inspected the vehicle yet. The tail lights, the dash lights, everything else works just fine. Neither the headlights or the brights will work. This has been going on for a few months. Why so long? The lights will work for weeks at a time, then unexpectedly shut off and stop working for days. Right now they are not working. I see from doing research on the internet that this seems to be a common problem with these vehicles, and I think that NTSB need to do something about it, like issuing a recall. This is a flawed design!

THE MANUFACTURER WAS MADE AWARE OF THE FAILURE. THE APPROXIMATE FAILURE MILEAGE WAS 146,412. INVALID VIN.
THE HEADLIGHTS STOPPED WORKING. THEY FLICKER WHEN STARTING THE CAR, BUT THEN SHUT OFF. STANDARD AND HIGH-BEAMS ARE BOTH INOPERABLE. TRIED REPLACING THE HEADLIGHT SWITCH, TO NO AVAIL. FOG-LIGHTS WORK, WHICH IS MY ONLY LIGHTING AT NIGHT. CHECKED ONLINE FOR A SOLUTION. APPARENTLY THIS IS A VERY COMMON PROBLEM IN THE TIPM. TRIED A HARD RESET, BUT AGAIN TO NO AVAIL. THERE IS NO FUSE OR RELAY THAT CAN BE REPLACED TO RESOLVE THIS ISSUE. THIS ORIGINALLY BEGAN IN JUNE, 2020, WHILE I WAS DRIVING AT NIGHT THE HEADLIGHTS JUST SHUT OFF. THE HEADLIGHTS FUNCTIONED INTERMITTENTLY FOR A FEW WEEKS, AND THEN FAILED COMPLETELY. THEY STILL FLICKER ONCE WHEN STARTING AND THEN GO OUT.
WHILE DRIVING AT NIGHT HEADLIGHTS FLICKERED AND THEN WENT OUT. AFTER PULLING OVER TO SIT FOR ABOUT 15 MIN, LIGHTS STILL DIDN'T WORK. COULDN'T DRIVE AT ALL. NEXT DAY TRIED LIGHTS DURING DAY TIME AND WORKED. AFTER 24 HOURS LIGHTS STOPPED WORKING AGAIN. NEW BULBS ARE IN PLACE, NO WARNING LIGHTS CAME IN THAT LIGHTS WERE NOT WORKING OR A SHORT WAS NOTED OR AN ELECTRICAL SYSTEM FAILURE. CAR IS NOT THROWING ANY ELECTRICAL CODE FAILURES EITHER.

THE VEHICLE WAS MOVING AT APPROXIMATELY 70 MPH ON INTERSTATE 79 ON A STRAIGHTAWAY NEAR EDINBORO PA. WHEN THE HEADLIGHTS SHUT OFF. THE RUNNING LIGHTS CONTINUED TO FUNCTION. THE LIGHTS HAVE DONE THE SAME THING PRIOR TO THIS INCIDENT AND THE VEHICLE WAS OPERATED TO A SAFE AREA USING THE FOG LIGHTS. THE LIGHTS STARTED WORKING AGAIN ON 3/12/2018 BUT WENT OUT AGAIN THE NEXT DAY.

HEADLIGHTS RANDOMLY WON'T TURN ON, OR WILL FLICKER AND DIE WITH NO WARNING. TOGGLING THE SWITCH DOES NOTHING, I CAN ONLY GET THE FOG LIGHTS TO GIVE ME ANY LIGHT TO SEE BY. INITIAL PROBLEM OCCURRED IN THE DRIVEWAY, CAR WAS STATIONARY. HAVE HAD THEM GO OUT WHILE DRIVING ON CITY STREETS AS WELL.

AFTER SITTING A WHILE THE HEADLIGHTS STARTING WORKING AGAIN THEN WITHOUT WARNING STOPPED WORKING AGAIN (TAIL LIGHTS, FOG LIGHTS ARE NOT AFFECTED). THIS IS THE 3RD TIME THE HEADLIGHT FAILURE HAS OCCURRED. I HAVE READ ON MESSAGE BOARDS THAT THIS HEADLIGHT ISSUE IS A RESULT OF A FAULTY TIPM.
HEADLIGHT STOP WORKING WHEN DRIVING AT NIGHT AND YOU CAN'T GET THEM TO TURN BACK ON. IT HAS HAPPENED 20 OR MORE TIME TO ME. IT HAS HAPPENED ALL READY ON VERY DARK ROADS IN THE COUNTRY, ON THE HIGHWAY OTHER TIMES, I WAS EVEN PULLED OVER FROM A POLICE OFFICER BEFORE TELLING ME TO PUT MY LIGHTS ON AND I SHOWED THEM THEY WERE ON BUT WEREN'T WORKING AGAIN. BUT WERE WORKING WHEN I LEFT MY HOUSE. THEN THE NEXT DAY THEY WILL WORK AGAIN WITH NO PROBLEM AND WORK FOR MONTHS THEN IT DOES IT AGAIN. YOU NEVER KNOW WHEN IT WILL HAPPEN, I'M AFRAID TO TAKE IT OUT WHEN IT'S DARK OUT. BUT IT'S MY ONLY CAR AND SOMETIMES I HAVE NO CHOICE BUT TO GO OUT AT NIGHT AND PRAY IT WON'T HAPPEN AGAIN, IT'S VERY DANGEROUS AND A SAFETY ISSUE, AND I HAVE SEEN A LOT OF COMPLAINTS FOR THE SAME THING WITH OTHER PEOPLE AS WELL SO IT'S NOT JUST MY VAN DOING THIS. SOMETHING NEEDS TO BE DONE BEFORE SOMEONE GET SERIOUSLY HURT OR DIES FROM THIS DEFECT AND THE COMPANY ENDS UP GETTING SUED FOR NOT FIXING THIS PROBLEM. IF IT WAS JUST MY VAN DOING IT THEN I WOULD THINK IT WAS JUST A FLUKE THING, BUT WHEN I HAVE READ IT HAS HAPPENED TO MANY OTHER OWNERS OF THE SAME KIND OF VAN THEN IT IS A DEFECT THAT NEEDS TO BE ADDRESSED AND FIXED BEFORE SOMETHING AWFUL HAPPENS TO SOMEONE. AND FROM WHAT I HAVE READ SOME PEOPLE SAID THEY WERE TOLD IT WAS SOMETHING TO DO WITH THE TIPM WHICH COSTS ABOUT $1200 OR SO TO FIX. SOME PEOPLE SAID THEY FIXED THAT AND IT STILL CONTINUED TO HAPPEN...I DON'T HAVE $1200 TO REPLACE THE TIPM AND THE PROBLEM STILL DOESN'T GET FIXED OR SOLVE THE PROBLEM...RIPOFF HAVING YOUR HEADLIGHTS STOP WORKING ON YOU ON A DARK ROAD AT NIGHT IS NOT SAFE AT ALL AND IS VERY DANGEROUS.
